Mittelnerven-Neuralmobilisierung und Ibuprofen lindern beide Zervikobrachialschmerzen, aber dies geschieht durch unterschiedliche Mechanismen und mit einem unterschiedlichen Auftreten von Nebenwirkungen. Neurale Mobilisation des Nervus medianus ist eine physikalische Therapietechnik, die eine Schmerzlinderung durch mechanische Stimulation des Nervus medianus und des Plexus brachialis erreicht. Die richtige mechanische Stimulation des Nervs und seines umgebenden Nervengewebes induziert eine Vielzahl von physiologischen Reaktionen, die Zervikobrachialschmerzen reduzieren können, einschließlich der Aktivierung von Schmerzmodulationsmechanismen des absteigenden Nervensystems, obwohl die gesamte Reihe der zugrunde liegenden Gründe für diese Schmerzreduktion nicht vollständig ist verstanden. Zu den schmerzlindernden Wirkungen der Neural Tissue Mobilization gehören: Verringerung des intraneuralen Ödems, Veränderungen des intraneuralen Nervendrucks, Verteilung entzündungsfördernder Substanzen und Erhöhung der Nervenmobilität
Ibuprofen ist ein nichtsteroidales entzündungshemmendes Medikament, das weltweit zur Kontrolle von Schmerzen, Fieber und Entzündungen eingesetzt wird. Die orale Einnahme von Ibuprofen erzeugt eine hypoalgetische Wirkung aufgrund der biochemischen Hemmung der COX-Enzyme, die Arachidonsäure in Prostaglandin H2 (PGH2) umwandeln. PGH2, das wiederum von anderen Enzymen in verschiedene Arten von Prostaglandinen und Thromboxanen (die Schmerz- und Entzündungsmediatoren sind) umgewandelt wird.
Nebenwirkungen, die von der oralen Einnahme von Ibuprofen herrühren, hängen mit seiner systemischen Wirkung zusammen und können bei einigen Patienten schwerwiegend sein. Diese Tatsache deutet darauf hin, dass orales Ibuprofen möglicherweise nicht für alle Arten von Patienten geeignet ist, die unter Zervikobrachialschmerzen leiden, während die Median Nerve Neural Mobilization wenig bis gar keine Nebenwirkungen bekannt hat. Dies zeigt einen interessanten Punkt bei der Bestimmung der vergleichenden Wirksamkeit zwischen diesen beiden therapeutischen Optionen bei der Behandlung von Zervikobrachialschmerzen.

Beschreibung
Einschlusskriterien:
- Klinische Diagnose von Zervikobrachialschmerzen, bestätigt durch Magnetresonanztomographie
- Vorhandensein von einseitigen Symptomen von Armschmerzen, Parästhesien oder Taubheitsgefühl in der oberen Extremität während mindestens 3 aufeinanderfolgenden Monaten vor der Anwendung der Behandlung.
- Positive Ergebnisse bei allen folgenden Tests: Spurling, Distraction und Upper Limb
Ausschlusskriterien:
- Kontraindikation bei der Anwendung von nichtsteroidalen Antirheumatika (NSAIDs)
- Die Verwendung jeglicher Art von Behandlung, Therapie, Verfahren oder Medikamenten zur Schmerzlinderung
- Patienten, die unter Antikonvulsiva, Antidepressiva oder Psychopharmaka stehen
- Wirbelinstabilität
- Osteoporose der Wirbel
- Wirbel- oder Wirbelsäuleninfektion.
- Neurologische Erkrankungen genetischen, infektiösen oder neoplastischen Ursprungs
- Zervikale Stenose Myelopathie
- Schwangerschaft
- Kinesiophobie
- Endokrine Störungen und Wechseljahre
- Geschichte der Wirbelsäulenchirurgie
- Geistige Behinderung, schwere Geisteskrankheit, Trunkenheit, schwerer Schlafentzug, Alzheimer-Krankheit.

EXPERIMENTAL: Neurale Mobilisierung des Mediannervs
15-minütige neurale Mobilisationssitzungen des Nervus medianus 5 Mal pro Woche (Montag bis Freitag) während 6 aufeinanderfolgenden Wochen Der maximale Grad der Ellbogenstreckung, der bei der neuralen Mobilisation des Nervus medianus angewendet wird, wurde durch die Verwendung eines universellen Goniometergeräts bestimmt.

Ein nicht-chirurgisches, nicht-invasives Verfahren zur neuralen Mobilisierung des Nervus medianus wurde von einem Physiotherapeuten kontinuierlich während 2 Minuten bei 5 verschiedenen Gelegenheiten mit 1 Minute Pause zwischen jeder 2-minütigen Anwendung der neuralen Mobilisierungstechnik angewendet.
Die Intervention wurde über einen Zeitraum von 6 Wochen durchgeführt.
Der maximale Bewegungsgrad der Ellbogenstreckung ohne Reproduktion von Symptomen während der Anwendung der neuralen Mobilisierungsbehandlung des Nervus medianus wurde durch die Basislinienverwendung eines universellen Goniometergeräts bestimmt.

ACTIVE_COMPARATOR: Ibuprofen
Eine tägliche Einzeldosis von 400 mg oralem Ibuprofen, die je nach Verträglichkeit des Patienten auf eine Höchstdosis von 1200 mg/Tag erhöht werden kann, die gleichmäßig auf 3 orale Einnahme des Arzneimittels alle 8 Stunden während 6 aufeinanderfolgenden Wochen verteilt ist.

Die aktive Vergleichsgruppe erhielt eine orale Ibuprofen-Behandlung.
Die Anfangsdosis betrug 400 mg/Tag und wurde je nach Verträglichkeit des Patienten auf maximal 1200 mg/Tag, aufgeteilt in 3 orale Einnahme alle 8 Stunden, erhöht. Die Intervention wurde über einen Zeitraum von 6 Wochen angewendet.
