TAK-226による輸血依存性貧血に対する有効性および安全性を評価するための、日本における極めて低リスク、低リスク、または中間リスク骨髄異形成症候群患者を対象とした第2相、多施設共同、非盲検、単一腕試験

本研究の主な目的は、輸血依存性貧血を有する低リスク骨髄異形成症候群の日本人患者において、TAK-226が症状をどのように改善するかを評価することです。

本研究は、スクリーニング期間(最長6週間)、治療期間、安全性追跡調査期間(8週間)、および長期追跡調査期間(研究薬初回投与から5年間、または最終投与から3年間のいずれか長い方)で構成されています。

本研究の参加者は、治療期間中にTAK-226を投与されます。その後、安全性追跡調査期間および長期追跡調査期間中に、研究治療に関連する副作用についてモニタリングされます。参加者の参加期間の概算は、最長約6年間です。

包含基準:

  1. 参加者またはその法的に認められた代理人は、ICFに署名し、プロトコル要件を遵守する意思と能力を有すること。
  2. インフォームドコンセント署名時に18歳以上の日本人成人参加者(男性または女性)。
  3. WHO 2016分類に基づき、IPSS-R分類で極めて低リスク、低リスク、または中間リスクのMDSに該当する、RSの有無を問わないMDSの診断(診断を確認するためにスクリーニング時に採取された評価可能な骨髄穿刺液で決定)。

    注記:輸血の影響が予想されるため、赤血球輸血後14日以内に採取された血液サンプルのHgb値および血小板輸血後7日以内に得られた血小板数は、適格性のIPSS-R評価には使用できません。

  4. 登録前16週間を2つの8週間ブロックに分けて評価した輸血依存性で、以下のいずれかに分類されるもの:

    a. LTB:16週間あたり4〜7単位の赤血球輸血と定義。または b. HTB:16週間あたり8単位以上の赤血球輸血と定義。および c. すべての参加者について: i. 輸血前Hgb <10 g/dLの場合のみ、適格性カウントに含まれる。 ii. 各8週間ブロックで少なくとも1回の輸血イベントがあり、登録前16週間以内に少なくとも7日以上間隔を空けた2回以上の輸血イベントがあること。および iii. 登録前16週間以内に、連続する56日間が赤血球輸血なしの期間であってはならない。

    注記:研究対象疾患のための輸血のみが、LTBまたはHTB参加者の分類にカウントされます。併存疾患(出血、外科的処置、感染症など)のための輸血は考慮されません。

  5. 既往のESA治療(登録4週間以上前に中止)に不応性または不耐容性がある、またはESA治療に反応する可能性が低いと定義されるもの、具体的には以下の通り:

    a. 既往のESA治療への不応性:単剤または併用(例:顆粒球コロニー刺激因子[G-CSF]との併用)による既往のESA含有レジメンへの無反応または維持されなくなった反応の記録。ESAレジメンは以下のいずれかでなければならない: i. 組換えヒトEPO 週40,000 IU以上を8回以上または同等量。または ii. ダルベポエチンアルファ 500 mcrgを3週間ごとに4回以上または同等量。 b. 既往のESA治療への不耐容性:単剤または併用(例:G-CSFとの併用)による既往のESA含有レジメンの、導入後の不耐容性または有害事象による中止の記録。 c.   7. ECOG performance statusが0〜2であること。
  8. 妊娠可能な女性(WOCBP):外科的不妊処置を受けていない、または自然閉経後12か月以上経過していない性的に成熟した女性は、

    1. インフォームドコンセント署名時から研究薬最終投与後60日まで、1つの高度有効な避妊法と1つの追加的有効(バリア)法を同時に使用することに同意する。または
    2. 参加者の好ましい通常の生活様式に沿う場合、真の禁欲を実践することに同意する。(周期的禁欲[例:カレンダー法、排卵期法、症状体温法、排卵後法]、体外射精、殺精子剤のみ、授乳期無月経は受容可能な避妊法ではありません。)
  9. 男性参加者は、外科的不妊処置後(すなわち、精管切除術後状態)であっても、

除外基準:

病歴

  1. Del(5q) MDSまたは治療関連(二次性)MDS。
  2. その他の既知の原因による貧血(例:サラセミア、溶血性貧血、出血イベント、または鉄、B12、および/または葉酸欠乏)。
  3. 登録前16週間以内に、基礎となるMDS以外の理由による赤血球輸血の受領。
  4. 臨床的に有意な心血管疾患として定義されるもの:

  17. 登録前8週間以内に開始された鉄キレート療法。8週間以上安定投与中の参加者は許可される。
  18. 登録前4週間以内に開始されたビタミンB12または葉酸療法。4週間以上安定補充投与中で、併存するビタミンB12または葉酸欠乏症がない参加者は許可される。
  19. 登録前8週間以内のアンドロゲン使用。性腺機能低下症に対する8週間以上安定投与中の参加者は許可される。
  20. 登録前4週間以内の高用量コルチコステロイド使用。プレドニゾン/プレドニゾロン≤10 mg/日またはコルチコステロイド同等量を4週間以上安定投与中の参加者は許可される。
  21. スクリーニング前28日以内、または製品の半減期が既知の場合はスクリーニング前の半減期の5倍以内(いずれか長い方)の、いかなる試験薬による治療。
  22. 他の介入臨床試験への継続的参加。検査除外基準(スクリーニング中)
