Estudio de fase 2, multicéntrico, abierto y de un solo brazo para evaluar la eficacia y seguridad de TAK-226 en la anemia dependiente de transfusiones en pacientes japoneses con síndromes mielodisplásicos de riesgo muy bajo, bajo o intermedio

El objetivo principal del estudio es evaluar cómo TAK-226 mejora los síntomas de la anemia dependiente de transfusiones en pacientes japoneses con síndromes mielodisplásicos de menor riesgo.

El estudio consta del Período de Selección (hasta 6 semanas), Período de Tratamiento, Período de Seguimiento de Seguridad (8 semanas) y Período de Seguimiento a Largo Plazo (5 años desde la primera dosis del fármaco del estudio o 3 años después de la última dosis, lo que sea más largo).

Criterios de inclusión:

  1. Los participantes o su representante legal autorizado deben estar dispuestos y ser capaces de firmar el formulario de consentimiento informado (ICF) y de cumplir con los requisitos del protocolo.
  2. Participante adulto japonés de sexo masculino o femenino con edad ≥18 años en el momento de firmar el consentimiento informado.
  3. Diagnóstico de síndrome mielodisplásico (SMD) con o sin sideroblastos en anillo (RS) (según se determine en un aspirado de médula ósea evaluable recogido durante la selección para confirmar el diagnóstico) de acuerdo con la clasificación de la OMS de 2016 que cumpla con la clasificación IPSS-R de SMD de riesgo muy bajo, bajo o intermedio.

    Nota: Debido al impacto esperado de las transfusiones, los valores de hemoglobina (Hgb) de muestras de sangre recogidas dentro de los 14 días posteriores a una transfusión de glóbulos rojos y el recuento de plaquetas obtenido dentro de los 7 días posteriores a una transfusión de plaquetas no pueden utilizarse para evaluar el IPSS-R para la elegibilidad.

  4. Dependencia transfusional evaluada en las 16 semanas inmediatamente anteriores al reclutamiento en dos bloques de 8 semanas clasificados como:

    a. Transfusión baja (LTB), definida como 4 a 7 unidades de glóbulos rojos por 16 semanas; o b. Transfusión alta (HTB), definida como ≥8 unidades de glóbulos rojos por 16 semanas; y c. Para todos los participantes: i. Solo se contabilizan para la elegibilidad los eventos de transfusión con una Hgb pretransfusión <10 g/dL; ii.   5. Refractario o intolerante a un tratamiento previo con agente estimulante de la eritropoyesis (AEE) (interrumpido ≥4 semanas antes del reclutamiento), o con baja probabilidad de respuesta al tratamiento con AEE, definido como sigue:

    a. Refractario a un tratamiento previo con AEE: documentación de falta de respuesta o de una respuesta que ya no se mantenía con un régimen previo que contenía AEE, ya sea como agente único o en combinación (p. ej., con factor estimulante de colonias de granulocitos [G-CSF]); el régimen con AEE debe haber sido: i. Eritropoyetina humana recombinante ≥40.000 UI/semana durante ≥8 dosis o equivalente; o ii. Darbepoetina alfa ≥500 mcrg cada 3 semanas durante ≥4 dosis o equivalente. b. Intolerante a un tratamiento previo con AEE: documentación de la interrupción de un régimen previo que contenía AEE, ya sea como agente único o en combinación (p. ej., con G-CSF), en cualquier momento tras su introducción debido a intolerancia o a un evento adverso (EA).

Antecedentes médicos

  1. SMD con del(5q) o SMD relacionado con tratamiento (secundario).
  2. Anemia debida a cualquier otra causa conocida (p. ej., talasemia, anemia hemolítica, eventos hemorrágicos o deficiencia de hierro, B12 y/o folato).

  4. Enfermedad cardiovascular clínicamente significativa definida como:
