バックグラウンド:
慢性閉塞性肺疾患 (COPD) は、米国における死亡率および罹患率の主な原因であり、1,000 万人近くが罹患しています。 COPD は、年間 60,000 人の死亡者を占めており、死亡原因の第 4 位にランクされています。 現在の傾向が続けば、2000 年までに米国の 4 番目または 3 番目に多い死亡原因になる可能性があります。
疫学的研究は一貫して、喫煙が肺機能の急速な低下とその後の COPD 発症の圧倒的な危険因子であることを示していました。 さらに、いくつかの研究から得られた証拠は、肺機能の低下率が、喫煙をやめるとより正常な低下率に近づいたことを示しています。

肺の健康研究Ⅰ
ランダム化および制御。 気道閉塞の証拠がある喫煙者は、イソプロテレノールとメタコリンに対するスパイロメトリー反応を含むベースラインテストを受け、3つのグループのいずれかに無作為に割り当てられました。介入なしまたは通常のケアグループ。禁煙プログラムと気管支拡張薬療法を受けているグループ。禁煙プログラムとプラセボ気管支拡張薬を受けているグループ。 プラセボ/気管支拡張薬療法は二重盲検でした。 喫煙介入は、行動の修正とニコチンガムの使用を組み合わせた集中的な 12 セッションの禁煙プログラムと、再発を最小限に抑えるための継続的な 5 年間の維持プログラムで構成されていました。 気管支拡張薬は、定量吸入器から 1 日 3 回、1 回に 2 回吸入するように処方された臭化イプラトロピウムで構成されていました。 すべてのグループは、5 年間、毎年追跡されました。 主要評価項目は、FEV1 の変化率でした。 呼吸器罹患率は副次評価項目でした。 募集は 1986 年 11 月に開始され、1989 年 1 月に完了しました。 試験の臨床段階は 1994 年 4 月に終了しました。 この研究は、データ分析と研究結果の普及のために、契約 N01-HR-46002 の下で 2004 年 9 月まで継続されます。
肺の健康研究Ⅲ
1998 会計年度から、LHS I の生存者全員が長期追跡調査に参加するよう招待されました。 この研究では、治療目的分析を使用して、LHS I 喫煙介入が臨床的に重要な呼吸器および心血管疾患の発生率を、研究登録後 12 ~ 15 年間にわたって有意に減少させるかどうかを判断します。 この研究では、喫煙歴を調整した後、心血管および呼吸器の罹患率と死亡率のリスクに対する FEV1 と FVC の影響の大きさも推定します。 性別、気道反応性、体重増加、併存疾患などの他の要因が肺機能の低下率や、心血管系および呼吸器系の罹患率と死亡率のリスクを決定する上での役割について研究が行われます。 また、禁煙に伴う肺機能の改善と呼吸器症状の軽減が、健康関連の生活の質(HRQL)の改善と抑うつの軽減につながるかどうかについても、追跡期間を延長して判断します。 研究者が開始した長期追跡研究である LHS III は、NIH が定義した臨床試験ではありません。
