Lungehelsestudie (LHS) I og III
I Lung Health Study I, for å bestemme effekten av spesialomsorg, sammenlignet med vanlig pleie, på frekvensen av nedgang i lungefunksjonen hos en gruppe sigarettrøkere som er identifisert som å ha milde abnormiteter i lungefunksjonen.

BAKGRUNN:
Kronisk obstruktiv lungesykdom (KOLS) er en viktig årsak til dødelighet og sykelighet i USA, og påvirker nesten 10 millioner mennesker. KOLS står for 60 000 dødsfall årlig og rangerer som den fjerde største dødsårsaken. Hvis dagens trender fortsetter, kan det bli landets fjerde eller til og med tredje største dødsårsak innen år 2000.
Epidemiologiske studier indikerte konsekvent at røyking var den overveldende risikofaktoren for akselerert reduksjon i lungefunksjon og påfølgende utvikling av KOLS. Videre indikerte bevis fra flere studier at frekvensen av reduksjon i lungefunksjon nærmet seg en mer normal frekvens av reduksjon ved opphør av sigarettrøyking.
En annen antatt risikofaktor for akselerert reduksjon i lungefunksjon var tilstedeværelsen av hyperreaktive luftveier, selv om det ikke var klart om bare tilstedeværelsen av hyperreaktive luftveier bidro til den akselererte reduksjonen, eller om reduksjonen var et resultat av reaksjonen i luftveiene til ulike irriterende stoffer over en lang periode. Det er mulig at hvis den hyperreaktive luftveien ble holdt ikke-reaktiv med farmakologiske midler over en periode på år, kan den forventede unormale nedgangen reduseres. Denne effekten kan forsterkes ved å slutte med sigarettrøyking.
Selv om bevisene var sterke på at røyking og hyperreaktive luftveier var risikofaktorer for KOLS, var det ikke påvist om fjerning av risikofaktorer på et stadium da mild funksjonssvikt allerede hadde oppstått, effektivt ville endre forløpet av KOLS.
DESIGN NARRATIV:
Lungehelsestudie I
Randomisert og kontrollert. Sigarettrøykere med tegn på luftveisobstruksjon gjennomgikk baseline-testing som inkluderte spirometriske responser på isoproterenol og metakolin og ble deretter tilfeldig fordelt i en av tre grupper: en gruppe uten intervensjon eller vanlig omsorg; en gruppe som mottar et røykesluttprogram og bronkodilatatorterapi; en gruppe som får et røykesluttprogram og en placebo bronkodilatator. Placebo/bronkodilatatorbehandlingen var dobbeltblind. Røykeintervensjonen besto av et intensivt 12-sesjoner røykesluttprogram som kombinerte atferdsendring og bruk av nikotintyggegummi, med et fortsatt femårig vedlikeholdsprogram for å minimere tilbakefall. Bronkodilatatoren besto av ipratropiumbromid foreskrevet tre ganger daglig, to drag per gang, fra en inhalator med avmålt dose. Alle gruppene ble fulgt årlig i fem år. Det primære endepunktet var endringshastigheten for FEV1. Respiratorisk sykelighet var et sekundært endepunkt. Rekrutteringen begynte i november 1986 og ble fullført i januar 1989. Den kliniske fasen av forsøket ble avsluttet i april 1994. Studien fortsetter under kontrakt N01-HR-46002 til og med september 2004 for dataanalyse og formidling av forskningsresultater.
Lungehelsestudie III
Fra og med regnskapsåret 1998 inviteres alle overlevende deltakere av LHS I til å delta i den langsiktige oppfølgingen. Studien vil avgjøre, ved hjelp av en intent-to-treat-analyse, om LHS I-røykeintervensjonen signifikant reduserer forekomsten av klinisk viktige respiratoriske og kardiovaskulære sykdommer over en 12- til 15-årsperiode etter studieregistrering. Studien vil også estimere omfanget av effektene av FEV1 og FVC på risikoen for kardiovaskulær og respiratorisk sykelighet og dødelighet, etter å ha kontrollert for røykehistorie. Studier vil bli utført på rollen til andre faktorer som kjønn, luftveisreaktivitet, vektøkning og komorbiditeter i å bestemme graden av reduksjon i lungefunksjon og risikoen for kardiovaskulær og respiratorisk sykelighet og dødelighet. Det vil også bli tatt stilling til om bedring i lungefunksjon og reduksjon av luftveissymptomer knyttet til røykeslutt gir økt helserelatert livskvalitet (HRQL) og mindre depresjon over en lengre oppfølgingsperiode. LHS III, en etterforsker initiert langtidsoppfølgingsstudie, er ikke en NIH-definert klinisk studie.
