Lungesundhedsundersøgelse (LHS) I og III
I Lung Health Study I, for at bestemme virkningerne af Special Care, sammenlignet med Usual Care, på hastigheden af fald i lungefunktionen hos en gruppe cigaretrygere, der er identificeret som havende milde abnormiteter i lungefunktionen.

BAGGRUND:
Kronisk obstruktiv lungesygdom (KOL) er en væsentlig årsag til dødelighed og sygelighed i USA, der påvirker næsten 10 millioner mennesker. KOL tegner sig for 60.000 dødsfald årligt og rangerer som den fjerde hyppigste dødsårsag. Hvis de nuværende tendenser fortsætter, kan det blive landets fjerde eller endda tredje hyppigste dødsårsag i år 2000.
Epidemiologiske undersøgelser indikerede konsekvent, at rygning var den overvældende risikofaktor for accelereret fald i lungefunktionen og efterfølgende udvikling af KOL. Ydermere viste beviser fra flere undersøgelser, at hastigheden af fald i lungefunktionen nærmede sig en mere normal hastighed af fald ved ophør med cigaretrygning.
En anden formodet risikofaktor for accelereret fald i lungefunktionen var tilstedeværelsen af hyperreaktive luftveje, selvom det ikke var klart, om blot tilstedeværelsen af hyperreaktive luftveje bidrog til det accelererede fald, eller om faldet skyldtes reaktionen i luftvejene på forskellige irriterende stoffer over en lang periode. Det er muligt, at hvis de hyperreaktive luftveje blev holdt ikke-reaktive med farmakologiske midler over en periode på år, kunne det forventede unormale fald blive mindsket. Denne effekt kan forstærkes af ophør med cigaretrygning.
Selvom evidensen var stærk for, at rygning og hyperreaktive luftveje var risikofaktorer for KOL, var det ikke blevet påvist, om fjernelse af risikofaktorer på et stadium, hvor mild dysfunktion allerede var opstået, effektivt ville ændre forløbet af KOL.

Lungesundhedsundersøgelse I
Randomiseret og kontrolleret. Cigaretrygere med tegn på luftvejsobstruktion gennemgik baseline-test, der omfattede spirometriske responser på isoproterenol og methacholin og blev derefter tilfældigt tildelt en af tre grupper: en gruppe uden intervention eller sædvanlig pleje; en gruppe, der modtager et rygestopprogram og bronkodilatatorbehandling; en gruppe, der modtager et rygestopprogram og en placebo bronkodilatator. Placebo/bronkodilatatorbehandlingen var dobbeltblind. Rygeinterventionen bestod af et intensivt 12-sessions rygestopprogram, der kombinerede adfærdsændring og brug af nikotintyggegummi med et fortsat femårigt vedligeholdelsesprogram for at minimere tilbagefald. Bronkodilatatoren bestod af ipratropiumbromid ordineret tre gange dagligt, to pust pr. gang, fra en inhalator med afmålt dosis. Alle grupper blev fulgt årligt i fem år. Det primære endepunkt var ændringshastigheden af FEV1. Respiratorisk morbiditet var et sekundært endepunkt. Rekruttering begyndte i november 1986 og blev afsluttet i januar 1989. Den kliniske fase af forsøget sluttede i april 1994. Undersøgelsen fortsætter under kontrakt N01-HR-46002 til og med september 2004 for dataanalyse og formidling af forskningsresultater.
Lungesundhedsundersøgelse III
Fra og med regnskabsåret 1998 inviteres alle overlevende deltagere af LHS I til at deltage i den langsigtede opfølgning. Undersøgelsen vil afgøre, ved hjælp af en intention-to-treat-analyse, om LHS I-rygeinterventionen signifikant reducerer forekomsten af klinisk vigtige respiratoriske og kardiovaskulære sygdomme over en 12- til 15-årig periode efter tilmelding til studiet. Undersøgelsen vil også estimere størrelsen af virkningerne af FEV1 og FVC på risikoen for kardiovaskulær og respiratorisk morbiditet og dødelighed efter kontrol for rygehistorie. Undersøgelser vil blive udført af andre faktorers rolle såsom køn, luftvejsreaktivitet, vægtøgning og følgesygdomme i bestemmelsen af hastigheden af fald i lungefunktion og risikoen for kardiovaskulær og respiratorisk morbiditet og dødelighed. Det vil også blive taget stilling til, om forbedringen af lungefunktionen og reduktionen af luftvejssymptomer forbundet med rygestop resulterer i forbedret helbredsrelateret livskvalitet (HRQL) og mindre depression over en længere opfølgningsperiode. LHS III, et langtidsopfølgningsstudie, der er iværksat af investigator, er ikke et NIH-defineret klinisk forsøg.
