PSC-833 (NSC# 648265) による MDR 調節の第 I 相試験と、未治療の AML 患者における RIL-2 (NSC # 373364) による免疫療法の第 II 相パイロット試験に続く、細胞遺伝学的リスクに適応した地固めのパイロット試験< 60年
理論的根拠: 化学療法で使用される薬剤は、がん細胞の分裂を止めるさまざまな方法を使用して、がん細胞の増殖を停止または死滅させます。 一部のがんは、化学療法薬に耐性になる場合があります。 PSC 833 と化学療法を併用すると、薬剤に対する耐性が低下し、がん細胞を死滅させることができます。 インターロイキン-2 は、人の白血球を刺激して白血病細胞を殺す可能性があります。

詳細な説明
目的: I. エトポシド、シタラビン、および PSC 833 (ADEP) と組み合わせて使用した場合、およびエトポシドおよびシタラビン (ADE) と組み合わせて使用した場合の、ダウノルビシンの最大耐用量 (MTD) を決定します。 60年未満。 Ⅱ. これらの患者で一定用量のダウノルビシンおよびシタラビン (ADE) と組み合わせて使用した場合のエトポシドの MTD を決定します。 III. 細胞遺伝学的所見が良好な患者は高用量シタラビン(HiDAC)による 3 回の強化を受け、平均から低リスクの患者は HiDAC/エトポシド/フィルグラスチム(G-CSF)を受けるように、リスクに応じた方法で寛解後療法を実施することの実現可能性と毒性効果を判断します。 )地固め療法と幹細胞の動員、その後の準備療法としてブスルファン/エトポシドを使用した末梢幹細胞(PBSC)移植。 IV. 概要: これは導入療法部分におけるダウノルビシンの用量漸増研究であり、同じ部分におけるエトポシドの別の用量漸増研究がある。 患者は、導入療法、強化療法、寛解後療法の 3 段階の治療で治療されます。 導入療法: 患者は、1 ~ 7 日目にシタラビン IV の持続注入に加えて、1 ~ 3 日目にダウノルビシン IV を 30 分かけて、エトポシド IV を 2 時間かけて投与されます (ADE レジメン)。 一部の患者は、1~3日目に持続注入としてPSC 833 IVも投与されます(ADEPレジメン)。 このコースは 14 日後に繰り返すことができます。 9 人の患者のコホートは、最大耐用量 (MTD) に達するまで、漸増用量のダウノルビシンをそれぞれ受け取ります。 MTD は、9 人の患者のうち 3 人が用量制限毒性を経験する用量として定義されます。 エスカレーションは、ADE レジメンと ADEP レジメンで別々に実施されます。 他の 9 人の患者のコホートはそれぞれ、ADE レジメンで一定用量のダウノルビシンとともに漸増用量のエトポシドを受けます。 MTDも同様に記述される。 強化療法: アーム I (白血病細胞に特定の遺伝的特徴を持つ患者): 患者は、シタラビン IV を 3 時間かけて 1 日 2 回、3 日間追加で 3 コース受けます。 コースは28日ごとに繰り返されます。 アーム II (これらの遺伝的特徴を持たない患者): 患者は末梢血幹細胞 (PBSC) 移植を受けます。 患者はまず、1~4 日目に高用量のシタラビン IV を 2 時間かけて投与し、1~4 日目にエトポシド IV を持続注入し、5 日目から血球数が回復するまでフィルグラスチム (G-CSF) を皮下投与します。 PBSC が収集されます。 約 4 ~ 6 週間後、患者は 1 ~ 4 日目に 1 日 4 回ブスルファンを経口投与され、5 日目にはエトポシド IV を 4 時間かけて投与されます。7 日目に PBSC が再注入されます。カウント回復。 アーム III (PBSC 移植を受けることができない患者): 患者はアーム II のようにシタラビン、エトポシド、および G-CSF を受け取り、次にアーム I のように高用量のシタラビンを受け取ります。 寛解後療法 (すべての患者): 患者は低用量のインターロイキン 2 を受け取ります(IL-2) を 2 週間毎日注射します。 15 日目に、患者は週 3 日断続的な高用量 IL-2 の投与を開始します。 患者は、IL-2 のこれらのコースを交互に行います: 14 日間の低用量 IL-2、3 日間の高用量 IL-2、1 日間の休息、低用量 IL-2 を 10 日間、その後 3 日間の高用量 IL-2 、その後1日休む。 このコースは3回繰り返されます。 その後、患者は低用量の IL-2 をさらに 16 日間投与されます。 患者は 1 か月ごとに追跡され、その後 3 か月ごとに 2 年間、その後 6 か月ごとに 2 年間、その後は 1 年ごとに追跡されます。
