Combinatiechemotherapie plus PSC 833 gevolgd door interleukine-2 bij de behandeling van patiënten met acute myeloïde leukemie
Fase I-studie van MDR-modulatie met PSC-833 (NSC# 648265) met een pilotstudie van cytogenetische risico-aangepaste consolidatie gevolgd door een fase II-pilotstudie van immunotherapie met RIL-2 (NSC # 373364) bij niet eerder behandelde patiënten met AML< 60 jaar
RATIONALE: Geneesmiddelen die bij chemotherapie worden gebruikt, gebruiken verschillende manieren om te voorkomen dat kankercellen zich delen, zodat ze stoppen met groeien of sterven. Sommige vormen van kanker kunnen resistent worden tegen geneesmiddelen voor chemotherapie. Het combineren van PSC 833 met chemotherapie kan de resistentie tegen de medicijnen verminderen en ervoor zorgen dat de kankercellen worden gedood. Interleukine-2 kan de witte bloedcellen van een persoon stimuleren om leukemiecellen te doden.
DOEL: Fase I/II-studie om de effectiviteit te bestuderen van combinatiechemotherapie plus PSC 833 gevolgd door aanvullende chemotherapie of perifere stamceltransplantatie en interleukine-2 bij de behandeling van patiënten met onbehandelde acute myeloïde leukemie.

DOELSTELLINGEN: I. Bepaal de maximaal getolereerde dosis (MTD) van daunorubicine bij gebruik in combinatie met etoposide, cytarabine en PSC 833 (ADEP), en in combinatie met etoposide en cytarabine (ADE) bij niet eerder behandelde patiënten met acute myeloïde leukemie die minder dan 60 jaar. II. Bepaal de MTD van etoposide bij gebruik in combinatie met een constante dosis daunorubicine en cytarabine (ADE) bij deze patiënten. III. Bepaal de haalbaarheid en toxische effecten van het toedienen van postremissietherapie op een risico-aangepaste manier, zodat patiënten met gunstige cytogenetische bevindingen drie intensiveringen krijgen met hoge dosis cytarabine (HiDAC), terwijl patiënten met gemiddeld tot slecht risico HiDAC/etoposide/filgrastim (G-CSF) krijgen ) voor consolidatietherapie en stamcelmobilisatie gevolgd door perifere stamceltransplantatie (PBSC) met busulfan/etoposide als voorbereidend regime. IV. Bepaal de haalbaarheid en toxische effecten van de consolidatiesequentie van HiDAC/etoposide/G-CSF gevolgd door 2 kuren met HiDAC bij patiënten die anders een PBSC-transplantatie zouden krijgen, maar hier om logistieke of institutionele redenen niet toe in staat zijn. V. Bepaal de haalbaarheid van intermitterende toediening van hoge dosis subcutaan interleukine-2 (IL-2) in combinatie met continue lage dosis subcutaan IL-2 bij patiënten die herstellen van PBSC-transplantatie of intensieve consolidatiechemotherapie.
OVERZICHT: Dit is een dosisescalatieonderzoek van daunorubicine in het inductietherapiegedeelte, met een afzonderlijk dosisescalatieonderzoek van etoposide in hetzelfde gedeelte. Patiënten worden behandeld met drie behandelingsfasen: inductie, intensivering en postremissietherapie. Inductietherapie: Patiënten krijgen cytarabine IV als een continu infuus op dag 1-7 plus daunorubicine IV gedurende 30 minuten en etoposide IV gedurende 2 uur op dag 1-3 (ADE-regime). Sommige patiënten krijgen ook PSC 833 IV als een continu infuus op dag 1-3 (ADEP-regime). Deze kuur kan 14 dagen later herhaald worden. Cohorten van 9 patiënten krijgen elk toenemende doses daunorubicine totdat de maximaal getolereerde dosis (MTD) is bereikt. De MTD is gedefinieerd als de dosis waarbij 3 van de 9 patiënten dosisbeperkende toxiciteit ervaren. Escalaties worden afzonderlijk uitgevoerd voor de ADE- en ADEP-regimes. Andere cohorten van 9 patiënten kregen elk stijgende doses etoposide met constante doses daunorubicine in het ADE-regime. De MTD wordt op dezelfde manier beschreven. Intensiveringstherapie: Arm I (patiënten met bepaalde genetische kenmerken in hun leukemiecellen): Patiënten krijgen 3 aanvullende kuren cytarabine IV gedurende 3 uur, tweemaal daags, gedurende 3 dagen. De cursussen worden elke 28 dagen herhaald. Arm II (patiënten die deze genetische kenmerken niet hebben): Patiënten ondergaan een perifere bloedstamceltransplantatie (PBSC). Patiënten krijgen eerst een hoge dosis cytarabine IV gedurende 2 uur op dag 1-4, etoposide IV als een continu infuus op dag 1-4, en filgrastim (G-CSF) subcutaan vanaf dag 5 totdat het aantal bloedcellen zich herstelt. PBSC worden vervolgens verzameld. Ongeveer 4-6 weken later krijgen patiënten 4 keer per dag oraal busulfan op dag 1-4 en etoposide IV gedurende 4 uur op dag 5. PBSC wordt opnieuw toegediend op dag 7. G-CSF wordt subcutaan toegediend vanaf dag 7 totdat de tellingen herstellen. Arm III (patiënten die geen PBSC-transplantatie kunnen ondergaan): Patiënten krijgen cytarabine, etoposide en G-CSF zoals in arm II, daarna een hoge dosis cytarabine zoals in arm I. Postremissietherapie (alle patiënten): Patiënten krijgen een lage dosis interleukine-2 (IL-2) door dagelijkse injectie gedurende 2 weken. Op dag 15 beginnen patiënten drie dagen per week met tussenpozen een hoge dosis IL-2 te krijgen. Patiënten wisselen deze kuren van IL-2 af: 14 dagen lage dosis IL-2, 3 dagen hoge dosis IL-2, 1 dag rust, lage dosis IL-2 gedurende 10 dagen, daarna 3 dagen hoge dosis IL-2 , daarna 1 rustdag. Deze cursus wordt 3 keer herhaald. Patiënten krijgen dan nog een kuur van 16 dagen met een lage dosis IL-2. Patiënten worden na 1 maand gevolgd, daarna elke 3 maanden gedurende 2 jaar, vervolgens elke 6 maanden gedurende 2 jaar en daarna jaarlijks.
VERWACHTE ACCRUAL: Ongeveer 410 patiënten zullen binnen 36 maanden in deze studie worden opgenomen.
