この研究の目的は、レニン-アンジオテンシン系の構成要素のゲノム変異と肝移植後のカルシニューリン阻害剤による腎障害の発症との関係を明らかにすることです。また、研究者は肝移植後の慢性腎不全との関係も評価します。そして死の危険性。 血液と尿のサンプルは、肝移植を受けた後に誰が慢性腎不全を発症するかを説明できる人々間の遺伝子の違いを決定するために、患者の遺伝子がどのように配置されているかを調べるために検査されます。

研究デザイン:
この研究は、肝移植を受け、少なくとも6か月の追跡調査を受けた肝移植レシピエントを対象とした横断的調査となります。 この研究はノースウェスタン記念病院、ノースウェスタン医学部財団、ノースウェスタン大学で実施されます。
肝移植を受け、移植後の追跡期間全体にわたって維持免疫抑制(カルシニューリン阻害剤(CI)、シクロスポリン(CsA)、またはタクロリムス(Tac)からなる)を受けている少なくとも6か月の追跡調査を受けた患者も分析に含まれます。
各患者について、次の情報が収集されます。
- 移植日、年齢、性別、人種、肝不全の原因、高血圧、糖尿病の発生率を含む過去の病歴、術後合併症、術後感染症、急性肝拒絶反応エピソードの回数、死亡、CsAおよびTacトラフレベル移植後のさまざまな時点および肝臓移植前、移植後1、6、12、24か月、および毎年のsCrレベルでのsCrレベル。
- 肝臓移植前、1、6、12、24 か月、および移植後毎年の GFR (糸球体濾過率) と MDRD 式。
- 肝移植前に既存の CRF を有するすべての患者を除外する試みでは、sCr<1.0mg/dL の患者のみが除外されました。 肝移植前も分析に含まれます。
ジェノタイピング:
- DNAは肝臓移植レシピエントから血液バフィーコートを使用して抽出されます。
- ACE 遺伝子型の決定は、ポリメラーゼ連鎖反応配列特異的プライマー (PCR-SSP) およびポリメラーゼ連鎖反応制限断片長多型 (PCR-RFLP) を使用して実行されます。
- MDRD 方程式による GRF 計算に基づいて、肝移植患者は慢性腎臓病の NKF 病期分類に基づいて 3 つのグループに分類されます。

- 1回の来院で20cc採血します。

統計計画 - 実行する統計テスト:
肝移植患者の遺伝子型が特定され、尿サンプルが収集されたら、尿中バイオマーカーおよび ACE 遺伝子の多型との関連性について肝移植後の CKD の発症を比較します。 2 つのサンプル t 検定を使用して、CKD の程度が異なる肝臓患者間の連続変数の差を比較します。カイ二乗検定は、離散変数の差を比較するために使用されます。 肝移植後の腎機能障害の発症に大きく関連する臨床変数と遺伝子多型の組み合わせを特定するために、多変量ロジスティック回帰分析も実行されます。
この研究により、CIへの曝露後に腎機能障害を発症する潜在的な遺伝的素因を学び、評価することが可能になります。 私たちの仮説が正しければ、RAS 遺伝子型の構成要素の検査は、CI 腎毒性のリスクがある患者を特定するのに役立ち、一次免疫抑制として CI の使用を避けるべき肝移植候補者の事前選択に役立つ可能性があります。
