体重再犯に対する腹腔鏡下胃バイパス再手術: 170 人の患者における私たちの経験
この研究の主な目的は、体重再犯に対する腹腔鏡下再手術ルーアンワイ胃バイパス手術後の従来の患者中心の複合中間転帰を分析し報告することである。
調査の概要
詳細な説明
初回および再肥満手術の標準化が欠如しており、長期転帰データが乏しいことがさらに悪化しています。
不十分な体重減少、体重の再犯、および一次肥満手術後の最も重篤な技術的合併症の治療は、依然として手術以外の治療には難治性である。
手術後2~3年での失敗率は、病的肥満(MO)と超肥満(SO)でそれぞれ最大20%と35%であると報告されています。
さらなる外科的介入の適応については、手術と内視鏡の両方でどのような種類の再手術を推奨するかについても議論の余地が残っています。
さらに、四肢の長さ、ポーチのサイズ、補綴補強の使用についても標準化されていません。
したがって、これらの患者へのアプローチは、元の手術と同じように個別化する必要があります。

説明
包含基準:
- 体重再犯または体重回復の「WR」パターンを伴う失敗した「RYGB」に対して腹腔鏡下再手術「RYGB」を受けた患者
除外基準:
- 不十分な初期減量「IIWL」パターンで失敗したRYGB
- 過去に大規模な肥満症への転換または食道・胃の手術を受けた患者
- 観血的外科的アプローチによって修正されたRYGB患者
- 他の場所で腹腔鏡下RYGB修正手術を受け、当プログラムでフォローアップケアを継続しているRYGB患者
- 記録が欠落している、および/または分析のための情報が乏しく連絡が取れない患者
