SPRINT は、SBP 130 mmHg 以上で少なくとも 1 つの追加の CVD 危険因子を持つ 50 歳以上の約 9,250 人の参加者を登録するよう努めました。 この試験では、集中的なSBP目標の治療プログラムに対するランダム化の効果と、標準目標の治療プログラムに対するランダム化の効果を比較しました。 目標SBP目標はそれぞれ<120 mm Hg対<140 mm Hgで、2つのランダム化グループ間に最小平均差10 mm Hgが生じました。 主な仮説は、集中治療群では CVD イベント発生率が低いであろうということでした。 参加者は、約 2 年間にわたって 5 つの臨床センター ネットワーク (CCN) 内の約 90 の診療所で募集され、4 ~ 6 年間追跡調査されました。
合計9,361名の参加者が登録しました。 NIH は、重要な予備結果を迅速に広めるため、当初の計画よりも早く血圧介入を中止しました。 介入後の認知および腎臓の転帰の追跡調査は、2018 年 5 月まで継続されます。

- 最大 4 つの薬剤で 130 ~ 150 mm Hg
リスク (次の 1 つ以上)
- 脳卒中以外の臨床的または無症状の心血管疾患の存在
- CKD、eGFR 20 ~ 59 ml/min/1.73m2 として定義
- 10 年間の CVD リスクのフラミンガム リスク スコア ≥ 15%
- 75歳以上の年齢
除外基準:
- 特定の血圧降下薬の適応があり、その人が服用していないが、その薬クラスに不耐症であることが証明されていない。
- プロトコールの安全性に関して懸念を引き起こす高血圧の既知の二次的原因。
- SBP < 110 mm Hg で 1 分間放置。
以下の範囲の蛋白尿(過去6か月以内の測定に基づく)
- 24 時間の尿中タンパク質排泄量 ≥1 g/日、または
- 24 時間の尿中アルブミン排泄量 ≥ 600 mg/日、または
- スポット尿タンパク質/クレアチニン比 ≥ 1 g/g クレアチニン、または
- スポット尿アルブミン/クレアチニン比 ≥ 600 mg/g クレアチニン、または
- 尿計量棒 ≥ 2+ タンパク質
- 腕の周囲が大きすぎるか小さすぎると、利用可能なデバイスで正確な血圧測定ができなくなります
- 糖尿病、
- 脳卒中の病歴(CEまたはステント留置ではない)
- 多発性嚢胞腎の診断
- 免疫抑制療法で治療されている、または免疫抑制療法で治療される可能性がある糸球体腎炎
- eGFR < 20 ml/分 /1.73m2 または末期腎疾患 (ESRD)
- 過去3か月以内の心血管イベントまたは心血管疾患処置(上記で研究参加のための臨床CVDとして定義)または不安定狭心症による入院
- 過去6か月以内に症候性心不全、または左心室駆出率(任意の方法による)<35%
- 生存期間が 3 年未満に制限される可能性がある病状、または過去 2 年以内に非黒色腫皮膚がん以外の悪性腫瘍が発生した
- 臨床チームが介入の順守を制限する可能性が高いと判断した要因。
- 参加者からインフォームドコンセントを取得できない場合
- 現在別の臨床試験(介入研究)に参加中。 注: 患者は、SPRINT のスクリーニングを受ける前に、自分の活動が完了するか、他の治験が完了するまで待つ必要があります。
- すでにランダム化された SPRINT 参加者と同じ世帯に住んでいる
- あらゆる臓器移植
- 過去 6 か月間に 10% を超える意図しない体重減少

実験的:SBPの集中制御
集中血圧群にランダムに割り当てられた参加者は、SBP <120 mm Hg を目標とします。集中治療参加者のほとんどで 2 剤併用療法が開始されました。年齢が75歳以上で、薬剤0~1錠でSBPが130~139mmHgである。最初は 1 種類の薬剤で開始できますが、SBP ≧ 130 mm Hg の場合は 1 か月後に 2 番目の薬剤を追加します。 SBP <120 mm Hgを達成するために、各訪問(毎月)で薬剤を追加および/または漸増する。定期的な「マイルポスト」訪問時: 目標に達していない場合は、別の薬剤の追加が「必要」です。

|
全原因認知症の発生事例を確認するには、3 段階のプロセスが使用されました。
まず、認知症の可能性のあるケースを特定するために、参加者全員に簡単な認知スクリーニングバッテリーが実施されました。
追跡調査中に認知障害の可能性について事前に指定されたスクリーニングカットポイントを下回るスコアを獲得した参加者には、より包括的かつ詳細な神経認知検査バッテリー(拡張認知評価バッテリー)と、毎日の機能障害を評価する機能評価アンケート(FAQ)が実施されました。認知障害の結果として生じる生活スキル。
最後に、上記の利用可能な検査とアンケート データはすべて、認知症専門家委員会による裁定のため、Web ベースの一元管理システムに送信され、認知症の可能性 (PD)、軽度認知障害 (MCI)、または障害なしの最終研究分類が割り当てられました (に)。
