SPRINT stræbte efter at tilmelde omkring 9250 deltagere i alderen ≥ 50 år med SBP ≥130 mm Hg og mindst én yderligere CVD-risikofaktor. Forsøget sammenlignede virkningerne af randomisering med et behandlingsprogram med et intensivt SBP-mål med randomisering med et behandlingsprogram med et standardmål. Mål for SBP var henholdsvis <120 vs <140 mm Hg for at skabe en minimumsmiddelforskel på 10 mm Hg mellem de to randomiserede grupper. Den primære hypotese var, at CVD-hændelsesraterne ville være lavere i den intensive arm. Deltagerne blev rekrutteret på cirka 90 klinikker inden for 5 kliniske center-netværk (CCN'er) over cirka en 2-årig periode og blev fulgt i 4-6 år.
I alt var 9361 deltagere tilmeldt. NIH stoppede blodtryksinterventionen tidligere end oprindeligt planlagt for hurtigt at formidle de væsentlige foreløbige resultater. Opfølgning for kognitive og nyreresultater fortsætter i post-interventionsfasen til maj 2018.

Inklusionskriterier:
- Mindst 50 år gammel
Systolisk blodtryk af
- 130 - 180 mm Hg på 0 eller 1 medicin
- 130 - 170 mm Hg på op til 2 medikamenter
- 130 - 160 mm Hg på op til 3 medikamenter
- 130 - 150 mm Hg på op til 4 medicin
Risiko (en eller flere af følgende)
- Tilstedeværelse af klinisk eller subklinisk kardiovaskulær sygdom bortset fra slagtilfælde
- CKD, defineret som eGFR 20 - 59 ml/min/1,73m2
- En Framingham-risikoscore for 10-års CVD-risiko ≥ 15 %
- Alder over 75 år
Ekskluderingskriterier:
- En indikation for en bestemt blodtrykssænkende medicin, som personen ikke tager, og personen ikke er dokumenteret intolerant over for medicinklassen.
- Kendt sekundær årsag til hypertension, der giver anledning til bekymring vedrørende protokollens sikkerhed.
- Et minuts stående SBP < 110 mm Hg.
Proteinuri i følgende områder (baseret på en måling inden for de seneste 6 måneder)
- 24 timers urinproteinudskillelse ≥1 g/dag, eller
- 24 timers urinalbuminudskillelse ≥ 600 mg/dag, eller
- pleturinprotein/kreatinin-forhold ≥ 1 g/g kreatinin, eller
- pleturin albumin/kreatinin-forhold ≥ 600 mg/g kreatinin, eller
- urinpind ≥ 2+ protein
- Armomkredsen er for stor eller lille til at tillade nøjagtig blodtryksmåling med tilgængelige enheder
- Diabetes mellitus,
- Anamnese med slagtilfælde (ikke CE eller stenting)
- Diagnose af polycystisk nyresygdom
- Glomerulonefritis behandlet med eller sandsynligvis vil blive behandlet med immunsuppressiv terapi
- eGFR < 20 ml/min /1,73 m2 eller nyresygdom i slutstadiet (ESRD)
- Kardiovaskulær hændelse eller procedure (som defineret ovenfor som klinisk hjerte-kar-sygdomme til undersøgelse) eller hospitalsindlæggelse for ustabil angina inden for de sidste 3 måneder
- Symptomatisk hjertesvigt inden for de seneste 6 måneder eller venstre ventrikel ejektionsfraktion (af enhver metode) < 35 %
- En medicinsk tilstand, der sandsynligvis begrænser overlevelsen til mindre end 3 år eller en anden malignitet end ikke-melanom hudkræft inden for de sidste 2 år

Eksperimentel: Intensiv kontrol af SBP
Deltagere randomiseret i den intensive BP-arm vil have et mål om SBP <120 mm Hg. 2-lægemiddelbehandling påbegyndt hos de fleste intensive deltagere; alder ≥75 år og SBP 130-139 mm Hg på 0-1 lægemiddel; kan begynde med 1 lægemiddel, men tilføje andet efter 1 måned, hvis SBP ≥130 mm Hg; lægemidler tilføjet og/eller titreret ved hvert besøg (månedligt) for at opnå SBP <120 mm Hg; ved periodiske "milepost"-besøg: tilføjelse af et andet lægemiddel "påkrævet", hvis ikke i mål.

En 3-trins proces blev brugt til at konstatere tilfælde af alle årsager demens.
Først for at identificere mulige tilfælde af demens blev der givet et kort kognitionsscreeningsbatteri til alle deltagere.
Deltagere, der scorer under det forudbestemte screening-cut-point for mulig kognitiv svækkelse under opfølgningen, fik administreret et mere omfattende og detaljeret neurokognitivt testbatteri (Extended Cognitive Assessment Battery) plus Functional Assessment Questionnaire (FAQ), som vurderer svækkelser i dagligdagen levefærdigheder som følge af kognitive svækkelser.
Til sidst blev alle ovennævnte tilgængelige tests og spørgeskemadata sendt til et centraliseret, webbaseret system til bedømmelse af et panel af demenseksperter, som tildelte endelige undersøgelsesklassifikationer af sandsynlig demens (PD), mild kognitiv svækkelse (MCI) eller ingen svækkelse ( NI).
