SPRINT si è sforzato di arruolare circa 9250 partecipanti di età ≥ 50 anni con SBP ≥130 mm Hg e almeno un ulteriore fattore di rischio CVD. Lo studio ha confrontato gli effetti della randomizzazione con un programma di trattamento di un obiettivo SBP intensivo con la randomizzazione con un programma di trattamento con un obiettivo standard. Gli obiettivi di SBP target erano <120 vs <140 mm Hg, rispettivamente, per creare una differenza media minima di 10 mm Hg tra i due gruppi randomizzati. L'ipotesi principale era che i tassi di eventi CVD sarebbero stati inferiori nel braccio intensivo. I partecipanti sono stati reclutati in circa 90 cliniche all'interno di 5 reti di centri clinici (CCN) per un periodo di circa 2 anni e sono stati seguiti per 4-6 anni.
Sono stati arruolati un totale di 9361 partecipanti. Il NIH ha interrotto l'intervento sulla pressione arteriosa prima di quanto originariamente pianificato al fine di diffondere rapidamente i significativi risultati preliminari. Il follow-up per gli esiti cognitivi e renali continua durante la fase post-intervento fino a maggio 2018.

Criterio di inclusione:
- Almeno 50 anni
Pressione arteriosa sistolica di
- 130 - 180 mm Hg con 0 o 1 farmaco
- 130 - 170 mm Hg con un massimo di 2 farmaci
- 130 - 160 mm Hg su un massimo di 3 farmaci
- 130 - 150 mm Hg su un massimo di 4 farmaci
Rischio (uno o più dei seguenti)
- Presenza di malattie cardiovascolari cliniche o subcliniche diverse dall'ictus
- CKD, definita come eGFR 20 - 59 ml/min/1,73 m2
- Un punteggio di rischio Framingham per rischio CVD a 10 anni ≥ 15%
- Età superiore a 75 anni
Criteri di esclusione:
- Un'indicazione per uno specifico farmaco per la riduzione della pressione arteriosa che la persona non sta assumendo e la persona non è stata documentata come intollerante alla classe di farmaci.
- Causa secondaria nota di ipertensione che causa preoccupazione per quanto riguarda la sicurezza del protocollo.
- SBP in piedi per un minuto < 110 mm Hg.
Proteinuria nei seguenti intervalli (basati su una misurazione negli ultimi 6 mesi)
- escrezione urinaria di proteine nelle 24 ore ≥1 g/giorno, o
- escrezione urinaria di albumina nelle 24 ore ≥ 600 mg/giorno, o
- rapporto proteine/creatinina nelle urine spot ≥ 1 g/g di creatinina, o
- rapporto albumina/creatinina nelle urine spot ≥ 600 mg/g di creatinina, o
- dipstick urinario ≥ 2+ proteine
- Circonferenza del braccio troppo grande o piccola per consentire una misurazione accurata della pressione sanguigna con i dispositivi disponibili
- Diabete mellito,
- Storia di ictus (non CE o stent)
- Diagnosi di rene policistico
- Glomerulonefrite trattata o suscettibile di essere trattata con terapia immunosoppressiva
- eGFR < 20 ml/min/1,73 m2 o malattia renale allo stadio terminale (ESRD)
- Evento o procedura cardiovascolare (come definito sopra come CVD clinico per l'ingresso nello studio) o ricovero per angina instabile negli ultimi 3 mesi
- Insufficienza cardiaca sintomatica negli ultimi 6 mesi o frazione di eiezione ventricolare sinistra (con qualsiasi metodo) < 35%
- Una condizione medica che può limitare la sopravvivenza a meno di 3 anni o un tumore maligno diverso dal cancro della pelle non melanoma negli ultimi 2 anni
- Tutti i fattori giudicati dal team clinico come suscettibili di limitare l'adesione agli interventi.
- Mancato ottenimento del consenso informato da parte del partecipante
- Attualmente partecipa a un altro studio clinico (studio di intervento). Nota: il paziente deve attendere il completamento delle sue attività o il completamento dell'altro studio prima di essere sottoposto a screening per SPRINT.
- Vivere nella stessa famiglia di un partecipante SPRINT già randomizzato
- Qualsiasi trapianto di organi
- Perdita di peso non intenzionale > 10% negli ultimi 6 mesi
- Gravidanza, che sta attualmente cercando di rimanere incinta, o in età fertile e che non utilizza il controllo delle nascite.

Sperimentale: Controllo intensivo di SBP
I partecipanti randomizzati nel braccio BP intensivo avranno un obiettivo di SBP <120 mm Hg. Terapia con 2 farmaci iniziata nella maggior parte dei partecipanti intensivi; età ≥75 anni e SBP 130-139 mm Hg con 0-1 farmaco; può iniziare con 1 farmaco, ma aggiungerne un secondo a 1 mese se SBP ≥130 mm Hg; farmaci aggiunti e/o titolati ad ogni visita (mensilmente) per raggiungere SBP <120 mm Hg; alle visite periodiche "milepost": aggiunta di un altro farmaco "necessario" se non all'obiettivo.

È stato utilizzato un processo in 3 fasi per accertare i casi incidenti di demenza per tutte le cause.
In primo luogo, per identificare possibili casi di demenza, è stata somministrata a tutti i partecipanti una breve batteria di screening cognitivo.
Ai partecipanti che hanno ottenuto un punteggio inferiore al cut-point di screening prestabilito per un possibile deterioramento cognitivo durante il follow-up è stata somministrata una batteria di test neurocognitivi più completa e dettagliata (la batteria di valutazione cognitiva estesa) più il questionario di valutazione funzionale (FAQ) che valuta i danni quotidianamente capacità di vivere a causa di disturbi cognitivi.
Infine, tutti i suddetti test disponibili e i dati del questionario sono stati sottoposti a un sistema centralizzato basato sul web per il giudizio da parte di un gruppo di esperti di demenza che ha assegnato le classificazioni finali dello studio di probabile demenza (PD), lieve deterioramento cognitivo (MCI) o nessun deterioramento ( NI).
