수축기 혈압 중재 시험 (SPRINT)

상세 설명
SPRINT는 수축기 혈압이 130mmHg 이상이고 CVD 위험 요소가 하나 이상 있는 50세 이상 참가자 약 9,250명을 등록하기 위해 노력했습니다. 시험은 집중 SBP 목표의 치료 프로그램에 대한 무작위배정의 효과를 표준 목표의 치료 프로그램에 대한 무작위배정과 비교했습니다. 목표 SBP 목표는 각각 120mmHg 미만과 140mmHg 미만으로 두 무작위 그룹 간에 평균 10mmHg의 최소 차이를 만들었습니다. 1차 가설은 CVD 발생률이 집중 팔에서 더 낮을 것이라는 것이었다. 참가자는 약 2년 동안 5개의 임상 센터 네트워크(CCN) 내 약 90개 클리닉에서 모집되었으며 4~6년 동안 추적 관찰되었습니다.
총 9361명의 참가자가 등록되었습니다. NIH는 중요한 예비 결과를 신속하게 배포하기 위해 원래 계획보다 일찍 혈압 개입을 중단했습니다. 인지 및 신장 결과에 대한 후속 조치는 중재 후 단계에서 2018년 5월까지 계속됩니다.

- 최대 4가지 약물에 대해 130~150mmHg
위험(다음 중 하나 이상)
- 뇌졸중 이외의 임상적 또는 준임상적 심혈관 질환의 존재
- eGFR 20 - 59 ml/min/1.73m2로 정의되는 CKD
- 10년 CVD 위험에 대한 Framingham 위험 점수 ≥ 15%
- 75세 이상
제외 기준:
- 환자가 복용하지 않는 특정 혈압 강하 약물에 대한 적응증이며 환자가 해당 약물 종류에 내성이 없는 것으로 문서화되지 않았습니다.
- 프로토콜의 안전성에 대한 우려를 야기하는 알려진 고혈압의 이차 원인.
- 1분 기립 SBP < 110mmHg.
다음 범위의 단백뇨(지난 6개월 이내 측정 기준)
- 24시간 소변 단백질 배설 ≥1g/일, 또는
- 24시간 소변 알부민 배설 ≥ 600mg/일, 또는
- 단백뇨 단백질/크레아티닌 비율 ≥ 1g/g 크레아티닌, 또는

- 소변 딥스틱 ≥ 2+ 단백질
- 팔 둘레가 너무 크거나 작아서 사용 가능한 장치로 정확한 혈압을 측정할 수 없습니다.
- 진성 당뇨병,
- 뇌졸중 병력(CE 또는 스텐트가 아님)
- 다낭성 신장 질환의 진단
- 면역억제 요법으로 치료했거나 치료할 가능성이 있는 사구체신염
- eGFR < 20 ml/min /1.73m2 또는 말기 신질환(ESRD)
- 심혈관 사건 또는 절차(연구 등록을 위한 임상 CVD로 위에서 정의된 대로) 또는 지난 3개월 이내에 불안정 협심증으로 인한 입원
- 지난 6개월 이내에 증상이 있는 심부전 또는 좌심실 박출률(모든 방법) < 35%
- 생존을 3년 미만으로 제한할 가능성이 있는 의학적 상태 또는 지난 2년 이내에 비흑색종 피부암 이외의 악성 종양
- 개입에 대한 준수를 제한할 가능성이 있다고 클리닉 팀이 판단한 모든 요인.
- 참가자로부터 정보에 입각한 동의를 얻지 못함
- 현재 다른 임상 시험(개입 연구)에 참여하고 있습니다. 참고: 환자는 SPRINT 검사를 받기 전에 자신의 활동이 완료되거나 다른 시험이 완료될 때까지 기다려야 합니다.

실험적: SBP 집중 관리
Intensive BP 팔로 무작위 배정된 참가자는 SBP <120mmHg의 목표를 갖게 됩니다. 대부분의 집중 참여자에서 시작된 2가지 약물 요법; 연령 ≥75세 및 0-1 약물에 대한 SBP 130-139mmHg; 1개의 약물로 시작할 수 있지만 SBP ≥130mmHg인 경우 1개월에 두 번째 약물을 추가할 수 있습니다. SBP <120mmHg를 달성하기 위해 각 방문 시(매월) 추가 및/또는 적정 약물; 주기적인 "이정표" 방문 시: 목표에 도달하지 않은 경우 "요구되는" 다른 약물 추가.
