O SPRINT se esforçou para inscrever cerca de 9.250 participantes com idade ≥ 50 anos com PAS ≥ 130 mm Hg e pelo menos um fator de risco cardiovascular adicional. O estudo comparou os efeitos da randomização a um programa de tratamento de uma meta intensiva de PAS com a randomização a um programa de tratamento de uma meta padrão. As metas de PAS alvo eram <120 vs <140 mm Hg, respectivamente, para criar uma diferença média mínima de 10 mm Hg entre os dois grupos randomizados. A hipótese primária era que as taxas de eventos cardiovasculares seriam menores no braço intensivo. Os participantes foram recrutados em aproximadamente 90 clínicas dentro de 5 redes de centros clínicos (CCNs) durante aproximadamente um período de 2 anos e foram acompanhados por 4-6 anos.
Um total de 9361 participantes foram inscritos. O NIH interrompeu a intervenção de pressão arterial antes do planejado originalmente para disseminar rapidamente os resultados preliminares significativos. O acompanhamento dos resultados cognitivos e renais continua durante a fase pós-intervenção até maio de 2018.

Critério de inclusão:
- Pelo menos 50 anos
Pressão arterial sistólica de
- 130 - 180 mm Hg em 0 ou 1 medicamento
- 130 - 170 mm Hg em até 2 medicamentos
- 130 - 160 mm Hg em até 3 medicamentos
- 130 - 150 mm Hg em até 4 medicamentos
Risco (um ou mais dos seguintes)
- Presença de doença cardiovascular clínica ou subclínica, exceto AVC
- CKD, definido como eGFR 20 - 59 ml/min/1,73m2
- Uma pontuação de risco de Framingham para risco de DCV em 10 anos ≥ 15%
- Idade superior a 75 anos
Critério de exclusão:
- Uma indicação para um medicamento específico para redução da pressão arterial que a pessoa não está tomando e que não foi documentada como intolerante à classe de medicamentos.
- Causa secundária conhecida de hipertensão que causa preocupação quanto à segurança do protocolo.
- PAS em pé em um minuto < 110 mm Hg.
Proteinúria nas seguintes faixas (com base em uma medição nos últimos 6 meses)
- excreção de proteína urinária de 24 horas ≥1 g/dia, ou
- Excreção urinária de albumina em 24 horas ≥ 600 mg/dia, ou
- relação proteína/creatinina na urina spot ≥ 1 g/g creatinina, ou
- razão de albumina/creatinina na urina localizada ≥ 600 mg/g de creatinina, ou
- vareta de urina ≥ 2+ proteína
- Circunferência do braço muito grande ou pequena para permitir a medição precisa da pressão arterial com os dispositivos disponíveis
- diabetes melito,
- História de acidente vascular cerebral (não CE ou stent)
- Diagnóstico de doença renal policística
- Glomerulonefrite tratada ou com probabilidade de ser tratada com terapia imunossupressora
- eGFR < 20 ml/min /1,73m2 ou doença renal terminal (ESRD)
- Evento ou procedimento cardiovascular (conforme definido acima como DCV clínica para entrada no estudo) ou hospitalização por angina instável nos últimos 3 meses
- Insuficiência cardíaca sintomática nos últimos 6 meses ou fração de ejeção do ventrículo esquerdo (por qualquer método) < 35%
- Uma condição médica que provavelmente limitará a sobrevida a menos de 3 anos ou uma malignidade diferente do câncer de pele não melanoma nos últimos 2 anos
- Quaisquer fatores julgados pela equipe clínica como prováveis de limitar a adesão às intervenções.
- Falha em obter o consentimento informado do participante
- Atualmente participando de outro ensaio clínico (estudo de intervenção). Nota: O paciente deve esperar até a conclusão de suas atividades ou a conclusão da outra tentativa antes de ser rastreado para SPRINT.
- Viver na mesma casa que um participante SPRINT já randomizado
- Qualquer transplante de órgão
- Perda de peso não intencional > 10% nos últimos 6 meses
- Gravidez, atualmente tentando engravidar ou com potencial para engravidar e sem uso de controle de natalidade.

Experimental: Controle Intensivo da PAS
Os participantes randomizados para o braço de PA intensiva terão uma meta de PAS <120 mm Hg. 2-terapia medicamentosa iniciada na maioria dos participantes do Intensivo; idade ≥75 anos e PAS 130-139 mm Hg em 0-1 droga; pode começar com 1 medicamento, mas adicionar o segundo em 1 mês se PAS ≥130 mm Hg; medicamentos adicionados e/ou titulados em cada consulta (mensalmente) para atingir PAS <120 mm Hg; em visitas periódicas "milhadas": adição de outra droga "necessária" se não atingir o objetivo.

Um processo de 3 etapas foi usado para determinar casos incidentes de demência de todas as causas.
Primeiro, para identificar possíveis casos de demência, uma breve bateria de triagem de cognição foi administrada a todos os participantes.
Os participantes que pontuaram abaixo do ponto de corte de triagem pré-designado para possível comprometimento cognitivo durante o acompanhamento receberam uma bateria de testes neurocognitivos mais abrangente e detalhada (a Bateria de Avaliação Cognitiva Estendida) mais o Questionário de Avaliação Funcional (FAQ) que avalia os comprometimentos nas atividades diárias habilidades de vida como resultado de deficiências cognitivas.
Por último, todos os testes e questionários disponíveis acima foram submetidos a um sistema centralizado baseado na web para julgamento por um painel de especialistas em demência que atribuíram classificações finais do estudo de provável demência (PD), comprometimento cognitivo leve (MCI) ou nenhum comprometimento ( NI).
