この研究の目的は、肝転移の切除のために 2 段階手術を必要とする結腸直腸癌患者の腹部および肝周囲の癒着を防止するための再吸収性バリア膜の有効性を評価することです。
適格な患者は、2 つのアームのいずれかにランダムに割り当てられます。
- セプラフィルム群(初回手術時に吸収性バリア膜を装着)
- 無処置対照群(初回手術時のセプラフィルムバリアなし)
主な目的は、肝転移の切除に 2 段階手術を必要とする結腸直腸癌患者において、2 回目の手術中に腹部および肝周囲の癒着を制限するための再吸収性バリア膜 (セプラフィルム) の有効性を確立することです。
この研究は、前向き多中心第II相、対照、無作為化、および非比較試験です。
合計 60 人の患者が登録されます。45 人はセプラフィルムを受け取り、15 人は無治療の対照群に割り当てられます。 包含期間は約 18 か月である必要があります。 2回目の手術後の経過観察期間は3年です。

この研究の二次的な目的は次のとおりです。
1回目の手術中および手術後 手術手順および吸収性メンブレンの使用に関連して、即時合併症(手術中)、早期合併症(手術後の入院中)および晩期合併症(1回目と2回目の手術の間)を評価する。
2回目の手術中および手術後
- 腹部および肝周囲の癒着(1回目の手術部位)を定性的および定量的に評価します。
- 腸管癒着(小腸)を定量的に評価する。
- 手術手順に関連して、即時合併症(手術中)、早期合併症(手術後の入院中)、晩期合併症(介入の翌月)を評価する。
- 術後のリハビリテーションを評価する
- 2 回目の手術後 3 年間にわたる患者の腫瘍の進展を評価すること。

- 18歳以上の女性と男性
- 同時性または異時性肝転移を伴う結腸直腸癌
- 肝転移の切除のために開腹術を伴う2段階手術を必要とする患者
- 2 つの手術は同じセンターでスケジュールする必要があります
- 2 回の手術は 1 ~ 6 か月の間隔でスケジュールする必要があります。
- 社会保障に加入している患者
- フランス語の読み書きができる患者
- 書面による自発的なインフォームド コンセント
除外基準:
- -以前に臍上切開による肝臓または胆道手術を受けた患者(6か月以上前に開腹術を伴う胆道手術を除く)
- -原始腫瘍の切除のための結腸直腸手術以外の以前の大手術を受けた患者
- 1 回の手術で転移を除去
- 切除不能な転移
- 社会的、地理的、家族的、または心理的な理由でフォローアップが不可能
- 自由を奪われた患者
- -別の実験的手術試験に登録された患者
- 妊娠中または授乳中の女性
