背景 局所抗生物質による定着除去は、NICU (新生児集中治療室) における多剤耐性細菌感染症の発生を予防および/または制御するために必要ですが、細菌耐性を引き起こす可能性があります。 この研究の目的は、MRSA(メチシリン・オキサシリン耐性黄色ブドウ球菌)に定着した新生児と母親との皮膚と皮膚の接触が、細菌の定着を生物学的に制御する効果的な代替手段となり得るかどうかを判断することであった。
方法: 研究者らは、ブラジルのサンルイスにある 3 つの公立病院で NICU に入院した新生児 102 人を調査しました。 対象基準は、出生体重が1300~1800g、滞在期間が4日以上、黄色ブドウ球菌および/またはブドウ球菌コアグラーゼ陰性メチシリン・オキサシリン耐性菌が定着した新生児、およびこれらの細菌が定着していない母親であった。 ランダム化は、コンピューターが生成した乱数アルゴリズムを使用して実行されました。 介入群と対照群への割り当ては、密封された不透明な封筒を使用して、適格な新生児ごとに実行されました。 介入群 (n = 53) では、母子の肌と肌の接触は 1 日 2 回行われました。 対照群(n = 49)は、肌と肌の接触を行わずに日常的なケアを受けました。 新生児の母親や研究者をマスクすることはなかったが、細菌培養を実施して結果を評価した個人には、グループの割り当てを知らされなかった。
主な結果は、7日間の介入後の新生児の鼻孔の除菌でした。 安全性は、介入中に新生児のバイタルサインをモニタリングすることによって評価されました。 副次転帰は、最初に何が起こったとしても、入院期間の終わりまたは生後28日までに遅発性敗血症と推定される症状が出現することであった。

実験的:肌と肌の接触
研究グループの新生児は、7日間(週末を含む)、1日2回(朝と夕方)、60分間、NICU内の母親と肌と肌の接触を行った。
|
肌と肌の接触は、乳児をわずかに着用した状態(おむつのみ)で腹臥位にし、母親の乳房に対して直立させて置くことで構成されていました。
乳児は母親と関わるトラックによってその位置に拘束された。
母親は幼児用ベッドの横にある椅子に座っていた。
介入に同行したチームメンバーは乳児の体温、心拍数、酸素飽和度を監視した。
