このパイロット試験の目的は、心室頻拍 (VT) の経皮的カテーテルアブレーションの治療戦略が最先端の薬物療法よりも優れているかどうかをテストすることを目的とした、大規模な多施設ランダム化臨床試験の実現可能性を判断することです。植込み型除細動器 (ICD) を装着し、可逆的な原因がない場合に VT の治療を受ける患者の全死因死亡率を減らすこと。

この試験は最大 8 か所で実施され、50 人の患者を 1 年間にわたってカテーテルアブレーション (n=25) と最先端の薬物療法 (n=25) の戦略に無作為に割り付けます。
-このパイロット試験への登録を検討するには、患者は少なくとも18歳で、一次または二次予防の適応症のICDを持っている必要があります。記録されたICDショックが1つ以上あるか、VTがない場合にVTに対して3つ以上のATP療法が必要です。治療する医師の意見では、さらなる治療が必要であり、カテーテルアブレーションと少なくとも1つの抗不整脈薬の両方の対象となる可逆的な原因。
患者は生後3か月と6か月で追跡され、生命状態は12か月で電話で決定されます。
パイロット研究で使用される治療法の安全性と有効性に関するデータが収集されます。
特に興味深いのは、カテーテルアブレーション手順に起因する有害事象と、抗不整脈薬の主な副作用です。

- 心臓再同期療法(すなわち、 CRT-D) 一次または二次予防適応症
- -壁運動異常の存在として定義される虚血性心疾患があり、冠動脈疾患が記録されている(1つ以上の主要冠動脈に70%以上の狭窄が1つある)
- -VTの無作為化前の6か月以内に1回以上の文書化されたICDショックまたは3回以上のATP治療を受けています。 VT は単形性で、レートが ≤ 260 bpm でなければなりません。
- 18歳以上であること
- カテーテルアブレーションの対象となる
- アミオダロン、ソタロール、およびメキシレチンの少なくとも 1 つの抗不整脈薬に対する不耐症または禁忌の病歴がない。
除外:
- 主治医が追加治療を受けるべきではないと判断した患者
- -患者が心房性不整脈または早期心室収縮(PVC)のために毎日200mg以下のアミオダロンを服用していない限り、過去3か月間に30日以上のアミオダロン治療を受けており、患者はより高用量のアミオダロンに適格です
- 即時治療を必要とする絶え間ない VT
- 虚血、非代償性心不全、および電解質障害を含むがこれらに限定されない VT の可逆的原因
- -VTのカテーテルアブレーションに対する禁忌の存在(可動性心室血栓の存在、急性心筋梗塞、冠動脈血行再建術、または過去30日間の脳卒中、不安定狭心症またはNYHAクラスIV HF、機械弁、または機能不全の存在を含む)抗凝固療法または抗血栓療法を受けるため)
- 非虚血性心筋症の患者
- 肥大型閉塞性、拘束性、または浸潤性心筋症の患者
- 急性心筋炎、先天性心疾患、今後1年以内に手術が必要と思われる弁膜症、および/または手術不能な閉塞性弁膜症の患者
- -心臓移植を受けた患者、または12か月以内に心臓移植を受ける予定の患者
- 左心室補助装置を装着している患者
- -すでにVTの抗不整脈薬療法(ベータ遮断薬以外)を受けている患者(心房性不整脈またはPVCの抗不整脈薬療法を受けている場合は除外されません) VTの追加の薬物療法の対象となる場合)。
- クラス III の薬剤による遺伝性不整脈またはトルサード ド ポワントのリスク増加
- -透析を必要とする末期腎疾患
- -心臓以外の原因による推定余命は1年未満
- 妊娠中または出産の可能性があり、信頼できる避妊方法を使用していない女性
