이 파일럿 시험의 목적은 심실성 빈맥(VT)의 경피적 카테터 절제 치료 전략이 최신 약물 요법보다 우수한지 여부를 테스트하기 위한 대규모 다기관 무작위 임상 시험의 타당성을 결정하는 것입니다. 가역적 원인 없이 VT 치료를 받는 이식형 제세동기(ICD) 환자의 모든 원인으로 인한 사망률 감소.

이 연구는 최대 8개 사이트에서 수행되며 1년 동안 50명의 환자를 카테터 절제술(n=25) 대 최첨단 약물 요법(n=25) 전략에 무작위 배정합니다.
이 파일럿 시험에 등록을 고려하려면 환자는 18세 이상이어야 하고 1차 또는 2차 예방 적응증에 대한 ICD가 있어야 하며, 문서화된 ICD 쇼크가 1회 이상 있거나 또는 VT에 대한 ATP 요법이 3회 이상 있어야 합니다. 치료 의사의 의견에 따라 추가 치료가 필요하고 카테터 절제술과 최소 1가지 항부정맥제를 모두 받을 수 있는 가역적 원인.
3개월과 6개월에 환자를 추적하고 12개월에 전화 통화를 통해 활력 상태를 결정합니다.

포함 기준:
- 심장 재동기화 요법(즉, CRT-D) 1차 또는 2차 예방 적응증
- 벽 운동 이상 및 기록된 관상 동맥 질환의 존재로 정의되는 허혈성 심장 질환이 있습니다(≥ 1개의 주요 관상 동맥에서 하나의 ≥ 70% 협착증).
- 치료 의사의 의견에 따라 추가 치료가 필요한 가역적 원인이 없는 경우 VT에 대한 무작위 배정 전 6개월 이내에 1회 이상의 문서화된 ICD 쇼크 또는 3회 이상의 ATP 요법을 받았습니다. VT는 단형이어야 하며 속도는 ≤ 260 bpm이어야 합니다.
- 18세 이상
- 카테터 절제술을 받을 수 있어야 합니다.
- 아미오다론, 소탈롤 및 멕실레틴과 같은 항부정맥제 중 적어도 하나에 대한 불내성 또는 금기의 병력이 없습니다.
제외:
- 치료 의사의 의견에 따라 추가 치료를 받지 않아야 하는 환자
- 환자가 심방성 부정맥 또는 조기 심실 수축(PVC)에 대해 매일 ≤ 200mg의 아미오다론을 사용했고 환자가 고용량의 아미오다론을 투여받을 자격이 없는 경우를 제외하고 지난 3개월 동안 30일 이상의 아미오다론 치료
- 즉각적인 치료가 필요한 지속적인 VT
- 허혈, 보상되지 않은 HF 및 전해질 교란을 포함하되 이에 국한되지 않는 VT의 가역적 원인
- VT의 카테터 절제에 대한 금기의 존재(움직이는 심실 혈전의 존재, 급성 심근경색, 관상동맥 재생술 또는 이전 30일 이내의 뇌졸중, 불안정 협심증 또는 NYHA 클래스 IV HF, 기계적 판막 또는 무능력 포함) 항응고제 또는 항혈전제 치료를 받기 위해)
- 비허혈성 심근병증 환자
- 비대성 폐쇄성, 제한성 또는 침윤성 심근병증 환자
- 급성 심근염, 선천성 심장병, 향후 1년 이내에 수술이 필요할 가능성이 있는 판막 질환 및/또는 수술 불가능한 폐쇄성 판막 질환이 있는 환자
- 심장이식을 받았거나 12개월 이내에 심장이식을 받을 것으로 예상되는 환자
- 좌심실 보조 장치가 있는 환자
- 이미 VT에 대한 항부정맥제 요법(베타 차단제 제외)을 받고 있는 환자(심방 부정맥에 대한 항부정맥제 요법을 받고 있거나 VT에 대한 추가 약물 요법을 받을 자격이 있는 경우 PVC를 받고 있는 환자는 제외되지 않습니다).
- 유전성 부정맥 또는 클래스 III 약물 사용 시 토르사드 드 포인트에 대한 위험 증가
- 투석이 필요한 말기 신질환
- 비심장성 원인으로 예상 수명이 1년 미만
