肥満はいくつかの健康状態のリスクを高めます。 初期体重の 5 ~ 10% の体重減少により、いくつかの病気のリスクが軽減されます。 この程度の体重減少は、行動的肥満プログラムで達成可能です。 しかし、最初に減少した体重の約 33% は 1 年以内に回復し、3 ~ 5 年以内にはわずかな体重減少が維持されます。 したがって、長期的な減量維持を改善する新しい戦略が必要です。 自己申告による満腹感と満腹感を高める食事戦略の 1 つは、低エネルギー密度 (ED) の食事を摂取することです。 低ED食では、消費される総エネルギーに比べてより多くの食品の重量が可能であり、これが、低ED食で見られる自己申告による満腹感と満腹感の増強について提案されているメカニズムです。 研究によると、参加者に低EDの食事を提供すると、摂取した食物の総重量は変化しないものの、食事のエネルギー摂取量が減少することが示されています。 重要なのは、低EDの食事を数日間にわたって摂取した場合、エネルギー摂取量の減少が継続的に発生し、エネルギー摂取量の補償の程度が示されていないことです。 食事性 ED と体重減少維持の関係に関するギャップに対処するために、我々は、低 ED 処方とその提案されたメカニズムが体重減少維持に及ぼす影響を調べるランダム化比較試験 (RCT) を実施することを提案します。

低EDの食事は、10、16、および22か月時のエネルギーバランスよりもEDが低く、エネルギーと脂肪からのエネルギーの割合が少なく、固形食品と繊維からのグラム数が多くなります。
二次仮説は次のとおりです。
ED を軽減することで長期的なエネルギー摂取量の低下を改善するメカニズム (メディエーター) を特定します。
- 低EDは、10、16、および22か月後のエネルギーバランスよりもEMA中に空腹感が低く、満腹感と満腹感が大きいと自己報告します。

- 年齢は18歳から70歳まで
- 体格指数 (BMI) が 27 ~ 45 kg/m2 である
除外基準:
- 身体活動準備アンケートで、心臓の状態、活動中または休息中の胸の痛み、または意識の喪失を報告してください。
- 関節の問題、処方薬の使用、または運動を制限する可能性のあるその他の病状を報告している人は、参加するには医師の書面による同意を得る必要があります。
- 立ち止まらずに2ブロック(1/4マイル)歩くことができないと報告する
- 主要な精神疾患または器質性脳症候群を報告します。
- 現在減量プログラムに参加している、および/または減量薬や食欲調整薬を服用している、または過去6か月間で体重の5%以上減少した
- 体重減少のための肥満手術を受けたことがある、または今後22か月以内に肥満手術を受ける予定がある
- 調査期間内に首都圏外に転居する予定がある

- 食事代替品を摂取できないと報告する
減量維持フェーズに参加するには、参加者は 4 か月の減量フェーズ終了時にベースライン測定値から体重の 8% 以上減少する必要があります。

この処方には他の栄養素の目標は含まれていないため、エネルギー目標はありません。

この状態では、食品の ED のみに基づいて食品摂取の決定を行うように指示されます。
ED 状態の目標は、1.0 kcal/g 以下の食品を少なくとも 10 品(果物と野菜、スープベースのスープ、無脂肪ヨーグルト、一部の豆類、卵代替品、一部の白身魚など)摂取し、一切摂取しないことです。 1日あたり3.0 kcal/g以上の食品(クラッカー、ポテトチップス、クッキー、ハードチーズ、ホットドッグ、サラダドレッシングなど)を2つ以上摂取する。

Energy Balance は、測定された安静時代謝率に 1.12 (男性) または 1.14 (女性) の身体活動レベル (PAL) (低活動量) を乗じた、個人に合わせた毎日のエネルギー目標を受け取ります。
