研究の質問:
腎臓移植レシピエントを最大限の MPA 用量(EC-MPS または Myfortic® の形態)で治療し、賢明な CNI 曝露(シクロスポリン/Neoral®)を行うと、7 年間の専攻のフラミンガム リスク スコアで測定される心血管転帰の改善につながりますか心臓有害事象 (MACE) スコアと心血管リスク炎症性バイオマーカー プロファイルは?
主な目的:
- 心血管疾患のリスクを推定する腎移植レシピエントのフラミンガムリスクスコアと7年MACEスコアを改善する。
- 心血管リスクの炎症性バイオマーカープロファイルを改善するため。
仮説:
全用量の Myfortic® と組み合わせた 2 時間レベル (C2) を使用してシクロスポリンをモニタリングすると、より一貫した薬物曝露と Cmax の低下が認められ、腎臓移植レシピエントのフラミンガム リスク スコア、MACE スコア、および炎症マーカーが低下します。
- CNI 最小化プロトコルは、同種移植片および血管損傷を改善する戦略として広く受け入れられています。
- 慢性同種移植片損傷および血管疾患は、既知の炎症状態です。
- MPA 誘導体は、顕著な抗炎症特性を持っています。

- 腎移植患者は外来患者として追跡され、MPA誘導体、CNI、プレドニゾンによる免疫抑制療法で現在安定化しているが、安定性とは血清クレアチニンの変化が10%未満、または過去3カ月での変化として定義される。
- 年齢は18歳から74歳まで。
- 移植後少なくとも6か月。
- 過去 12 週間以内に移植拒絶反応がないこと。
- 登録時の血清クレアチニンが 300 umol/L 未満。
- 妊娠の可能性のある女性患者の尿妊娠検査が陰性。
- 研究への同意。
- 過去90日以内に別の介入臨床試験に参加していない。
除外基準:
- 他の種類の固形臓器移植を受けた患者。
- あらゆる形態の薬物乱用または重度の精神障害のある患者。
- 急性または慢性の下痢、既知の腸疾患、または既知の胃不全麻痺のある患者。
- 過去6か月以内に拒絶反応に対する抗リンパ球治療を受けている患者。
- ミコフェノール酸誘導体を投与されていない患者。
- Myfortic® の最大 1 日総用量 1440 mg OD に耐えられない患者。
- 重度の肝疾患を患っている患者は、ビリルビン値が正常範囲の上限値の少なくとも 2 倍上昇していると定義されます。
- 研究に支障をきたす可能性のある不安定な病状を患っている患者。
- HIV、B型肝炎およびC型肝炎による慢性ウイルス感染症の患者。
- 過去 4 週間以内に入院を必要とする急性疾患の存在。
- 妊娠。
- 過去12週間以内のMI、脳卒中、TIAなどの重大な心血管イベント、または制御されていない高血圧。
- 過去 1 か月以内に免疫抑制剤が変更された。
