Onderzoeksvraag:
Zal het behandelen van ontvangers van niertransplantaten met maximale MPA-doseringen (in de vorm van EC-MPS of Myfortic®) samen met verstandige CNI-blootstelling (cyclosporine/Neoral®) leiden tot verbeterde cardiovasculaire uitkomsten, zoals gemeten door de Framingham Risk Score, 7-jaar major ongunstige cardiale gebeurtenissen (MACE)-score en cardiovasculair risico-inflammatoire biomarkerprofiel?
Primaire doelen:
- Verbeteren van de Framingham Risk Score en 7-jaars MACE-score voor ontvangers van een niertransplantatie, die het risico op hart- en vaatziekten inschatten.
- Om het inflammatoire biomarkerprofiel voor cardiovasculair risico te verbeteren.
Hypothese:
De meer consistente geneesmiddelblootstelling en lagere Cmax waargenomen bij het monitoren van ciclosporine met behulp van de 2-uurs niveaus (C2) in combinatie met de volledige dosis Myfortic® zal de Framingham Risk Score, MACE-score en ontstekingsmarkers bij ontvangers van een niertransplantaat verlagen, omdat:
- CNI-minimaliseringsprotocollen worden algemeen aanvaard als een strategie om allograft- en vasculair letsel te verbeteren.
- Chronisch transplantaatletsel en vaatziekten zijn bekende ontstekingsaandoeningen.
- De MPA-derivaten bezitten significante ontstekingsremmende eigenschappen.

Beschrijving
Inclusiecriteria:
- Niertransplantatiepatiënten volgden als poliklinische patiënten die momenteel gestabiliseerd zijn op immunosuppressieve therapie met een MPA-derivaat, een CNI en prednison waarbij stabiliteit wordt gedefinieerd als een verandering in serumcreatinine van minder dan 10% of gedurende de laatste drie maanden.
- Leeftijd 18-74 jaar oud.
- Minimaal zes maanden na transplantatie.
- Gebrek aan transplantaatafstoting in de afgelopen 12 weken.
- Serumcreatinine minder dan 300 umol/L bij inschrijving.
- Negatieve urine-zwangerschapstest voor vrouwelijke patiënten die zwanger kunnen worden.
- Toestemming voor de studie.
- Niet opgenomen in een ander interventioneel klinisch onderzoek in de afgelopen 90 dagen.
Uitsluitingscriteria:
- Patiënten met andere soorten solide orgaantransplantaties.
- Patiënten met enige vorm van middelenmisbruik of een ernstige psychiatrische stoornis.
- Patiënten met acute of chronische diarree, bekende darmziekte of bekende gastroparese.
- Patiënten die in de afgelopen zes maanden een antilymfocytenbehandeling hebben ondergaan wegens afstoting.
- Patiënten die geen mycofenolzuurderivaat krijgen.
- Patiënten die de maximale totale dagelijkse dosis Myfortic® van 1440 mg eenmaal daags niet verdragen.
- Patiënten met een significante leveraandoening gedefinieerd als een verhoogd bilirubine met ten minste tweemaal de bovenwaarde van het normale bereik.
- Patiënten met een onstabiele medische aandoening die de studie zou kunnen verstoren.
- Patiënten met chronische virale infectie met HIV, Hepatitis B & C.
- Aanwezigheid van een acute ziekte die opname in het ziekenhuis gedurende de laatste 4 weken vereist.
- Zwangerschap.
- Significante cardiovasculaire voorvallen zoals MI, beroerte of TIA in de afgelopen 12 weken of ongecontroleerde hypertensie.
- Immunosuppressieve veranderingen in de afgelopen maand.

Maximalisatie van mycofenolzuur (MPA)-derivaat (tot een totale dagelijkse dosering van 1440 mg Myfortic®) en verlaging van de ciclosporinedosering (zoals gedefinieerd door C2-monitoring) bij 50 stabiele niertransplantatiepatiënten die eerder immunosuppressieve therapie kregen met ciclosporine, een MPA-derivaat en prednison.

25 patiënten die een derivaat van mycofenolzuur (MPA) gebruikten, tacrolimus en prednison zullen tijdens dezelfde wervingsperiode worden gevolgd als aanvullende vergelijking, aangezien dit de andere calcineurineremmer (CNI) is, die wordt gebruikt bij niertransplantatie.
