研究者らは、抗 VEGF 延長治療レジメンに対する反応が不完全な滲出型加齢黄斑変性症患者を対象に、1 日 1 回低用量の経口ドキシサイクリン 50 mg とプラセボの効果を評価するために、ダブルマスク無作為試験を実施する予定です。
調査は 9 か月にわたって実施され、3 か月ごとの評価が行われます。ドキシサイクリン対プラセボ(ダブルマスク、無作為化)の6か月の投与期間と3か月のフォローアップ期間。 参加者の訪問は、標準治療の硝子体内注射 (IVI) のための診療所訪問中に行われます。
医療情報は、標準治療の光コヒーレンストモグラフィー (OCT)、視力 (VA)、および投与された IVI (薬の数、頻度、および種類) から取得されます。
血漿および房水(前房タップ)サンプルおよび治験薬の忍容性は、ベースライン、6か月および9か月で取得されます。 血漿は、炎症マーカーをアッセイするために取得されます。MMP-9 レベルとアンケートは、1) 視覚の生活の質、および 2) ベースライン、6 か月および 9 か月での薬物耐性について取得されます。

包含基準:
- 湿性加齢黄斑変性症 (wAMD);
- wAMDによるアクティブなCNVに対して抗VEGF IVIのみで治療。 ただし、登録された患者は、抗VEGF IVIで治療されていない糖尿病性網膜症や静脈閉塞などの他の網膜病変を有する可能性があります。
- -抗VEGF剤を少なくとも3回連続して注射したにもかかわらず、wAMDからのアクティブなCNVにより、網膜内液の有無にかかわらず持続的な網膜下が必要です。
- テトラサイクリン薬による以前の副作用に遭遇してはなりません。
除外基準 - 眼球:
- -ブドウ膜炎(眼内炎を含む)の病歴または眼内炎症の存在;
- 黄斑解剖学的構造の歪みを引き起こす重要な網膜上膜または黄斑孔の存在;
- OCTでの液体の識別を妨げるメディアの不透明性の存在;
- -過去3か月以内の以前の眼科手術(YAGまたは網膜レーザーを含む)、または眼科手術(白内障摘出を含む)の必要性が予想される ランダム化後9か月;
- -過去6か月以内の調査対象の目または仲間の目への眼球周囲コルチコステロイド注射の履歴;
- -過去4か月以内の研究眼への硝子体内トリアムシノロンアセトニド注射の履歴;
- -調査対象の眼に眼の状態(AMD以外)が存在し、調査担当者の意見では、調査の過程で視力が変化する可能性があります(例:網膜静脈閉塞、ブドウ膜炎または他の眼の炎症性疾患、血管新生緑内障、およびアーバイン・ガス症候群);
- wAMD以外の原因によるCNV;
- 募集後6か月目と9か月目の時点でフォローアップできない;
- 6か月の治療期間中に2回以上連続して注射を逃した;
- -9か月の試用期間中に、IVI抗VEGF薬の切り替えまたは光線力学療法などの別の治療介入が差し迫って必要な患者。
- 地理的萎縮または円板状瘢痕に関連する体液の存在;
- CNV によるものではない網膜下液および/または網膜内液を有する患者
- アーバイン・ガス症候群の積極的な治療を受けている患者。
除外基準 - 全身:
- -不安定な病状(例:血糖コントロール、血圧、心血管疾患、9か月の試用期間を完了する可能性が低い、またはできない個人)を有するおよび/または発症した患者 治験責任医師の意見;
- -重大な腎疾患(血清クレアチニン> 2.5 mg / dLとして定義);
- -収縮期血圧> 180 mm Hgまたは拡張期血圧> 110 mm Hg;
- テトラサイクリン療法に伴う頭痛の病歴
- -偽脳腫瘍の病歴;
- -過去6か月以内のテトラサイクリン療法の履歴;
- -妊娠中または妊娠を希望している患者 試用期間の9か月以内。 出産の可能性のある女性の場合、妊娠検査が行われます。
- -出産の可能性のある性的に活発な女性は、研究期間中(少なくとも24か月)、医学的に認められたデバイスまたは治療法(つまり、子宮内デバイス、ホルモン避妊薬、またはバリアデバイス)を使用して避妊を積極的に実践していません。 ドキシサイクリンはホルモン避妊薬の効果を妨げる可能性があるため、これは重要です。 したがって、ホルモン避妊薬を使用する出産の可能性のある性的に活発な女性は、研究に参加している間、避妊の失敗を防ぐために2番目の避妊法を使用する必要があります。
