MMP-9-remming voor recalcitrante natte leeftijdsgebonden maculadegeneratie (AMD)
Natte (of neovasculaire) vorm van leeftijdsgebonden maculadegeneratie (wAMD) is de meest voorkomende oorzaak van blindheid in de westerse wereld. Momenteel blijven antivasculaire endotheliale groeifactor (VEGF) intravitreale injecties (IVI) de standaardbehandeling voor wAMD. Eerdere studies tonen aan dat ongeveer 90% van de behandelde patiënten na 2 jaar follow-up een minimale visuele functie verliest. Er is nog steeds een subgroep van 15% patiënten, onvolledige responders, die niet verbeteren en mogelijk verslechteren als gevolg van de aanhoudende subretinale vloeistof (met of zonder intraretinale vloeistof) bij chronische behandeling.
De onderzoekers zijn van plan om het effect van orale doxycycline versus placebo op de anatomische en functionele resultaten te evalueren in aanhoudende subretinale oogvloeistof bij neovasculaire natte leeftijdsgebonden maculaire degeneratie. Deze subgroep is onvolledig of reageert niet op de huidige intravitreale anti-VEGF-therapie.

Gedetailleerde beschrijving
De onderzoekers zijn van plan een dubbelblinde, gerandomiseerde studie uit te voeren om het effect te beoordelen van een lage dosis, orale doxycycline 50 mg eenmaal daags versus placebo bij patiënten met natte leeftijdsgebonden maculaire degeneratie die onvolledig reageren op uitgebreide anti-VEGF-behandelingsregimes.
Het onderzoek zal gedurende 9 maanden worden uitgevoerd met elke 3 maanden evaluaties; 6 maanden toedieningsperiode van doxycycline versus placebo (dubbel gemaskeerd, gerandomiseerd) en een follow-upperiode van 3 maanden. De bezoeken van de deelnemers vinden plaats tijdens kliniekbezoeken voor intravitreale injecties (IVI) volgens de standaardbehandeling.
Medische informatie zal worden verkregen uit de standaardzorg optische coherentietomografie (OCT), gezichtsscherpte (VA) en toegediende IVI (aantal, frequentie en type medicijn).
Monsters van bloedplasma en waterige vloeistof (voorste kamerkraan) en verdraagbaarheid van het studiegeneesmiddel zullen worden verkregen bij aanvang, 6 maanden en 9 maanden. Plasma zal worden verkregen om te testen op ontstekingsmarkers: MMP-9-niveaus en vragenlijsten zullen worden verkregen over 1) visuskwaliteit van leven, en 2) studiegeneesmiddeltolerantie bij baseline, 6 maanden en 9 maanden.

Beschrijving
Inclusiecriteria:
- Natte leeftijdsgebonden maculaire degeneratie (wAMD);
- Alleen behandeld met anti-VEGF IVI voor actieve CNV vanwege wAMD. Geregistreerde patiënten kunnen echter andere netvliespathologieën hebben, zoals diabetische retinopathie of aderocclusie waarvoor ze niet worden behandeld met anti-VEGF IVI;
- Moet een aanhoudend subretinaal hebben met of zonder intra-retinale vloeistof als gevolg van actieve CNV van wAMD ondanks het ontvangen van ten minste drie opeenvolgende injecties met een anti-VEGF-middel;
- Mag geen eerdere bijwerkingen van tetracycline-medicatie hebben ondervonden.
Uitsluitingscriteria - Oculair:
- Geschiedenis van uveïtis (inclusief endoftalmitis) of aanwezigheid van intraoculaire ontsteking;
- Aanwezigheid van een aanzienlijk epiretinaal membraan of maculagat dat vervorming van de maculaire anatomie veroorzaakt;
- Aanwezigheid van ondoorzichtigheid van de media verhindert het onderscheiden van vloeistof op OCT;
- Elke eerdere oogoperatie (inclusief YAG of retinalaser) binnen de voorgaande 3 maanden of verwachte noodzaak voor een oogoperatie (inclusief cataractextractie) gedurende 9 maanden na randomisatie;
- Geschiedenis van peribulbaire injectie van corticosteroïden in het bestudeerde oog of het andere oog in de afgelopen 6 maanden;
- Geschiedenis van intravitreale triamcinolonacetonide-injectie in het bestudeerde oog in de afgelopen 4 maanden;
- Er is een oogaandoening (anders dan LMD) aanwezig in het bestudeerde oog die, naar de mening van de onderzoeker, de gezichtsscherpte in de loop van het onderzoek zou kunnen veranderen (bijv. en Irvine-Gass-syndroom);
- CNV door andere oorzaken dan wAMD;
- Onvermogen om op te volgen op de tijdstippen van de 6e en 9e maand na werving;
- Twee of meer opeenvolgende injecties missen tijdens de behandelingsperiode van zes maanden;
- Patiënt die dringend IVI anti-VEGF-medicatie nodig heeft of een andere behandelingsinterventie, zoals fotodynamische therapie, tijdens de proefperiode van 9 maanden;
- Aanwezigheid van vocht geassocieerd met geografische atrofie of disciform litteken;
- Elke patiënt met subretinale en/of intraretinale vloeistof die niet te wijten is aan CNV (bijv. bovenliggende gebieden met geografische atrofie;
- Elke patiënt die actief wordt behandeld voor het syndroom van Irvine-Gass.
Uitsluitingscriteria - Systemisch:
- Patiënt met en/of die een onstabiele medische status heeft ontwikkeld (bijv. glykemische controle, bloeddruk, hart- en vaatziekten, personen die de proefperiode van 9 maanden waarschijnlijk niet of niet kunnen voltooien) naar de mening van de onderzoeker;
- Significante nierziekte (gedefinieerd als een serumcreatinine >2,5 mg/dl);
- Systolische bloeddruk >180 mm Hg of diastolische bloeddruk >110 mm Hg;
- Geschiedenis van hoofdpijn geassocieerd met tetracyclinetherapie
- Geschiedenis van pseudotumor cerebri;
- Geschiedenis van tetracyclinetherapie in de afgelopen 6 maanden;
- Zwangerschap of patiënt die van plan is zwanger te worden binnen de 9 maanden van de proefperiode. Bij vrouwen in de vruchtbare leeftijd wordt een zwangerschapstest uitgevoerd;
- Seksueel actieve vrouwen in de vruchtbare leeftijd die tijdens de onderzoeksperiode (minstens 24 maanden) niet actief anticonceptie toepassen door gebruik te maken van een medisch geaccepteerd apparaat of therapie (d.w.z. intra-uterien apparaat, hormonaal anticonceptiemiddel of barrièreapparaat). Dit is belangrijk omdat doxycycline de effectiviteit van hormonale anticonceptiva kan verstoren. Daarom zullen seksueel actieve vrouwen in de vruchtbare leeftijd die een hormonaal anticonceptiemiddel gebruiken, tijdens hun deelname aan het onderzoek een tweede vorm van anticonceptie moeten gebruiken om te voorkomen dat de anticonceptie faalt;
- Bekende allergie/intolerantie voor doxycycline, tetracyclines of een ingrediënt in het onderzoeksgeneesmiddel of placebo;
- Patiënten die fenytoïne, barbituraten, carbamazepine, digoxine of isotretinoïne krijgen; patiënten met gastroparese; patiënten met een voorgeschiedenis van gastrectomie, maagbypassoperatie of anderszins geacht achloorhydrisch te zijn, moeten allemaal worden uitgesloten vanwege een veranderde farmacokinetiek en/of biologische beschikbaarheid van doxycycline;
- Patiënten die strontium, acitretine of tretinoïne gebruiken, moeten worden uitgesloten vanwege de mogelijkheid van ernstige interacties met doxycycline;
- Patiënten met abnormale ALAT of ASAT bij baseline zullen worden doorverwezen naar hun huisarts voor medische goedkeuring voor deelname aan dit onderzoek.
