浸潤性真菌感染症 (IFIs) は、同種造血幹細胞移植 (aHSCT) を受ける免疫抑制患者の死亡率と罹患率の主要な原因のままです。 酵母菌とカビの両方が、このコホートの患者に重篤な IFI を引き起こします。特に、生着前の好中球減少期と、重度の移植片対宿主病 (GVHD) のために患者がコルチコステロイドの長期投与を必要とする生着後の期間に発生します。 IFIs の他のリスク要因には、環境要因、遺伝的要因、および共ウイルス感染が含まれます。 カンジダ spp は侵襲性酵母感染症のほとんどを占めていますが、アスペルギルス spp は aHSCT レシピエントで最も一般的なカビです。 抗真菌薬による予防を行わない場合の、証明された可能性のある侵襲性カンジダ感染症の推定発生率は、8% から 24% の範囲でした。一方、侵襲性アスペルギルス症の発生率は 3% から 14% の間であると報告されています。 IFI は多くの場合、60% もの高い死亡率と関連しており、多変量解析における独立した死亡予測因子の 1 つとして特定されています。 その他の合併症には、侵襲性真菌感染症による入院期間の延長、集中治療室への入院、莫大な治療費、重大な罹患率、患者の生活の質の低下などがあります。
