経皮的脛骨神経刺激療法 (PTNS) は、短期および中期の有効性が証明されている保存的治療に難治性の便失禁 (FI) 患者に対して検証された外来治療です。
したがって、研究者は、3 年間の追跡調査を考慮して、PTNS の長期有効性を評価することを目的としました。 研究者はまた、反応の予測因子を特定し、部分反応者のための新しいアプローチを提案することも目指しました.

PTNS で治療された FI 患者の前向き単一施設コホートが分析された。
PTNS セッションは 3 段階で実施されました。第 1 段階では毎週 3 か月間、第 2 段階では隔週で 3 か月間、第 3 段階と最終段階では 6 か月間毎月行われました。
各フェーズの終わりに臨床管理を行い、36 か月後に追加の追跡調査を実施しました。 Wexner スコア、便意切迫感、排便習慣、および FI の生活の質が評価されました。
患者は 3 つのグループに分類されました。ウェクスナー スコアが 50% を超えて改善した場合に最適なレスポンダー。 Wexner スコアの改善が 25 ~ 50% の場合は部分応答者。 Wexner スコアの改善が 25% 未満の場合、非応答者。
最適応答者と部分応答者は相次ぐフェーズに進みましたが、非応答者は PTNS を放棄し、他の治療オプションが提供されました。

包含基準:
- 6か月以上の便失禁の診断で、保存療法に抵抗性。
除外基準:
- 手術が必要な解剖学的損傷。
- 180°を超える外肛門括約筋病変。
- 定期的な外来診療に通うことができない。
- 主要な心理的または精神医学的合併症
