Effectiviteit op lange termijn van percutane tibiale zenuwstimulatie voor fecale incontinentie en nieuwe aanpak voor partiële responders
Percutane tibiale zenuwstimulatie (PTNS) is een ambulante therapie die gevalideerd is voor patiënten met fecale incontinentie (FI) die ongevoelig zijn voor conservatieve behandeling, met bewezen werkzaamheid op korte en middellange termijn.
De onderzoekers probeerden daarom de werkzaamheid van PTNS op lange termijn te evalueren, rekening houdend met een follow-up van 3 jaar. De onderzoekers probeerden ook voorspellers van reacties te identificeren en een nieuwe aanpak voor partiële responders voor te stellen.

Prospectieve single-center cohort van patiënten met FI behandeld met PTNS werd geanalyseerd.
PTNS-sessies werden uitgevoerd in drie fasen: wekelijks gedurende drie maanden in de eerste fase, tweewekelijks gedurende drie maanden in de tweede fase en maandelijks gedurende zes maanden in de derde en laatste fase.
Klinische controle aan het einde van elke fase en een aanvullende follow-up na 36 maanden. Wexner-score, fecale urgentie, stoelgang en kwaliteit van leven voor FI werden beoordeeld.
Patiënten werden ingedeeld in drie groepen: optimale responders wanneer er een verbetering was in de Wexner-score > 50%; partiële responders als de verbetering van de Wexner-score 25-50% was; en non-responders, wanneer de verbetering van de Wexner-score < 25% was.
Optimale responders en partiële responders gingen door naar opeenvolgende fasen, terwijl non-responders de PTNS verlieten en andere behandelingsopties werden aangeboden.

Inclusiecriteria:
- Diagnose van fecale incontinentie gedurende meer dan 6 maanden, ongevoelig voor conservatieve behandeling.
Uitsluitingscriteria:
- Anatomische verwondingen die een operatie vereisen.
- Externe anale sluitspierlaesie van > 180º.
- Onbeschikbaarheid om regelmatig naar de polikliniek te gaan.
- Ernstige psychische of psychiatrische comorbiditeiten
