第一目的:
I. 6週間のスウェーデン式マッサージ療法(SMT)介入が、前立腺がん(PCa)のライトタッチ(LT)または待機リストコントロール(WLC)よりもがん関連疲労(CRF)を有意に減少させるかどうかを評価するための予備研究を実施する放射線療法とアンドロゲン除去療法の両方を受けた生存者。
副次的な目的:
I. SMT による癌関連疲労の仮説上の減少が、慢性炎症の根底にある特定の免疫系因子の調節と関連しているかどうかを評価すること。
探索的目的:
I. SMT が自己申告による疲労、生活の質、抑うつ、および不安を LT または WLC より有意に改善するかどうかを評価すること。
Ⅱ. 介入後 6 週間および 12 週間で、6 週間の SMT と CRF の LT の持続的作用に関する予備データを収集すること。
概要: 患者は 3 群のうちの 1 群に無作為に割り付けられます。
アーム I: 患者は SMT を週に 1 回、6 週間にわたって 45 分以上受けます。
アーム II: 患者は 45 分以上の LT を週 1 回、6 週間受けます。
アーム III: 患者は 6 週間介入を受けません。
研究の完了後、患者は 12 週間と 18 週間でフォローアップされます。

包含基準:
- -45歳以上の男性被験者
- -前立腺癌の組織学的に確認された診断
- -被験者は放射線療法を完了している必要があります>= 2か月、しかし= <登録の6年前
- -血清テストステロンレベルが20 ng / ml未満のアンドロゲン除去療法を受けている被験者
- -スクリーニング時の簡易疲労インベントリ(BFI)でスコアが25を超える被験者
- 英語を話すことと読むことに堪能な対象
国際疾病分類 (ICD)-10 で提案されている基準に基づいて、患者は、病歴、身体検査、および検査所見から、疲労ががんまたはがん治療の結果であり、主な結果ではないという証拠に基づいて CRF と診断されている必要があります。次のいずれか:
- 併存する精神疾患
- 貧血(ヘモグロビン10g/dl未満)
- 甲状腺機能低下症(甲状腺刺激ホルモン(TSH)が4.6マイクロ国際単位(uIU)/mLを超える)
- コントロールできない痛み
研究者の病歴、身体検査、および評価に基づいて、臨床的に疲労に寄与していると思われる医学的または精神医学的状態または投薬。 これらの医学的状況には以下が含まれる場合があります。
- オピオイド、鎮静抗ヒスタミン薬、または神経弛緩薬などの薬物の使用;
- 疲労に関連する医学的問題:慢性閉塞性肺疾患、うっ血性心不全、腎疾患、肝機能障害、制御不能な自己免疫疾患、多発性硬化症やパーキンソン病などの神経疾患、睡眠時無呼吸

除外基準:
- マッサージ介入の性質上、一度に 1 時間仰臥位で横になることができない
- 体格指数 18.5 (kg/m^2) 未満
- -コルチコステロイドまたは他の免疫抑制剤による治療= <登録の6か月前
- -登録の6年以上前に放射線を完了した被験者
- 何らかの理由でプロトコルを遵守できない被験者
- 非ステロイド性抗炎症薬や抗炎症作用があると考えられている天然物を含む抗炎症薬の定期的な使用
- -不安またはうつ病の処方薬の処方量の変更= <登録の4週間前。
- 登録前=<8週間以内のフルオキセチン用量の変更
- -現在の物質使用診断の基準を満たす被験者、または統合失調症、うつ病、全般性不安障害、双極性障害、認知症、せん妄、または強迫性障害(OCD)の現在の診断
- 積極的に自殺または殺人を行っている被験者

- 違法薬物の使用
- 交代制勤務
- 現在のダイエット

実験的:スウェーデンマッサージ療法(SMT)
患者は45分間のSMTを週1回、6週間にわたって受けます。
|
セラピストは無香料クリームを使用し、身体の上で長くストロークを行うことを容易にします。
スウェーデンマッサージは、被験者がシートで覆われた状態で行われ、この技術は「ドレーピング」と呼ばれます。
身体の一部が露出され、マッサージされ、その後再び覆われてから別の部分に移動します。
研究プロトコル療法で使用される主要な技術は、エフルラージュ、ペトリサージュ、揉捏法、タポトメント、および親指摩擦です。
これらの技術は、非常に正確で、注意深く練られた方法で実行されます。
セッションは、被験者がマッサージテーブル上で腹臥位で完全に覆われた状態で始まり、約22分後に被験者は仰向けになるよう指示されます。
最後に、セラピストは被験者の頭部エリアに移動し、エフルラージュと親指摩擦を使用して肩、首、頭の作業を開始し、頭部に軽いタポトメントを使用して終了します。

多次元疲労インベントリー(MFI)-20は、疲労の5つの次元(一般的な疲労、身体的疲労、意欲の低下、活動の低下、精神的疲労)を評価するために設計された20項目の尺度です。参加者は各項目を、1「はい、それは真実です」から5「いいえ、それは真実ではありません」までの5段階リッカート尺度で評価します。 このアウトカム指標では、5つのサブスケール(一般的な疲労、身体的疲労、活動の低下、意欲の低下、精神的疲労)と総合生スコアを報告します。すべてのスコアは1〜5の範囲で、スコアが高いほど疲労が強く、スコアが低いほど疲労が少ないことを示します。 |
