상세 설명
기본 목표:
I. 전립선암(PCa) 중 6주간의 스웨덴식 마사지 요법(SMT) 중재가 Light Touch(LT) 또는 Wait List Control(WLC)보다 암 관련 피로(CRF)를 유의하게 감소시키는지 여부를 평가하기 위한 예비 연구 수행 방사선 치료와 안드로겐 차단 치료를 모두 받은 생존자.
2차 목표:
I. SMT로 인한 암 관련 피로의 가설적 감소가 만성 염증의 기저에 있는 특정 면역계 요인의 조절과 관련이 있는지 여부를 평가합니다.
탐구 목표:
I. SMT가 자가 보고한 피로, 삶의 질, 우울증 및 불안을 LT 또는 WLC보다 유의하게 개선하는지 평가합니다.
II. 개입 후 6주 및 12주에 CRF에 대한 6주 SMT 대 LT의 지속적인 조치에 대한 예비 데이터를 수집합니다.
개요: 환자는 3개 팔 중 1개로 무작위 배정됩니다.
I군: 환자는 6주 동안 매주 1회 45분 동안 SMT를 받습니다.
Arm II: 환자는 6주 동안 매주 1회 45분 동안 LT를 받습니다.
3군: 환자는 6주 동안 개입을 받지 않습니다.

설명
포함 기준:
- 45세 이상 남성 피험자
- 조직학적으로 확인된 전립선암 진단
- 피험자는 방사선 요법을 완료해야 합니다 >= 2개월, 그러나 =< 등록 전 6년
- 혈청 테스토스테론 수치 < 20 ng/ml로 안드로겐 저하 요법을 받는 피험자
- 스크리닝 시 단기 피로도(BFI) 점수 > 25인 피험자
- 영어 말하기 및 읽기에 능통한 과목
국제 질병 분류(International Classification of Diseases, ICD)-10 제안 기준에 따라 환자는 병력, 신체 검사 및 피로가 암 또는 암 치료의 결과이며 주로 결과가 아니라는 실험실 결과의 증거와 함께 CRF 진단을 받아야 합니다. 다음 중 하나:
- 동반이환 정신 장애
- 빈혈(헤모글로빈 10g/dl 미만)
- 갑상선 기능 저하증(4.6 마이크로 국제 단위(uIU)/mL보다 큰 갑상선 자극 호르몬(TSH))
- 제어되지 않는 통증
조사자의 이력, 신체 검사 및 평가에 기초하여 임상적으로 피로에 기여한다고 느껴지는 모든 의학적 또는 정신과적 상태 또는 약물. 이러한 의료 상황에는 다음이 포함될 수 있습니다.
- 오피오이드, 진정 항히스타민제 또는 신경이완제와 같은 약물 사용;
- 피로와 관련된 의학적 문제: 만성 폐쇄성 폐질환, 울혈성 심부전, 신장 질환, 간 기능 장애, 조절되지 않는 자가 면역 질환, 다발성 경화증 또는 파킨슨병과 같은 신경 장애, 수면 무호흡증
- 정보에 입각한 동의를 제공할 수 있고 연방 및 기관 지침을 준수하는 승인된 동의서에 기꺼이 서명할 수 있습니다.
제외 기준:
- 마사지 개입의 특성상 한 번에 한 시간 동안 반듯이 누울 수 없음
- 체질량 지수 18.5(kg/m^2) 미만
- 코르티코스테로이드 또는 기타 면역억제제 치료 = < 등록 전 6개월
- 방사선을 완료한 피험자 >= 등록 전 6년
- 어떤 이유로든 프로토콜을 준수할 수 없는 피험자
- 비스테로이드성 항염증제 및 항염증 특성이 있는 것으로 생각되는 천연물을 포함한 항염증제의 정기적인 사용
- 불안 또는 우울증에 대한 약물 처방 용량 변경 =< 등록 4주 전.
- 등록 전 =< 8주 이내의 플루옥세틴 용량 변경
- 현재 물질 사용 진단 또는 정신분열증, 우울증, 범불안장애, 조울증, 치매, 정신 착란 또는 강박 장애(OCD)의 현재 진단 기준을 충족하는 피험자
- 능동적으로 자살 또는 타살하려는 피험자
치료 조사자의 의견에 따라 다음을 포함하여 연구 참여에 부정적인 영향을 미칠 수 있는 기타 상태 또는 행동:
- 불법 약물 사용
- 교대 근무
- 현재 다이어트
- 과도하고 규칙적인 음주
- 모든 폭음(24시간 동안 7잔 이상) =< 등록 6개월 전
- 피로 치료를 위한 마사지의 현재 및/또는 과거 사용.
- 의학적, 정신과적, 인지적, 또는 피험자의 피험자 정보 이해, 사전 동의 제공, 연구 프로토콜 준수 또는 연구 완료 능력을 손상시킬 수 있는 기타 상태

실험적: 스웨덴식 마사지 요법 (SMT)
환자는 6주 동안 주 1회 45분 동안 SMT를 받습니다.
|
치료사는 몸 전체에 긴 스트로크를 하기 위해 무향 크림을 사용합니다.
스웨디시 마사지는 대상자가 시트로 덮인 상태에서 이루어지며, 이 기법을 "드레이핑"이라고 합니다.
몸의 한 부분을 드러내어 마사지를 한 후 다시 덮고, 다른 부분으로 이동합니다.
연구 프로토콜 치료에서 사용되는 주요 기법은 에플뢰라주, 페트리사주, 니딩, 타포트망 및 엄지 마찰입니다.
이러한 기법들은 매우 정확하고 신중하게 세밀하게 수행됩니다.
세션은 대상자가 마사지 테이블에 엎드린 자세로 완전히 덮인 상태에서 시작되며, 약 22분 후 대상자는 등을 대고 누운 자세로 돌아가도록 지시받습니다.
마지막으로 치료사는 대상자의 머리 부분으로 이동하여 어깨, 목 및 머리에 에플뢰라주와 엄지 마찰을 사용하여 작업을 시작하고, 머리에 가벼운 타포트망을 사용하여 마무리합니다.
전체 마사지의 총 소요 시간은 45분입니다.

다차원 피로도 인벤토리(MFI)-20은 일반 피로, 신체 피로, 동기 저하, 활동 저하, 정신적 피로 등 피로의 5가지 차원을 평가하기 위해 설계된 20문항 척도입니다. 참가자는 각 문항에 대해 1점 "예, 그렇다"부터 5점 "아니오, 그렇지 않다"까지의 5점 리커트 척도로 평가합니다. 이 결과 측정치는 5개의 하위 척도(일반 피로, 신체 피로, 활동 저하, 동기 저하, 정신적 피로)와 총 원점수를 보고합니다. 모든 점수는 1-5점 범위이며, 높은 점수는 더 많은 피로를 나타내고 낮은 점수는 더 적은 피로를 나타냅니다. |
