この研究の目的は、結腸直腸癌肝転移患者から癌の患者由来モデル (すなわち、MOS) を作成する可能性を評価することです。 この研究の研究者は、CRC 肝転移患者に対する精密医療戦略に焦点を当てています。
参加する被験者は患者の肝臓病変の生検を受け、CRC肝転移の診断は病理学によって検証されます。 患者は、担当医によって決定された標準治療で治療されます。 標準治療の一環として、患者はCEA(癌胎児性抗原)や胸部、腹部、骨盤のCT検査を含む臨床検査を受け、ステージングと腫瘍サイズの測定を2か月ごとに行います。
最大250人の患者が研究に登録されます。 登録された被験者は、インフォームド コンセントを与える被験者として定義されます。 スクリーニング失敗は、インフォームド コンセントを与え、適格基準を満たさない被験者として定義されます。 発生した被験者は、インフォームド コンセントを提供し、適格基準を満たす被験者として定義されます。 評価可能な被験者は、発生し、試験生検を受け、細胞学的または組織学的に確認された、肝臓に転移した結腸または直腸の腺癌を有する者として定義される。 生検組織が結腸または直腸の腺癌であることが確認されない場合、被験者は評価不能と見なされ、交換されます。
MOSは研究生検から生成され、標準治療に対する感受性を決定するために薬物スクリーニングが行われます。 この方法は、腫瘍生検サンプルから MOS を生成し、標準治療に対する感度をテストする成功率が 75% であることが証明されています。

主な採用基準:
- 被験者は、有効な治験審査委員会(IRB)/倫理委員会(EC)が承認したインフォームドコンセントフォームに署名しました。
- -書面によるインフォームドコンセントが得られたときの18歳以上の男性または女性。
- -研究候補者は、プロトコルに必要なすべての手順と評価を喜んで遵守することができます。
研究候補のいずれか
- 肝臓の生検が予定されている、または予定されている、または
- -肝臓に転移している結腸および/または直腸の腺癌の組織学的または細胞学的に確認された以前の診断がある。
主な除外基準:
- 肝臓の生検がすでに予定されている場合: 結腸直腸癌とは無関係の疾患 (例: 非アルコール性脂肪肝疾患、慢性 B 型または C 型肝炎、自己免疫性肝炎、アルコール性肝疾患、原発性胆汁性肝硬変、原発性硬化性胆管炎、ヘモクロマトーシス、ウィルソン病など)。
研究候補者は、別の臨床研究に参加している (または参加する予定である) かつ以下のいずれか:
- -現在、その研究の一環として肝臓に転移している結腸および/または直腸の腺癌に対する治験中の全身療法を受けている、または
- その研究では、肝臓に転移した結腸および/または直腸の腺癌に対する治験的全身療法のみが許可されます (被験者がすでに無作為化されているか、または無作為化される可能性がある場合、臨床試験の標準治療対照群への同時参加は許可されます)コントロールアーム)。
- -神経内分泌結腸直腸がんの組織学的または細胞学的診断が確認されている。

-肝臓に転移している結腸および/または直腸の腺癌が疑われる肝生検を受けている患者。
|
MOS と呼ばれる患者由来の癌モデルは、肝臓に転移した結腸および/または直腸の被験者の腺癌の生検から生成され、使用される標準治療薬を使用した薬物スクリーニングが完了します (オキサリプラチン、イリノテカン、5-FU/カペシタビン(ゼローダ)、ベバシズマブ、パニツムマブまたはセツキシマブ、トリフルリジン/チピラシル(ロンサーフ)、レゴラフェニブおよびペムブロリズマブまたはニボルマブ)。
被験者は、担当医によって指示されたCRCの標準治療を受けます。
