상세 설명
이 연구의 목적은 결장직장암 간 전이 환자로부터 암의 환자 유래 모델(즉, MOS)을 생성하는 타당성을 평가하는 것입니다. 이 연구의 연구자들은 CRC 간 전이 환자를 위한 정밀 의학 전략에 초점을 맞춥니다.
참여 피험자는 환자의 간 병변 생검을 받고 CRC 간 전이의 진단은 병리학에 의해 확인됩니다. 환자는 치료 의사가 결정한 표준 치료 요법으로 치료됩니다. 표준 치료 요법의 일환으로 환자는 2개월마다 종양 크기의 병기 결정 및 측정을 위해 CEA(암 배아 항원) 및 흉부, 복부 및 골반 CT를 포함한 실험실 작업을 받게 됩니다.
최대 250명의 환자가 연구에 등록됩니다. 등록된 피험자는 정보에 입각한 동의를 한 피험자로 정의됩니다. 화면 실패는 정보에 입각한 동의를 제공하고 자격 기준을 충족하지 않는 피험자로 정의됩니다. 누적 피험자는 정보에 입각한 동의를 제공하고 적격성 기준을 충족하는 피험자로 정의됩니다. 평가 가능한 피험자는 축적되어 연구 생검을 받고 간으로 전이되는 결장 또는 직장의 세포학적 또는 조직학적으로 확인된 선암종을 갖는 자로 정의됩니다. 생검 조직이 결장 또는 직장의 선암종으로 확인되지 않는 경우 대상자는 평가할 수 없는 것으로 간주되어 교체됩니다.
연구 생검에서 MOS가 생성되고 표준 치료 요법에 대한 민감도를 결정하기 위해 약물 스크리닝이 수행됩니다. 이 방법은 MOS 생성 성공률이 75%이고 종양 생검 샘플에서 표준 치료 요법에 대한 민감도를 테스트하는 것으로 입증되었습니다.

- 간 생검이 이미 예정된 경우: 간 생검은 대장암과 관련이 없는 질병(예: 비알코올성 지방간 질환, 만성 B형 또는 C형 간염, 자가면역 간염, 알코올성 간질환, 원발성 담즙성 간경변증, 원발성 경화성 담관염, 혈색소침착증, 윌슨병 등).
연구 후보가 다른 임상 연구에 참여 중(또는 참여하려고 함) 그리고 다음 중 하나:
- 현재 해당 연구의 일부로 간으로 전이된 결장 및/또는 직장의 선암종에 대한 연구용 전신 요법을 받고 있거나

간으로 전이된 결장 및/또는 직장의 선암종 생검을 통해 MOS라고 하는 환자 유래 암 모델이 생성되고 사용된 표준 치료 약물(옥살리플라틴)을 사용한 약물 스크리닝이 완료됩니다. , 이리노테칸, 5-FU/카페시타빈(Xeloda), 베바시주맙, 파니투무맙 또는 세툭시맙, 트리플루리딘/티피라실(Lonsurf), 레고라페닙 및 펨브롤리주맙 또는 니볼루맙).
