集中治療室で実施されたクロスオーバー無作為化臨床試験。
人工呼吸器を24時間以上使用している患者が研究に含まれます。
次の技術が適用されます: エア スタッキング技術を使用した人工呼吸器を使用した過膨張操作に関連する 10 秒間の呼気休止を伴う閉鎖システムでの吸引、および 10 秒間の呼気休止を伴う閉鎖型吸引システムでの吸引技術。
調査の概要
詳細な説明
無作為化後、すべての患者は頭を 30 度上げた状態で背臥位になり、閉鎖吸引システムと 150 cmH2O に制限された真空で 1 回吸引されます。
2 時間後、血行動態と換気のパラメーターが収集され、記録されます。
プロトコル 1 (エア スタッキング テクニック) に無作為化された場合、クローズド システムと呼気休止を伴う吸引テクニックは、同じ口径と同じ真空値のプローブを使用して、30 秒間隔で 3 回実行されます。 生理食塩水 1 ml を吸引システムに注入して、閉じた吸引回路を洗浄します。
その直後に、エアスタッキング操作が実行されます。 プロトコル 2 (No Air Stacking 法) の場合は、クローズド システムでの気管吸引のみ。
適用の1分、10分および30分後に血行動態および換気パラメータを収集する。 コレクション フラスコに吸引された分泌物は、研究の一部ではない盲検化された協力者によって精密天秤で計量され、その重量がデータ収集に転写されます。

- 48 時間以上人工呼吸器を使用している患者
- 従量式または従圧式換気モードの患者
- -血行動態が安定している患者(平均血圧が等しいか60 mmHg以上、およびノルエピネフリンの投与量が1μg / Kg /分未満)
- 法定代理人が研究への参加を承認した患者。
除外基準:
- 排出されていない気胸と血胸、および皮下気腫
- 肋骨の骨折
- ピーク圧が 40 cm/H2O を超える換気パラメータ
