詳細な説明
この調査には 3 つの主な目的があります。1) ケア ナビゲーターの労働力を創出および維持するために何が必要かを学び、調査のためにケア ナビゲーターを採用した経験を文書化する、2) KPNC 内で AURORA 戦略を実施および評価する、3) 改良および適応する将来のカイザー パーマネンテ (KP) の普及に向けた AURORA 戦略。
AURORA 戦略は、早発性 2 型糖尿病 (T2D、45 歳未満と定義) のラテン系成人に合わせた 3 回の遠隔医療訪問を積極的に実施するケア ナビゲーターを中心としています。
AURORA 戦略は、次の 5 つの目標で実施されます。1) KP 内での T2D ケアの親しみやすさと使用の評価、2) 個人の自己管理サポートのニーズの評価、3) 行動の変化をサポートするための動機付けの面接、4) 実用的なスキルのトレーニング、自己管理をサポートするための重要なテクノロジーの使用、および 5) メンバーのニーズを既存の KPNC T2D リソースと一致させることを含みます。

- KPNC メンバー、20 歳以上 45 歳未満
- -過去1〜8か月以内のT2Dの新しい臨床診断
- HbA1c≧8%。 研究チームが適切なサンプルを募集するのに問題がある場合、研究チームは HbA1c 閾値を 7.5% 以上に下げることがあります。
- 英語またはスペイン語に堪能(筆記および会話)
