静脈血栓塞栓症 (VTE) とは、腕や脚 (深部静脈血栓症 [DVT])、腹部 (門脈血栓症)、肺 (肺塞栓症 [PE]) などの身体の静脈に形成される血栓を指します。 これらの血栓は、抗凝固薬と呼ばれる血液凝固を軽減する薬で治療されます。 抗凝固薬の主な合併症は出血で、その大部分は胃または腸(消化管)から起こります。 抗凝固剤は出血を引き起こしませんが、出血を悪化させる可能性があります。 まれに、重篤な胃腸(GI)出血が発生し、入院や死に至る場合もあります。 出血の可能性が最も高くなるのは、抗凝固薬の投与を開始してから最初の数か月です。
プロトンポンプ阻害剤 (PPI) は、胃酸含有量を低下させる薬です。 この研究で使われた薬剤は、オメプラゾールと呼ばれるプロトンポンプ阻害剤の一種で、カナダでは胃潰瘍、胸やけ、ヘリコバクター・ピロリと呼ばれる胃感染症の治療薬として承認されている。 この研究におけるオメプラゾールの使用は研究段階であると考えられています。 これは、カナダ保健省が抗凝固薬を服用している患者の胃腸出血を防ぐための治療法としてオメプラゾールの使用を承認していないことを意味します。 いくつかの研究では、抗凝固薬を服用している人の胃腸出血を軽減する可能性があることが示唆されています。
研究者らは、オメプラゾールと呼ばれるプロトンポンプ阻害剤による治療が、静脈血栓塞栓症のために抗凝固薬を服用している高齢者の胃腸出血を軽減するかどうかを研究している。
研究者らは、治療の効果をテストする最良の方法である大規模なランダム化試験を実施する予定だ。 これを行うために、この研究の参加者の一部はオメプラゾールを投与され、他の参加者はプラセボ(研究のオメプラゾールに似ているが有効成分も薬効成分も含まない物質)を投与されます。 この研究のプラセボは出血に対する効果を意図したものではありません。 プラセボは研究結果の信頼性を高めるために使用されます。
主な目的 急性 VTE に対して抗凝固療法を受けている高齢者において、オメプラゾールがプラセボと比較して上部消化管出血のリスクを軽減するかどうかを判定する、本格的な二重盲検プラセボ対照ランダム化試験の実現可能性を評価すること。

包含基準:
- 入学時に65歳以上の男女。 年齢は出血の重要な変更不可能な危険因子であるため、登録は高齢者に限定されます。 これにより、すべての参加者が出血の危険因子を少なくとも 1 つ持っているため、(危険因子を持たない参加者と比較して)オメプラゾールの恩恵を受ける可能性が高い参加者が研究対象集団に含まれることが保証されます。
- 過去7日以内に診断された急性VTE。これには、上肢または下肢のDVT、PE、脳静脈血栓症、門脈血栓症、その他の内蔵静脈血栓症など(ただしこれらに限定されない)、任意の部位のVTEが含まれます。
- 抗凝固剤による抗凝固療法を3か月(90日)以上行う計画がある。
- 患者または代理人は、同意書に記載されているフォローアップ検査に従うことができ、また従う意思があること。
除外基準:
- 現在、定期的に毎日使用するために PPI が処方されている (H2 受容体拮抗薬を投与されている患者は除外されない)、
- 以前の上部消化管出血、
- 二重抗血小板療法の必要性、
- オメプラゾールに対する禁忌(オメプラゾールまたは他の置換ベンズイミダゾール PPI に対する過敏症、リルピビリンを含む製品との併用、施設調査員の裁量による)、
- 余命は3か月未満です。
