転移性再発乳がんにおけるBRIA-OTS細胞免疫療法の研究
これは非盲検の第 1/2a 相試験です。 フェーズ 1 で BC1 細胞株単独の安全性が証明されたら、フェーズ 2 では、患者は Bria-OTS レジメン (下記参照) と臨床的に利用可能なチェックポイント阻害剤 (CPI) で治療されます。

画像検査で進行性疾患を発症した患者は、治験責任医師が臨床的利益を得ていると感じ、合理的で有意義な臨床代替療法が利用できない限り、治療を継続することができます。 対象者は、その後の治療(PFS2)および生存期間について、電話または医療記録の確認によって3か月ごとに最長2年間追跡され続ける。
研究薬物、投与量、および投与方法:
フェーズ 1 のパート 1 単独療法フェーズは、各患者に対して次のように進行します。

最初に、これら 3 つの被験者について安全性が評価されます。 DLT は、研究治療に関連する可能性が疑われる CTCAE グレード 3 または 4 の有害事象として定義されます。
第 1 相被験者 3 名中 1 名が DLT を経験した場合、併用相が始まる前にその用量コホートが別の 3 名の患者に拡大されます。 拡張コホート内の患者の中に DLT を経験した患者がいない場合(DLT 率 1/4)、研究は Bria-OTS と CPI の併用に移行します。 組み合わせの用量は、最初の 3 人の患者の間で DLT が観察されない最高用量、または 3 人の患者の拡張コホート用量レベルのいずれかになります。 フェーズ 1 では、最初の予定された評価まで、BC1 の併用量を決定するための DLT が観察されます。 第 1 相 DLT 率が 2/4 以上の場合、さらに 3 人の患者に、次に低い用量レベルまたは 1,000 万細胞(どちらか高い方)を使用して投与されます。 3 人の被験者のいずれも用量制限毒性 (DLT) を経験していない場合、その決定された BC1 MTD 用量は、CPI とともにフェーズ 1 のパート 2 併用フェーズに含まれます。 BC1 細胞株の耐用量が特定できない場合、さらなる調査は一時停止され、データが再検討され、プロトコールの修正と治験審査委員会の承認があった場合にのみ、より低い用量で再開することができます。

3 人の患者が Bria-OTS レジメンと CPI で 2 サイクル安全に治療されたら、第 2 相では追加の最大 9 人の被験者で構成される拡大コホートが登録されます (合計 12 人が Bria-OTS レジメンと CPI で治療されます) )。

以前に失敗した再発性転移性乳がんが組織学的に確認された
治療法は次のように定義されます。
- ヒト上皮成長因子 2 (EGFR2、HER2) 陽性腫瘍は、少なくとも 2 種類の抗 HER2 薬による治療が失敗している必要があります
- HER2陰性かつERまたはPR陽性腫瘍:ホルモン療法に抵抗性であり、少なくとも2つのホルモンベースの標的療法を含むレジメンで以前に治療されている必要があります。
- トリプルネガティブおよび炎症性腫瘍の場合は、タキサンおよびプラチナベースの薬剤による事前の治療を含む、他の治癒目的の治療法を使い果たしている必要があります
- 他のすべてのMBCタイプは、利用可能な承認薬を有するゲノムまたは生殖細胞系標的療法を含む、他の治癒目的の治療法を使い果たしている必要があります。
- 脳に転移した新規または進行性乳がんの患者は、以下の条件を満たす場合に対象となります。
私。 脳転移は、初回投与前に少なくとも 4 週間臨床的に安定していなければなりません(画像診断による進行性疾患の証拠がない)。

患者は、以下を含む以前の治療による既知または予想される毒性がすべて安定していなければなりません。
- 以前の免疫関連毒性は、安定した内分泌障害を除き、グレード 2 を超えてはなりません (内分泌障害は、適切に管理されていれば除外されません)。
- -グレード1以下またはベースラインまで回復していない以前の治療の毒性(あらゆるグレードの脱毛症、適切に治療された内分泌障害、および輸血支援を必要としない貧血を除く)。
除外基準:
- 抗がん剤治療も同時並行。
- 最初のプロトコル治療から 3 週間以内に最近の化学療法、放射線療法、またはその他の抗がん治療を受けている。
- 参加者は治験薬の開始前に外科的介入による毒性および/または合併症から十分に回復していない。
- プロトコールに指定されている指定された治療法、またはこの研究で細胞株の調製に使用された成分に対する臨床的過敏症の病歴。
- プロトコルに指定された治療法に対する臨床的過敏症の病歴。
- BUN >30 とクレアチニン >2、または計算クレアチニン クリアランス (CrCl) <30 mL/min (クレアチニンまたは CrCl の代わりに GFR を使用できます)。
- 絶対顆粒球数 < 1000;血小板<50,000。

- 中程度以上の重症度の胸水または心膜液。
- 妊娠の可能性のある女性(つまり、過去1年以内に月経周期があり、避妊手術を受けていない)。ただし、以下の場合を除きます。 研究期間中に妊娠を避けるために適切な予防措置を講じることに同意し、7日以内に血清妊娠検査結果が陰性である場合。治療を始める前に。
- 生殖能力/生殖能力のある男性は、研究期間中、子供の父親になることを避けるために適切な予防措置を講じる必要があります。
- 妊娠中または授乳中の女性。
- 二次悪性腫瘍を併発している患者。
- 過去24か月以内に治療が必要な悪性腫瘍を患っている人。
- AIDSを示す臨床的または検査上の特徴があり、HIV陽性である患者(自己申告による)。
- -免疫不全と診断されている、または治験治療の初回投与前の21日以内に慢性全身ステロイド療法(プレドニゾン換算で1日あたり10mgを超える用量)またはその他の形態の免疫抑制療法を受けている。
- 治験責任医師および治験依頼者によって特に承認されていない限り、自己免疫疾患の治療を受けている患者。
- 治験責任医師および治験依頼者の承認がない限り、重度の精神疾患(統合失調症、双極性障害、境界性パーソナリティ障害など)またはその他の臨床的に進行性の重大な医学的問題を抱えている患者。
- 治験責任医師および治験依頼者によって承認されない限り、患者は同時に臨床試験に参加することはできません。
