大血管閉塞による急性脳卒中患者に対する直接虚血ポストコンディショニングの有効性と生物学的メカニズム:ランダム化対照パイロット試験
この臨床試験の目的は、直接虚血ポストコンディショニング (IPostC) が血管内血栓除去術 (EVT) を受けた患者の虚血再灌流傷害 (I/R) を軽減できるかどうかを判断することです。 さらに、この研究は、直接 IPostC の基礎となるメカニズムを調査することを目的としています。

参加者は、EVT 単独グループまたは EVT と直接 IPostC グループの 2 つのグループのいずれかにランダムに割り当てられます。 直接 IPostC は、再灌流の機械的中断の 4 サイクルを通じて EVT 直後に投与されます。 最終的な梗塞体積、梗塞体積の増加、臨床パラメータ、I/R関連の画像および臨床検査バイオマーカーに基づいて結果を評価します。 さらに、直接 IPostC の詳細なメカニズムを解明するために、探索的なマルチオミクス解析が実施されます。

包含基準:
- 虚血性脳卒中はCTまたはMRIによって確認されます。
- 頭蓋内内頚動脈 (ICA) および中大脳動脈 (MCA M1/M2) を含む、CTA または MRA によって確認された大血管閉塞。
4. eTICIグレード2b/3の閉塞血管の再開通。血栓除去後のDSAによって確認。
5. 患者または法的に権限を与えられた代理人がインフォームドコンセントフォームに署名していること。
除外基準:
- 何らかの理由で MRI または CT スキャンを実行できない。
- 神経学的評価または精神障害を妨げる何らかの状態の存在。
- 発作を伴う脳卒中の発症により、正確な NIHSS ベースラインを取得できなくなります。
- 妊娠。
- 他の重篤な、進行した、または末期の病気の存在。

血栓除去術のみ。
血栓除去術後、閉塞部位でバルーンを 4 atm 以下の圧力まで膨張させ、2 分間血流を遮断したことが血管造影で確認されました。
次にバルーンを収縮させ、血流を 2 分間再開させました。
