カルベジロール非反応者における食道静脈瘤出血の一次予防のためのカルベジロールと内視鏡的結紮の有無:ランダム化比較試験
この臨床試験の目的は、カルベジロール非反応例において、カルベジロールと内視鏡的静脈瘤結紮術(EVL)が、食道静脈瘤出血の一次予防において、カルベジロール単独よりも効果的であるかどうかを評価することです。 また、肝硬変患者におけるカルベジロールとEVLの併用の安全性についても学ぶ予定です。
回答することを目的とした主な質問は次のとおりです。
カルベジロールに反応しない肝硬変患者の最初の食道静脈瘤出血の予防において、カルベジロールとEVLの併用が、カルベジロール単独よりも効果的かどうか。
カルベジロールを服用している場合、またはカルベジロールとEVLを併用している場合、参加者はどのような医学的問題を抱えていますか? 研究者らは、カルベジロールに反応しない肝硬変患者の最初の食道静脈瘤出血の予防における、EVLの有無にかかわらずカルベジロールの有効性と安全性を比較する予定である。
参加者は次のことを行います:
カルベジロールを毎日服用し(6.25 mg/日から開始し、許容できる場合は12.5 mg/日まで増量)、静脈瘤が根絶されるまで3~4週間ごとにEVLを受け、その後プロトコールに従って定期的に内視鏡によるフォローアップを行うか、または毎日カルベジロールを単独で服用します。 1 日 (6.25 mg/日から開始し、許容できる場合は 12.5 mg/日まで増量します)。
2~3ヶ月に1回程度の頻度でご来院いただき、検査・検査を行ってください。 症状だけでなくバイタルサイン(SBP、DBP、HR)も日記に記録してください。

胃食道静脈瘤出血は門脈圧亢進症の主要な合併症であり、再出血率と死亡率が高くなります。 肝硬変および門脈圧亢進症の患者における最初の食道静脈瘤出血(EVB)をどのように防ぐかは、常に非常に重要な問題です。 2021年12月に発表された門脈圧亢進症の治療に関する最新のバヴェーノVIIコンセンサスによると、臨床的に重大な門脈圧亢進症患者の代償不全を予防するには、まず非選択的β遮断薬(NSBB)を使用する必要がある。 内視鏡的静脈瘤結紮術(EVL)は、NSBB に対する禁忌または不耐性のある高リスク食道静脈瘤(EV)の補償患者に推奨されます。
カルベジロールは NSBB の一種であり、特別な固有の抗α1 アドレナリン活性を持っています。 カルベジロールは、プロプラノロールよりも肝静脈圧勾配(HVPG)を低下させることができ、現在、肝硬変およびCSPH患者におけるEVBの一次予防に選択される薬剤です。 カルベジロールの HVPG 低下効果はプロプラノロールよりも強力ですが、CSPH 患者の最大 38 ~ 46% は依然としてカルベジロール非反応者です。 これらの患者の追跡上部消化管内視鏡検査では、EVが大きくなったり、数が増えたり、新たな赤い色の兆候が現れたりする可能性があり、これはEVBのリスクの増加を示しています。 しかし、カルベジロール療法に反応しない肝硬変患者およびEV患者の治療選択肢について議論した研究はほとんどありません。
このプロジェクトでは、研究者らは、カルベジロール非反応例である肝硬変患者におけるEVBの一次予防におけるカルベジロールとEVLの併用の有効性を比較することを目的とした非盲検ランダム化対照試験を開始する予定である。 研究者らはまた、他の上部消化管出血、非出血性肝代償不全(腹水の新規発症/悪化、肝性脳症、自然発生性細菌性腹膜炎、肝腎症候群など)、全生存期間に関して2つのグループ間に違いがあるかどうかも調査する予定である。 、有害事象、忍容性、安全性。 研究者らはまた、カルベジロール非反応者であるさまざまな程度の肝硬変患者の最初のEVBの予防において、2つのグループに違いがあるかどうかも分析する予定である。

説明
包含基準:
- カルベジロール治療後も静脈瘤の拡大が続くか、新たな赤い色の徴候が発現する食道静脈瘤を有する肝硬変患者。
除外基準:
- 18歳未満または90歳以上。
- 食道静脈瘤出血の病歴。
- -内視鏡的静脈瘤結紮術、内視鏡的硬化療法、経頸静脈的肝内門脈大循環シャント、または手術などの食道静脈瘤の治療歴がある。
- 重度の房室ブロック、慢性閉塞性肺疾患(COPD)、喘息、コントロール不良の糖尿病、重度の末梢動脈疾患などの非選択的ベータ遮断薬の禁忌。
- 肝細胞癌やその他の末期癌、心不全、腎不全などの末期臓器疾患の存在。
- 妊娠中の女性。
- 研究への参加を拒否する。
