がんサバイバーの多くは、疲労、うつ病、筋力低下など、治療をはるかに超えた長期的な副作用に悩まされており、これが生活の質(QoL)の低下の一因となっています。 身体活動 (PA) は、全体的な QoL、がん特異的死亡率の改善、治療関連毒性の軽減などの臨床転帰にプラスの影響を与えます。 これらの利点にもかかわらず、がん生存者の大多数は、非がん生存者と同様に、公衆衛生の PA ガイドラインを満たしていません。 新型コロナウイルス感染症(COVID-19)による規制が緩和される中、がん生存者は新型コロナウイルス感染症(COVID-19)に感染した後の罹患率と死亡率が高くなるため、ニューノーマルの健康への影響は長期にわたる可能性がある。 しかし、がんサバイバーは、症状を自己管理するために、実質的にリアルタイムで質の高い魅力的なケアを受けることができることに気づき始めています。 これは、がんの臨床支持療法と研究試験の両方において遠隔ベースの介入をテストし、提供するまたとない機会となります。 短期の監視付きPAプログラムは、がん生存者の体力と参加者報告の転帰を改善する可能性があるが、PAは治療後に著しく低下し、長期のアドヒアランスは低いことが多い。 これは、利用可能なプログラムや、たとえ存在するとしても簡単にアクセスできる施設ベースの監視付きプログラムが不足していることも原因である可能性があります。 長期的な健康上の利益を達成するには、行動の変化を持続させる必要があります。 行動変容介入は、多くの相互作用するコンポーネントを含む複雑であり、特にメンテナンスの定義方法に関しては十分に説明されていないことがよくあります。 これは、PA のメンテナンスを容易にする可能性のある介入コンポーネントの理解を妨げます。 行動変容介入は介入の過程で PA を改善します。ただし、フォローアップ評価間の期間が長くなるにつれて、PA の低下はより顕著になります。 それにもかかわらず、理論的要素を含めることで、これらの介入における行動変化の可能性が高まります。
がん生存者向けの最近の PA ガイドラインでは、臨床上の効果を得るために、週に 90 分間の中等度から高強度の PA (MVPA) と、週に少なくとも 2 日間の筋力トレーニングを推奨しています。 PA 関連の利点は、がん生存者が PA を遵守し、維持する場合にのみ実現されます。 しかし、現在、がん生存者の 74.8% と 86.1% が、有酸素性 PA と複合 PA のガイドラインをそれぞれ満たしていません。 がん生存者が対面での PA に参加するにはいくつかの障壁 (例: 臨床/地域プログラムからの距離、治療関連の副作用) に直面していることを考慮すると、遠隔ベースのアプローチの有効性を開発し、評価する必要があります。 遠隔医療以外の在宅での運動やリハビリテーション介入と比較した、遠隔ベースの介入の質と有効性はまだ不明です。 PA の導入と維持を促進するための主要な動機付けの成果を特定するための理論的アプローチは限られています。 自己監視、目標設定、社会的サポート、行動計画などの行動変容テクニックが効果的なテクニックであることが示されています。 がん患者に対する行動理論を用いた介入は、行動変化に対する全体的な効果量が最大でした。 しかし、どの介入メディエーター(行動変容技術など)が長期的なPA遵守の原因となっているかについてはほとんどわかっていない。
PA およびがん生存者の研究における主要な理論的アプローチは、社会的認知理論です。 有益ではありますが、理論は意図と行動のギャップ メカニズムを制定することによる維持に焦点を当てることはほとんどありません。 マルチプロセス行動制御 (M-PAC) フレームワークは、個人が意図形成から行動制御の導入、そして行動制御の維持へと移行する因果構造を持っています。 M-PAC によれば、内省的プロセス(つまり、手段的態度 [PA を行うことで期待される利益]、感情的判断 [PA を行うことで期待される喜び]、知覚された能力 [PA を実行する自分の能力]、および知覚された機会 [知覚された社会/環境状況] PA を実行するための)は、がんサバイバーの PA 意図形成に必要です。 これらの期待が強く前向きである場合、それらは最終的に PA の意図の形成 (つまり、定期的な PA を制定する決定) に達します。 定期的な PA を開始する際の主要な決定要因は、規制プロセスの制定によって特徴付けられます。 調節プロセスは、意図を PA に変換するために実行される行動、認知、および感情の調節戦略 (計画、監視、注意の集中など) を表します。 最後に、再帰的プロセス (つまり、習慣 [学習された手がかりと行動の関連付け] とアイデンティティ [役割の自己分類] は、時間の経過とともに繰り返される成功した行動結果の結果として発達する構築物です。 したがって、M-PAC は時間の経過とともに相互に構築される反射的、規制的、および再帰的なプロセスを表しますが、それぞれが独自の独立した効果とともに、PA への何らかの媒介フィードバックを持つことが期待されます。 まとめると、行動の変化は、当初の意図を促進して継続的な行動を成功に導く反射的、規制的、再帰的なプロセスの産物です。

参加者は介入期間中、2週間ごとにQEPによる行動カウンセリングサポートセッションも受けます(合計12回)。
