EBC DCB研究は、調査官が開始し、前向き、多施設、非盲検、無作為化(1:1)非in菌試験です。
血行再建を必要とする左左の主要分岐患者と主容器と側面の両方の枝が大幅に病気にかかっています(≥2.5mmおよびメディナ1/1/1、1/0/1または0/1/1)は、ソース集団を構成します。 2つの追加冠動脈疾患を持つ患者は、研究のために考慮される可能性があり、分岐病変は1つだけ含まれています。
紹介する医師によって事前に選択された患者は、研究チームによる適格性のためにスクリーニングされます。 患者がすべての包含を果たし、除外基準を満たしていない場合、研究の目的とコースについて通知され、参加と書面によるインフォームドコンセントを求められます。 ヨーロッパの7か国と韓国で、合計750人の患者のために、20のサイトが関与します。 2年間の募集期間の後、試験には6か月、1年、3年、5年、8年の追跡期間が含まれます。 1年間のチェックポイントは、分岐指向の複合エンドポイント(BOCE)が非劣性について分析されるプライマリエンドポイントを構成します(非劣性が満たされた場合、優位性のさらなる分析)。

- MVとSBの両方の関与の両方を伴う真の非左主の分岐疾患(つまり、メディナ1/1/1、1/0/1、0/1/1)
少なくとも1つの急性または慢性冠症候群の原因となる分岐病変:
- ≥70%血管造影MVおよびSB径狭窄
- 虚血の陽性非侵襲的検査
- 虚血の陽性生理学
直径の狭窄症≥50%で記載されているように、重要なSBおよび次のすべて:
- SB長さ≥73mm
- SB直径≥2.5mm
- MVから遠位に出現する別のSBの欠如
- SBが対角線枝の場合、非支配的なcircirflex動脈。

- 18歳未満の患者
- STEMI <48時間

アクティブコンパレータ:段階的な暫定ステント戦略
冠動脈ガイドのワイヤは、主容器(MV)と側枝(SB)の両方に渡されます。 MVのステント留置は、SBの流れとアクセスを保持するためにSBで投獄されたワイヤーで行われます。 ステント径は、遠位MVの直径に従って選択されます。 ステントの展開に続いて、近位MVのステント部分は、完全な拡張と並置を実現するために、短い非準拠バルーン(POT技術)で拡張されます。 最後に、ポットテクニックの後、SBが再配線され、キスバルーンインフレ(KBI)がプラークやカリナシフトに対処し、ステントストラットを開くために行われます。 したがって、遠位MVおよびSBの直径の大きさの非準拠バルーンは、高圧で連続的に膨らんでおり、同時に低圧で同時に膨張します。 さらなるSB治療は、<Timi 3の流れ、重大な狭窄> 70%、または有意な解剖の場合にのみ必要です。 したがって、KBIを伴う側の容器ステント埋め込みが必須です。 |

実験的:薬物コーティングバルーン戦略
冠動脈ガイドのワイヤは、主容器(MV)と側枝(SB)の両方に渡されます。 残留狭窄症のままである場合、30%を超える場合、またはティミの流れ<3、または解剖の伝播、MVのステント留置が必須です。 それ以外の場合、パクリタキセル - ドラッグコーティングバルーン(DCB)が、準備されたSBで公称圧力で60秒間膨らみます。 残留狭窄症が50%を超えている場合、またはティミの流れ<3、または解剖の伝播が残っている場合、オペレーターはSBの長時間のバルーン拡張(1〜2分)またはステント埋め込みを決定します。 SB処理後、MVは、DCBで公称圧力のみで処理され、近位MVで1:1を60秒間サイズとして処理します。 結果が受け入れられない場合、MVステント留置は、バルーンのインフレとDCBまたはSBへのステント埋め込みでキスすることで続いて必須です。 MVの残留狭窄症が30%未満であり、Timi 3の流れがあり、解剖の伝播がない場合、手順は完了します。 SBに残留狭窄が50%未満であり、Timi 3の流れがあり、解剖の伝播がありません。 |
