De EBC DCB-studie is een door de onderzoeker geïnitieerde, prospectieve, multi-center, open-label, gerandomiseerde (1: 1) niet-inferioriteitstudie.
Patiënten met niet-linkse hoofdbifurcaties die revascularisatie vereisen en zowel het hoofdvat als de zijtak aanzienlijk ziek (≥2,5 mm en Medina 1/1/1, 1/0/1 of 0/1/1), vormen de bronpopulatie. Patiënten met niet meer dan twee extra coronaire ziekten kunnen worden overwogen voor het onderzoek, met slechts één bifurcatie -laesie.
Patiënten, vooraf geselecteerd door hun verwijzende artsen, worden gescreend op geschiktheid door het studieteam. Als patiënten alle inclusie vervullen en niet voldoen aan de uitsluitingscriteria, zullen ze worden geïnformeerd over het doel en de cursus van de studie en worden ze gevraagd om deelname en schriftelijke geïnformeerde toestemming. Er zullen 20 locaties worden betrokken bij 7 Europese landen en Zuid -Korea, voor in totaal 750 patiënten. Na een wervingsperiode van 2 jaar zal de proef een follow-upperiode omvatten na 6 maanden, 1 jaar, 3 jaar, 5 jaar en 8 jaar. Het 1-jarige controlepunt vormt het primaire eindpunt waarbij het bifurcatie-georiënteerde samengestelde eindpunt (BoCE) zal worden geanalyseerd op niet-inferioriteit (verdere analyse voor superioriteit, als niet-inferioriteit heeft gehaald).

Inclusiecriteria:
- Ware niet-linkse hoofdbifurcatieziekte met zowel MV- als SB-betrokkenheid (d.w.z. Medina 1/1/1, 1/0/1, 0/1/1)
Bifurcatie -laesie die verantwoordelijk is voor een acuut of chronisch coronair syndroom met ten minste één van de volgende:
- ≥70% angiografische MV- en SB -diameter stenose
- Positieve niet-invasieve tests voor ischemie
- Positieve coronaire fysiologie voor ischemie
Significante SB zoals beschreven door stenose met diameter ≥50% en alle volgende:
- SB -lengte ≥73 mm
- SB -diameter ≥2,5 mm
- Afwezigheid van een andere SB die distaal uit de MV komt
- Niet-dominante circumflex-slagader, als SB een diagonale tak is.
Uitsluitingscriteria
- Patiënten <18 jaar oud
- STEMI <48 uur
- Cardiogene schok
- Chronische totale occlusie waarbij doelwitbevestigingsvaten betrokken zijn
- In stent restenose
- Levensverwachting van de patiënt <12 maanden
- > 2 Andere coronaire laesies (doel of niet-doel) gepland voor behandeling
- Syntaxis score> 32
- Ploedplaatjestelling ≤50x109/mm3
- Linker ventriculaire ejectiefractie ≤20%
- Deelname aan een ander onderzoek naar onderzoek of apparaat
- Patiënt kan niet geïnformeerde toestemming geven

Actieve vergelijker: Stapsgewijze voorlopige stentstrategie
Coronaire geleidedraden worden doorgegeven in zowel het hoofdvaartuig (MV) als zijtak (SB). Stenting van de MV wordt ondernomen met een draad die in de SB is gevangengezet om SB -stroom en toegang te behouden. Stentdiameter wordt gekozen volgens de diameter van de distale MV. Na stentimplementatie wordt het stentgedeelte in de proximale MV verwijd met een korte niet-conforme ballon (pottechniek) om volledige uitbreiding en appositie te bereiken. Ten slotte wordt SB na de pottechniek opnieuw bedraad en wordt een kussenballoninflatie (KBI) ondernomen om een plaquette of carina -verschuiving aan te pakken en stentstruts te openen. Aldus worden niet-conforme ballonnen maat tot de diameter van de distale MV en SB opeenvolgend opgeblazen bij hoge druk en vervolgens tegelijkertijd bij lage druk. Verdere SB -behandeling is alleen vereist als <Timi 3 -stroom, significante stenose> 70% of significante dissectie. Aldus is een zijvatstentimplantatie gevolgd door KBI verplicht. |
Een percutane coronaire interventie wordt in beide armen gebruikt, waarbij het medicijn zotarolimus voor stentstrategie en paclitaxel voor ballonstrategie. Coronaire geleidedraden worden doorgegaan in zowel het distale hoofdvat (distale MV) als zijtak (SB) en de voorkeur van MV op een 1: 1 -basis is vereist. De apparaatcomponent verwijdt het vatlumen door percutane transluminale coronaire angioplastiek (PTCA), terwijl het medicijn bedoeld is om de proliferatieve respons geassocieerd met restenose te verminderen.
In de stapsgewijze voorlopige stentstrategie wordt stenting van de MV uitgevoerd met een draad in de SB.
Vervolgens worden niet-conforme ballonnen maat tot de diameter van de distale MV en SB opeenvolgend opgeblazen bij hoge druk en vervolgens tegelijkertijd bij lage druk (kussende balloninflatie; KBI).

Experimenteel: Drugscoate ballonstrategie
Coronaire geleidedraden worden doorgegeven in zowel het hoofdvaartuig (MV) als zijtak (SB). Als de resterende stenose> 30%of Timi -stroom <3 of dissectie voortplanting blijft, is de stenting van de MV verplicht. Anders wordt een paclitaxel-drug gecoate ballon (DCB) opgeblazen in de voorbereide SB gedurende 60 seconden bij nominale druk. Als de resterende stenose> 50%of Timi-stroom <3 of dissectie voortplanting blijft, zal de operator beslissen tussen langdurige ballondilatatie (1-2 minuten) of stentimplantatie, in de SB. Na SB -behandeling wordt MV alleen 60 seconden met een DCB behandeld met een DCB bij nominale druk, 1: 1 met de proximale MV. Als het resultaat onaanvaardbaar is, zijn MV -stenting gevolgd door kussen balloninflatie en DCB of stentimplantatie in SB, verplicht. De procedure is voltooid wanneer de MV resterende stenose <30%heeft, Timi 3 -stroom heeft en geen dissectie -verspreiding heeft; Wanneer de SB resterende stenose <50%heeft, Timi 3 -stroom heeft en geen dissectiepropagatie heeft. |

In de strategie van de geneesmiddelen gecoate ballon (DCB) wordt SB bereid met een ballon met een diameter van 1: 1, gevolgd door een 1: 1 -formaat DCB -inflatie gedurende 60 seconden bij nominale druk.
Vervolgens wordt MV opnieuw geëvalueerd en behandeld met DCB alleen bij nominale druk, 1: 1 met de proximale MV gedurende 60 seconden.
Na behandeling met geneesmiddelen gecoate ballon in het hoofdvat (MV) in de door geneesmiddelen gecoate ballonarm, als het resultaat onaanvaardbaar is in de MV, met ofwel Timi> 3 -stroom en/of dissectie propagatie, is MV -stent verplicht gevolgd met kussenballoninflatie ( KBI), waar niet-conforme ballonnen-maat van de diameter van de distale hoofd- en zijtak opeenvolgend worden opgeblazen bij hoge druk en vervolgens tegelijkertijd bij lage druk.
Als de cross-over stentimplantatie in de MV wordt gebruikt, wordt een verdere SB-behandeling met DCB- of stentimplantatie gebruikt, volgens de voorkeur van de operator.
