O estudo da EBC DCB é um estudo de não-inferioridade iniciado no investigador, prospectivo, multicêntrico, aberto, randomizado (1: 1).
Pacientes com bifurcações principais não esquerdas que requerem revascularização e o vaso principal e o ramo lateral doentes significativamente (≥2,5 mm e Medina 1/1/1, 1/0/1 ou 1/1/1), constituem a população de origem. Pacientes com não mais que duas doenças coronarianas adicionais podem ser consideradas para o estudo, com apenas uma lesão de bifurcação incluída.
Os pacientes, pré-selecionados por seus médicos referentes, serão examinados quanto à elegibilidade pela equipe de estudo. Se os pacientes cumprirem toda a inclusão e não atenderem aos critérios de exclusão, serão informados sobre o objetivo e o curso do estudo e serão solicitados a participação e consentimento informado por escrito. 20 locais estarão envolvidos, em 7 países europeus e na Coréia do Sul, para um total de 750 pacientes. Após um período de recrutamento de 2 anos, o estudo incluirá um período de acompanhamento em 6 meses, 1 ano, 3 anos, 5 anos e 8 anos. O ponto de verificação de 1 ano constituirá o endpoint primário em que o ponto de extremidade compósito orientado à bifurcação (BOCE) será analisado quanto à não menoridade (análise adicional da superioridade, se não a menos inferioridade atende).

Critérios de inclusão:
- Doença de bifurcação principal não-esquerda verdadeira com envolvimento de MV e SB (ou seja, Medina 1/1/1, 1/0/1, 0/1/1/1)
Lesão da bifurcação responsável por uma síndrome coronariana aguda ou crônica com pelo menos um dos seguintes:
- ≥70% MV angiográfico e estenose de diâmetro sb
- Testes positivos não invasivos para isquemia
- Fisiologia coronariana positiva para isquemia
SB significativo, conforme descrito pela estenose de diâmetro ≥50% e todos os seguintes:
- Comprimento SB ≥73 mm
- Diâmetro SB ≥2,5 mm
- Ausência de outro SB emergindo distalmente do MV
- Artéria circunflex não dominante, se SB for um ramo diagonal.

- Pacientes <18 anos
- Stemi <48 horas
- Choque cardiogênico
- Oclusão total crônica envolvendo vasos de bifurcação -alvo
- Restenose no stent
- Expectativa de vida do paciente <12 meses
- > 2 Outras lesões coronárias (alvo ou não-alvo) planejadas para tratamento
- Pontuação da sintaxe> 32
- Contagem de plaquetas ≤50x109/mm3
- Fração de ejeção do ventrículo esquerdo ≤20%
- Participação em outro estudo de medicamento ou dispositivo investigacional
- Paciente incapaz de dar consentimento informado

Comparador Ativo: Estratégia de stent provisória gradual
Os fios guia coronarianos são passados para o navio principal (MV) e o ramo lateral (SB). O stent do MV é realizado com um fio preso no SB para preservar o fluxo e o acesso SB. O diâmetro do stent é escolhido de acordo com o diâmetro do MV distal. Após a implantação do stent, a parte do stent no MV proximal é dilatada com um balão não compatível (técnica de maconha) para obter expansão e aposição completas. Finalmente, após a técnica da maconha, o SB é reconectado e uma inflação de balões de beijo (KBI) é realizada para abordar qualquer placa ou mudança de carina e para abrir o stent. Assim, balões não compatíveis dimensionados para o diâmetro do MV e SB distais são inflados sequencialmente a alta pressão e depois simultaneamente a baixa pressão. Um tratamento adicional de SB é necessário apenas se <timi 3 fluxo, estenose significativa> 70% ou dissecção significativa. Assim, um implante de stent de embarcação lateral seguido com KBI é obrigatório. |
Uma intervenção coronariana percutânea é usada em ambos os braços, com a droga Zotarolimus para estratégia de stent e paclitaxel para estratégia de balão. Os fios da guia coronariana são passados para o vaso principal distal (MV distal) e o ramo lateral (SB) e a predilação de MV em base 1: 1 é necessária. O componente do dispositivo dilata o lúmen do vaso por angioplastia coronária transluminal percutânea (PTCA), enquanto o medicamento se destina a reduzir a resposta proliferativa associada à reestenose.
Na estratégia de stent provisória gradual, o stent do MV é realizado com um fio preso no SB.
Em seguida, os balões não compatíveis dimensionados para o diâmetro do MV e SB distais são inflados sequencialmente em alta pressão e, simultaneamente, a baixa pressão (beijando a inflação do balão; KBI).

Experimental: Estratégia de balão revestido a drogas
Os fios guia coronarianos são passados para o navio principal (MV) e o ramo lateral (SB). Se a estenose residual permanecer> 30%, ou fluxo timi <3, ou propagação de dissecção, o stent do MV é obrigatório. Caso contrário, um balão revestido com drogas do paclitaxel (DCB) é inflado no SB preparado por 60 segundos à pressão nominal. Se a estenose residual permanecer> 50%, ou fluxo timi <3, ou propagação de dissecção, o operador decidirá entre a dilatação prolongada do balão (1-2 minutos) ou a implantação do stent, no SB. Após o tratamento com SB, a MV é tratada com um DCB apenas com pressão nominal, tamanho 1: 1 com o MV proximal, por 60 segundos. Se o resultado for inaceitável, o stent MV seguido com a inflação do balão e o implante de DCB ou stent no SB é obrigatório. O procedimento está concluído quando o MV possui estenose residual <30%, possui fluxo TIMI 3 e não tem propagação de dissecção; Quando o SB possui estenose residual <50%, possui fluxo TIMI 3 e não possui propagação de dissecção. |

Se o implante de stent cruzado no MV for usado, é usado um tratamento SB adicional com DCB ou implante de stent, de acordo com a preferência do operador.
