- 参加薬局のいずれかに来局する。 対象薬局は、コミュニティ薬局強化サービスネットワーク - NY(CPESN-NY)に登録されているか、Tops Pharmaciesと提携している必要があります。 薬局はHealthelinkおよび紹介パートナー(例:Wellconnected)と連携する能力を持ち、コミュニティヘルスワーカー研修を伴うSDoHスクリーニングプログラムを実施するスタッフ体制を有している必要があります。 薬局の所在地はエリー郡内であること
