急性虚血性脳卒中(AIS)は、世界中で死亡および障害の主要な原因の一つであり続けています。
静脈内血栓溶解療法と血管内治療により再灌流成功率が著しく向上したものの、臨床実践では予後を決定する3つの重要な課題が残っています。第一に、治療時間窓内に実際に再灌流療法を受けられる患者は限られた割合に留まります。第二に、再灌流が成功した場合でも、微小循環灌流不全、炎症カスケード、血栓再形成などの二次的損傷が依然として発生する可能性があります。
第三に、特に早期神経学的悪化と早期再発リスクといった急性期の変動が顕著であり、機能転帰の改善を直接制限しています。
3 頭蓋内アテローム硬化性狭窄(ICAS)は、中国における虚血性脳卒中の病因の最大50%を占めています。
その病理学的連鎖-"プラーク不安定性-血栓症-微小循環障害"-は急性期と亜急性期の両方に浸透し、早期再発と不良転帰に密接に関連しています。プロプロテインコンバータゼサブチリシン/ケキシンタイプ9(PCSK9)は、古典的に低密度リポ蛋白コレステロール(LDL-C)受容体の分解を促進し、LDL-Cレベルを上昇させることで、アテローム硬化症の進行を促進します。

患者は無作為割り付け法を用いて2群に無作為に割り付けられます:(1)試験群:注射用レカチシマブ450mg(3バイアル)を単回皮下投与。(2)対照群:注射用レカチシマブプラセボ450mg(3バイアル)を単回皮下投与。
各群には606名の患者が含まれます。
両群とも、試験薬に加えて標準的なガイドライン推奨治療を受けます。
長期有効性は、0時間、24時間(±2または±12時間)、7日(±2日)または退院時、90日(±7日)、1年時点での患者来院および評価を通じて評価されました。

- 入院時の NIHSS スコアが 4-25
- 画像検査(CTA/DSA/MRA)が脳卒中の原因として頭蓋内アテローム性動脈硬化症を支持し、以下の基準のいずれかを満たす:i. 責任血管が頭蓋内アテローム性動脈狭窄(ICAS)を示し、狭窄度が 50-99% である;ii. 責任血管が頭蓋内アテローム性動脈閉塞(ICAS-LVO)を示し、機械的血栓回収術による再開通が成功している(即時拡張脳梗塞内血栓溶解 [eTICI] グレード 2b50-3)

除外基準:
- 非アテローム性頭蓋内動脈狭窄(動脈解離、モヤモヤ病、全身性血管炎など)
- 心原性塞栓症の特定可能な原因(心房細動、機械弁、左心室血栓、卵円孔開存など)
- 画像所見と臨床症状が、このイベントの主要な病態生理が脳小血管疾患(穿通枝動脈閉塞/ラクナ梗塞など)により一貫していることを示唆する
- この虚血性イベント以前からの既存の障害(修正Rankinスケール ≥ 2 点)
- CT または MRI 所見が広範な脳梗塞を示唆する(例:ASPECTS スコア < 6 または梗塞体積 ≥ 70 ml)
- 今後3か月以内に血管ステント留置術を受けた、または受ける予定である
- 登録前3か月以内に発生した頭蓋内出血

The event must include imaging-confirmed intracranial hemorrhage, an increase of at least 4 points in the total NIHSS score compared with baseline or a previously recorded NIHSS score, and neurological deterioration that cannot be explained by a cause other than the intracranial hemorrhage.
Qualifying hemorrhage types include parenchymal hematoma type 1 or 2, remote intracranial hemorrhage, subarachnoid hemorrhage, intraventricular hemorrhage, or subdural hemorrhage.
