Este estudio es un ensayo clínico prospectivo, multicéntrico, doble ciego, aleatorizado y controlado con placebo, diseñado para evaluar si la administración temprana de inhibidores de PCSK9 puede mejorar eficazmente los resultados funcionales a los 90 días en pacientes con accidente cerebrovascular isquémico (AIS) asociado a estenosis aterosclerótica intracraneal (ICAS), evaluado principalmente mediante la Escala de Rankin modificada a los 90 días.

El accidente cerebrovascular isquémico agudo (AIS) sigue siendo una de las principales causas de muerte y discapacidad en todo el mundo.
Aunque la trombólisis intravenosa y las terapias endovasculares han mejorado significativamente las tasas de éxito de la reperfusión, tres desafíos críticos persisten en la práctica clínica que determinan el pronóstico: En primer lugar, solo una proporción limitada de pacientes puede recibir realmente terapia de reperfusión dentro de la ventana terapéutica; En segundo lugar, incluso con una reperfusión exitosa, aún pueden ocurrir lesiones secundarias como el fracaso de la perfusión microcirculatoria, las cascadas inflamatorias y la reformación del trombo.
En tercer lugar, las fluctuaciones de la fase aguda, particularmente el deterioro neurológico temprano y el riesgo de recurrencia temprana, siguen siendo prominentes, lo que limita directamente las mejoras en los resultados funcionales.
3 La estenosis aterosclerótica intracraneal (ICAS) representa hasta el 50% de las etiologías del accidente cerebrovascular isquémico en China.
Su cadena patológica-"inestabilidad de la placa-trombosis-deterioro microcirculatorio"-impregna tanto las fases aguda como subaguda, correlacionándose estrechamente con la recurrencia temprana y los malos resultados. La proproteína convertasa subtilisina/kexina tipo 9 (PCSK9) clásicamente promueve la degradación de los receptores de colesterol de lipoproteínas de baja densidad (LDL-C) y eleva los niveles de LDL-C, impulsando así la progresión de la aterosclerosis.
Más importante aún, la creciente investigación básica y traslacional sugiere que la PCSK9 puede no solo funcionar como una "proteína del metabolismo lipídico" sino también participar en procesos como la activación endotelial, las cascadas inflamatorias, la reactividad plaquetaria y la disfunción microcirculatoria.
Esto la posiciona como un eje potencial que conecta el eje "placa-trombo-inflamación".
En consecuencia, los inhibidores de PCSK9 pueden ofrecer beneficios neurovasculares protectores adicionales más allá de los efectos de reducción de lípidos durante la fase aguda del accidente cerebrovascular isquémico agudo (AIS).La evidencia básica y clínica existente sugiere que los inhibidores de PCSK9 pueden ejercer un efecto de intervención combinada en los procesos patológicos clave que impulsan la aterosclerosis durante la fase aguda del AIS a través de un mecanismo dual que involucra tanto vías dependientes de lípidos como no dependientes de lípidos.
Los estudios mecanicistas revelan que la inhibición de PCSK9 modula simultáneamente las respuestas inflamatorias, la activación endotelial, la disfunción y las tendencias trombogénicas.
En modelos de isquemia-reperfusión, demuestra señalización neuroprotectora al mitigar la lesión cerebral y mejorar la función neurológica.
Clínicamente, grandes ensayos aleatorizados confirman su capacidad para mejorar rápidamente los efectos de reducción de lípidos más allá de las estatinas y reducir el riesgo de accidente cerebrovascular isquémico.
Una evidencia traslacional adicional en enfermedad cerebrovascular indica que en poblaciones con ICAS sintomática, el inhibidor de PCSK9 más la terapia de reducción de lípidos potenciada con estatinas reduce la carga de placa, alivia la estenosis y aumenta la estabilidad de la placa.
En la fase aguda del AIS, la adición temprana de inhibidores de PCSK9 se correlaciona con una reducción del deterioro neurológico dentro de los 7 días, una disminución del riesgo de recurrencia dentro de los 30 días y una mejora en los resultados funcionales a los 90 días.
En conjunto, esta cadena de evidencia que abarca desde mecanismos hasta la práctica clínica proporciona una clara justificación científica y necesidad clínica para agregar inhibidores de PCSK9 al tratamiento del AIS asociado a ICAS.Este estudio es un ensayo clínico nacional, prospectivo, multicéntrico, doble ciego, aleatorizado y controlado con placebo.
Incluirá a 1.212 pacientes que cumplan los criterios de inclusión y exclusión.
Los pacientes serán asignados aleatoriamente a dos grupos utilizando un método de asignación aleatoria: (1) Grupo experimental: 450 mg de Recaticimab para inyección (3 viales) administrados como una sola inyección subcutánea.
(2) Grupo control: Placebo de Recaticimab para inyección 450 mg (3 viales) administrado como una sola inyección subcutánea.
Cada grupo incluirá 606 pacientes.
Ambos grupos recibirán el tratamiento estándar recomendado por las guías además del fármaco del estudio.
La eficacia a largo plazo se evaluó a través de visitas y evaluaciones de los pacientes a las 0 horas, 24 horas (±2 o ±12 horas), 7 días (±2 días) o al alta, 90 días (±7 días) y 1 año.Las medidas de resultado primarias incluyeron: Análisis basado en el principio de intención de tratar utilizando un modelo ANCOVA para todos los pacientes aleatorizados con HRMRI basal y una puntuación en la Escala de Rankin modificada (mRS) de 0-2 a los 90 días.

Criterios de inclusión:
- Edad ≥30 y ≤80;
- Accidente cerebrovascular isquémico agudo dentro de las 72 horas desde el inicio (determinado por TC/RM junto con síntomas de déficit neurológico)

- Puntuación NIHSS de 4-25 al ingreso
- Los estudios de imagen (CTA/DSA/RM) respaldan la aterosclerosis intracraneal como causa del accidente cerebrovascular, cumpliendo uno de los siguientes criterios: i. El vaso culpable presenta estenosis aterosclerótica intracraneal (ICAS) con un grado de estrechamiento del 50-99%; ii. El vaso culpable presenta oclusión aterosclerótica intracraneal (ICAS-LVO), con recanalización exitosa lograda mediante trombectomía mecánica (grado eTICI inmediato expandido 2b50-3)
- Consentimiento informado firmado
Criterios de exclusión:
- Estenosis arterial intracraneal no aterosclerótica (como disección arterial, enfermedad de Moyamoya, vasculitis sistémica, etc.)
- Cualquier fuente identificable de embolia cardiogénica (como fibrilación auricular, válvulas mecánicas, trombo ventricular izquierdo, foramen oval permeable, etc.)
- Los hallazgos de imagen y la presentación clínica sugieren que la fisiopatología principal de este evento es más consistente con enfermedad de pequeños vasos cerebrales (por ejemplo, oclusión de arteria perforante/infarto lacunar)

Proportion of participants with symptomatic intracranial hemorrhage according to the modified Heidelberg criteria.
The event must include imaging-confirmed intracranial hemorrhage, an increase of at least 4 points in the total NIHSS score compared with baseline or a previously recorded NIHSS score, and neurological deterioration that cannot be explained by a cause other than the intracranial hemorrhage.
Qualifying hemorrhage types include parenchymal hematoma type 1 or 2, remote intracranial hemorrhage, subarachnoid hemorrhage, intraventricular hemorrhage, or subdural hemorrhage.
