Questo studio è uno studio clinico prospettico, multicentrico, in doppio cieco, randomizzato e controllato con placebo, progettato per valutare se la somministrazione precoce di inibitori PCSK9 possa migliorare efficacemente i risultati funzionali a 90 giorni in pazienti con ictus ischemico (AIS) associato a stenosi aterosclerotica intracranica (ICAS), valutati principalmente utilizzando la Scala di Rankin modificata a 90 giorni.

L'ictus ischemico acuto (AIS) rimane una delle principali cause di morte e disabilità a livello mondiale. Sebbene la trombolisi endovenosa e le terapie endovascolari abbiano migliorato significativamente i tassi di successo della riperfusione, tre sfide critiche persistono nella pratica clinica che determinano la prognosi: In primo luogo, solo una proporzione limitata di pazienti può effettivamente ricevere la terapia di riperfusione entro la finestra terapeutica; In secondo luogo, anche con una riperfusione riuscita, possono comunque verificarsi lesioni secondarie come il fallimento della perfusione microcircolatoria, le cascate infiammatorie e la riformazione del trombo. In terzo luogo, le fluttuazioni della fase acuta, in particolare il deterioramento neurologico precoce e il rischio di recidiva precoce, rimangono prominenti, limitando direttamente i miglioramenti negli esiti funzionali. 3 La stenosi aterosclerotica intracranica (ICAS) rappresenta fino al 50% delle eziologie dell'ictus ischemico in Cina. La sua catena patologica—"instabilità della placca-trombosi-compromissione microcircolatoria"—permea sia la fase acuta che quella subacuta, correlandosi strettamente con la recidiva precoce e gli esiti sfavorevoli. La proproteina convertasi subtilisina/kexina di tipo 9 (PCSK9) classicamente promuove la degradazione dei recettori del colesterolo lipoproteico a bassa densità (LDL-C) ed eleva i livelli di LDL-C, guidando così la progressione dell'aterosclerosi. Ancora più importante, ricerche di base e traslazionali in crescita suggeriscono che la PCSK9 potrebbe non funzionare solo come una "proteina del metabolismo lipidico" ma anche partecipare a processi come l'attivazione endoteliale, le cascate infiammatorie, la reattività piastrinica e la disfunzione microcircolatoria. Questo la posiziona come un potenziale hub che collega l'asse "placca-trombo-infiammazione". Di conseguenza, gli inibitori della PCSK9 potrebbero offrire ulteriori benefici neurovascolari protettivi oltre agli effetti ipolipemizzanti durante la fase acuta dell'ictus ischemico acuto (AIS). Le evidenze di base e cliniche esistenti suggeriscono che gli inibitori della PCSK9 possano esercitare un effetto di intervento combinato sui processi patologici chiave che guidano l'aterosclerosi durante la fase acuta dell'AIS attraverso un duplice meccanismo che coinvolge sia vie dipendenti dai lipidi che non dipendenti dai lipidi. Studi meccanicistici rivelano che l'inibizione della PCSK9 modula simultaneamente le risposte infiammatorie, l'attivazione e la disfunzione endoteliale, e le tendenze trombogeniche. Nei modelli di ischemia-riperfusione, dimostra una segnalazione neuroprotettiva mitigando il danno cerebrale e migliorando la funzione neurologica. Clinicamente, ampi studi randomizzati confermano la sua capacità di potenziare rapidamente gli effetti ipolipemizzanti oltre le statine e ridurre il rischio di ictus ischemico. Ulteriori evidenze traslazionali nelle malattie cerebrovascolari indicano che nelle popolazioni con ICAS sintomatica, la terapia ipolipemizzante potenziata con inibitore della PCSK9 più statina riduce il carico di placca, allevia la stenosi e aumenta la stabilità della placca. Nella fase acuta dell'AIS, l'aggiunta precoce di inibitori della PCSK9 si correla con una riduzione del deterioramento neurologico entro 7 giorni, una diminuzione del rischio di recidiva entro 30 giorni e un miglioramento degli esiti funzionali a 90 giorni. Collettivamente, questa catena di evidenze che spazia dai meccanismi alla pratica clinica fornisce una chiara giustificazione scientifica e una necessità clinica per aggiungere inibitori della PCSK9 al trattamento dell'AIS associato a ICAS. Questo studio è una sperimentazione clinica nazionale, prospettica, multicentrica, in doppio cieco, randomizzata, controllata con placebo. Arruolerà 1.212 pazienti che soddisfano i criteri di inclusione ed esclusione. I pazienti saranno assegnati casualmente a due gruppi utilizzando un metodo di allocazione randomizzata: (1) Gruppo sperimentale: 450 mg di Recaticimab per iniezione (3 flaconcini) somministrati come singola iniezione sottocutanea. (2) Gruppo di controllo: Placebo di Recaticimab per iniezione 450 mg (3 flaconcini) somministrato come singola iniezione sottocutanea. Ogni gruppo includerà 606 pazienti. Entrambi i gruppi riceveranno il trattamento standard raccomandato dalle linee guida in aggiunta al farmaco dello studio. L'efficacia a lungo termine è stata valutata attraverso visite ed valutazioni dei pazienti a 0 ore, 24 ore (±2 o ±12 ore), 7 giorni (±2 giorni) o alla dimissione, 90 giorni (±7 giorni) e 1 anno. Le misure di esito primarie includevano: Analisi basata sul principio dell'intenzione di trattare utilizzando un modello ANCOVA per tutti i pazienti randomizzati con HRMRI basale e un punteggio della scala modificata di Rankin (mRS) di 0-2 a 90 giorni. Criteri di inclusione:
- Età ≥30 e ≤80;2.Ictus ischemico acuto entro 72 ore dall'esordio (determinato da TC/RM in combinazione con sintomi di deficit neurologico)
- Ictus ischemico acuto entro 72 ore dall'esordio (diagnosticato mediante TC/RM in combinazione con sintomi di deficit neurologico)
- Punteggio NIHSS di 4-25 al momento del ricovero
- Gli studi di imaging (CTA/DSA/ARM) supportano l'aterosclerosi intracranica come causa dell'ictus, soddisfacendo uno dei seguenti criteri: i. Il vaso responsabile presenta stenosi aterosclerotica intracranica (ICAS) con un grado di restringimento del 50-99%; ii. Il vaso responsabile presenta occlusione aterosclerotica intracranica (ICAS-LVO), con rivascolarizzazione ottenuta con successo mediante trombectomia meccanica (grado immediato di trombosi cerebrale estesa [eTICI] 2b50-3)

Criteri di esclusione:
- Stenosi arteriosa intracranica non aterosclerotica (come dissezione arteriosa, malattia di Moyamoya, vasculite sistemica, ecc.)
- Qualsiasi fonte identificabile di embolia cardiogena (come fibrillazione atriale, valvole meccaniche, trombo ventricolare sinistro, forame ovale pervio, ecc.)
- I risultati di imaging e la presentazione clinica suggeriscono che la fisiopatologia primaria di questo evento è più coerente con la malattia dei piccoli vasi cerebrali (ad esempio, occlusione dell'arteria perforante/infarto lacunare)
- Disabilità preesistente prima di questo evento ischemico (Scala di Rankin modificata ≥ 2 punti)
- I risultati della TC o della RM suggeriscono un infarto cerebrale esteso (ad esempio, punteggio ASPECTS < 6 o volume dell'infarto ≥ 70 ml)
- Ha subito o è programmato per sottoporsi a un intervento di impianto di stent vascolare nei prossimi tre mesi
- Qualsiasi emorragia intracranica verificatasi nei 3 mesi precedenti l'arruolamento;

The event must include imaging-confirmed intracranial hemorrhage, an increase of at least 4 points in the total NIHSS score compared with baseline or a previously recorded NIHSS score, and neurological deterioration that cannot be explained by a cause other than the intracranial hemorrhage.
Qualifying hemorrhage types include parenchymal hematoma type 1 or 2, remote intracranial hemorrhage, subarachnoid hemorrhage, intraventricular hemorrhage, or subdural hemorrhage.
