Cette étude est un essai clinique prospectif, multicentrique, en double aveugle, randomisé et contrôlé par placebo, conçu pour évaluer si l'administration précoce d'inhibiteurs de PCSK9 peut améliorer efficacement les résultats fonctionnels à 90 jours chez les patients ayant subi un accident vasculaire cérébral ischémique (AVCi) associé à une sténose athéroscléreuse intracrânienne (SAIC), principalement évalués à l'aide de l'échelle modifiée de Rankin à 90 jours.

L'accident vasculaire cérébral ischémique aigu (AVCi) demeure l'une des principales causes de décès et d'invalidité dans le monde.
Bien que la thrombolyse intraveineuse et les thérapies endovasculaires aient significativement amélioré les taux de succès de la reperfusion, trois défis critiques persistent en pratique clinique et déterminent le pronostic : Premièrement, seule une proportion limitée de patients peut effectivement recevoir une thérapie de reperfusion dans la fenêtre thérapeutique ; Deuxièmement, même avec une reperfusion réussie, des lésions secondaires telles que l'échec de la perfusion microcirculatoire, les cascades inflammatoires et la reformation du thrombus peuvent encore survenir.
Troisièmement, les fluctuations de la phase aiguë, en particulier la détérioration neurologique précoce et le risque de récidive précoce, restent importantes, limitant directement les améliorations des résultats fonctionnels.
La sténose athérosclérotique intracrânienne (SAIC) représente jusqu'à 50 % des étiologies des accidents vasculaires cérébraux ischémiques en Chine.
Sa chaîne pathologique - "instabilité de la plaque-thrombose-altération microcirculatoire" - imprègne à la fois les phases aiguës et subaiguës, corrélant étroitement avec la récidive précoce et les mauvais résultats.
La proprotéine convertase subtilisine/kexine de type 9 (PCSK9) favorise classiquement la dégradation des récepteurs du cholestérol à lipoprotéines de basse densité (LDL-C) et élève les niveaux de LDL-C, favorisant ainsi la progression de l'athérosclérose.
Plus important encore, des recherches fondamentales et translationnelles croissantes suggèrent que la PCSK9 pourrait non seulement fonctionner comme une "protéine du métabolisme lipidique" mais aussi participer à des processus tels que l'activation endothéliale, les cascades inflammatoires, la réactivité plaquettaire et la dysfonction microcirculatoire.
Cela la positionne comme un hub potentiel reliant l'axe "plaque-thrombus-inflammation".
Par conséquent, les inhibiteurs de PCSK9 pourraient offrir des bénéfices neurovasculaires protecteurs supplémentaires au-delà des effets hypolipidémiants pendant la phase aiguë de l'accident vasculaire cérébral ischémique aigu (AVCi).
Les preuves fondamentales et cliniques existantes suggèrent que les inhibiteurs de PCSK9 pourraient exercer un effet d'intervention combiné sur les processus pathologiques clés conduisant l'athérosclérose pendant la phase aiguë de l'AVCi par un double mécanisme impliquant à la fois des voies dépendantes et non dépendantes des lipides.
Les études mécanistiques révèlent que l'inhibition de PCSK9 module simultanément les réponses inflammatoires, l'activation endothéliale, la dysfonction et les tendances thrombogènes.
Dans les modèles d'ischémie-reperfusion, elle démontre une signalisation neuroprotectrice en atténuant les lésions cérébrales et en améliorant la fonction neurologique.
Cliniquement, de grands essais randomisés confirment sa capacité à améliorer rapidement les effets hypolipidémiants au-delà des statines et à réduire le risque d'accident vasculaire cérébral ischémique.
Des preuves translationnelles supplémentaires dans la maladie cérébrovasculaire indiquent que dans les populations symptomatiques de SAIC, l'inhibiteur de PCSK9 plus la thérapie hypolipidémiante renforcée par statine réduit la charge de plaque, soulage la sténose et augmente la stabilité de la plaque.
Dans la phase aiguë de l'AVCi, l'ajout précoce d'inhibiteurs de PCSK9 est corrélé avec une réduction de la détérioration neurologique dans les 7 jours, une diminution du risque de récidive dans les 30 jours et une amélioration des résultats fonctionnels à 90 jours.
Collectivement, cette chaîne de preuves s'étendant des mécanismes à la pratique clinique fournit une justification scientifique claire et une nécessité clinique pour l'ajout d'inhibiteurs de PCSK9 au traitement de l'AVCi associé à la SAIC.
Cette étude est un essai clinique national, prospectif, multicentrique, en double aveugle, randomisé et contrôlé par placebo.
Elle recrutera 1 212 patients répondant aux critères d'inclusion et d'exclusion.
Les patients seront répartis au hasard en deux groupes selon une méthode d'allocation randomisée : (1) Groupe expérimental : 450 mg de Recaticimab pour injection (3 flacons) administrés en une seule injection sous-cutanée.
(2) Groupe témoin : Placebo de Recaticimab pour injection 450 mg (3 flacons) administrés en une seule injection sous-cutanée.
Chaque groupe comprendra 606 patients.
Les deux groupes recevront un traitement standard recommandé par les directives en plus du médicament de l'étude.
L'efficacité à long terme a été évaluée par des visites et des évaluations des patients à 0 heure, 24 heures (±2 ou ±12 heures), 7 jours (±2 jours) ou à la sortie, 90 jours (±7 jours) et 1 an.
Les mesures de résultat primaires incluaient : Analyse basée sur le principe de l'intention de traiter utilisant un modèle ANCOVA pour tous les patients randomisés avec une HRMRI de base et un score d'échelle de Rankin modifiée (mRS) de 0-2 à 90 jours.
Les mesures de résultat secondaires incluaient : (1) Score mRS de 0-1 à 90 jours (indiquant une bonne fonction du patient) ; (2) Distribution des scores mRS à 90 jours ; (3) Tout accident vasculaire cérébral (incluant ischémique et hémorragique) dans les 90 jours ; (4) Accident vasculaire cérébral ischémique dans les 90 jours ; (5) Événements vasculaires composites (incluant accident vasculaire cérébral, infarctus du myocarde et décès vasculaire) dans les 90 jours ; (6) Score d'échelle européenne de qualité de vie à 5 dimensions et 5 niveaux (EQ-5D-5L) à 90 jours ; (7) Score d'indice de Barthel (BI) à 90 jours.

Critères d'inclusion :
- Âge ≥ 30 et ≤ 80 ; 2. Accident vasculaire cérébral ischémique aigu survenu dans les 72 heures suivant le début (déterminé par TDM/IRM conjointement avec des symptômes de déficit neurologique)
- Accident vasculaire cérébral ischémique aigu survenu dans les 72 heures suivant le début (diagnostiqué par TDM/IRM conjointement avec des symptômes de déficit neurologique)
- Score NIHSS de 4 à 25 à l'admission
- Les études d'imagerie (CTA/DSA/ARM) soutiennent l'athérosclérose intracrânienne comme cause de l'AVC, répondant à l'un des critères suivants : i. Le vaisseau responsable présente une sténose athéroscléreuse intracrânienne (ICAS) avec un degré de rétrécissement de 50 à 99 % ; ii. Le vaisseau responsable présente une occlusion athéroscléreuse intracrânienne (ICAS-LVO), avec une recanalisation réussie réalisée par thrombectomie mécanique (grade eTICI immédiatement élargi 2b50-3)
- Consentement éclairé signé
Critères d'exclusion :
- Sténose artérielle intracrânienne non athéroscléreuse (telle que dissection artérielle, maladie de Moyamoya, vascularite systémique, etc.)
- Toute source identifiable d'embolie cardiogénique (telle que fibrillation auriculaire, valves mécaniques, thrombus ventriculaire gauche, foramen ovale perméable, etc.)
- Les résultats d'imagerie et la présentation clinique suggèrent que la physiopathologie principale de cet événement est plus cohérente avec une maladie des petits vaisseaux cérébraux (par exemple, occlusion de l'artère perforante/infarctus lacunaire)
- Handicap préexistant avant cet événement ischémique (Échelle de Rankin modifiée ≥ 2 points)
- Les résultats de la TDM ou de l'IRM suggèrent un infarctus cérébral étendu (par exemple, score ASPECTS < 6 ou volume d'infarctus ≥ 70 ml)
- A subi ou est programmé pour subir une procédure d'implantation de stent vasculaire dans les trois prochains mois
- Tout saignement intracrânien survenu dans les 3 mois précédant l'inclusion ;
- Tumeurs intracrâniennes, anévrismes cérébraux ou malformations artério-veineuses évalués comme ayant des indications pour un traitement interventionnel
- Tendance hémorragique active sévère ou trouble de la coagulation
- Dysfonctionnement sévère d'organes vitaux tels que le cœur, le foie et les reins
- A reçu un traitement par inhibiteur monoclonal d'anticorps PCSK9 dans le mois précédant l'inclusion ou un traitement par inhibiteur d'ARNsi PCSK9 dans les 6 mois précédant l'inclusion
- Une contre-indication claire aux statines ou des antécédents d'intolérance à celles-ci
- Grossesse, allaitement ou planification d'une grossesse ; 14. Participation actuelle à une autre étude
