Niniejsze badanie to prospektywne, wieloośrodkowe, podwójnie zaślepione, randomizowane, kontrolowane placebo badanie kliniczne, zaprojektowane w celu oceny, czy wczesne podanie inhibitorów PCSK9 może skutecznie poprawić wyniki funkcjonalne po 90 dniach u pacjentów z niedokrwiennym udarem mózgu (AIS) związanym ze zwężeniem wewnątrzczaszkowym miażdżycowym (ICAS), oceniane głównie za pomocą zmodyfikowanej skali Rankina po 90 dniach.

Ostry udar niedokrwienny mózgu (AIS) pozostaje jedną z głównych przyczyn śmierci i niepełnosprawności na świecie.
Chociaż dożylna tromboliza i terapie wewnątrznaczyniowe znacząco poprawiły wskaźniki sukcesu reperfuzji, w praktyce klinicznej utrzymują się trzy kluczowe wyzwania determinujące rokowanie: Po pierwsze, tylko ograniczony odsetek pacjentów może faktycznie otrzymać terapię reperfuzyjną w ramach terapeutycznego okna czasowego; Po drugie, nawet przy udanej reperfuzji, wtórne uszkodzenia, takie jak niewydolność perfuzji mikrokrążenia, kaskady zapalne i ponowne tworzenie się skrzepliny, mogą nadal wystąpić.

Pacjenci zostaną losowo przydzieleni do dwóch grup metodą randomizacji: (1) Grupa eksperymentalna: 450 mg Recaticimabu do wstrzykiwań (3 fiolki) podane jako pojedynczy podskórny wstrzyknięcie.

- Wynik NIHSS w zakresie 4-25 przy przyjęciu
- Badania obrazowe (CTA/DSA/MRA) potwierdzają miażdżycę wewnątrzczaszkową jako przyczynę udaru, spełniając jedno z poniższych kryteriów: i. Naczynie odpowiedzialne wykazuje zwężenie miażdżycowe wewnątrzczaszkowe (ICAS) o stopniu 50-99%; ii. Naczynie odpowiedzialne wykazuje miażdżycową okluzję wewnątrzczaszkową (ICAS-LVO), z udaną rekanalizacją osiągniętą za pomocą mechanicznej trombektomii (natychmiastowy rozszerzony stopień trombolizy w niedokrwieniu mózgu [eTICI] 2b50-3)

Kryteria wykluczenia:
- Niemiażdżycowe zwężenie tętnic wewnątrzczaszkowych (takie jak rozwarstwienie tętnicy, choroba Moyamoya, układowe zapalenie naczyń, itp.)
- Jakiekolwiek identyfikowalne źródło zatorowości sercowo-naczyniowej (takie jak migotanie przedsionków, zastawki mechaniczne, skrzeplina lewej komory, otwarty otwór owalny, itp.)

Health-related quality of life will be assessed using the EQ-5D-5L descriptive system, covering mobility, self-care, usual activities, pain/discomfort, and anxiety/depression.
Overall self-rated health will also be assessed using the EQ visual analogue scale ranging from 0 to 100, with higher scores indicating better perceived health.

Incidence of moderate-to-severe symptomatic intracranial hemorrhage within 72 hours

Proportion of participants with symptomatic intracranial hemorrhage according to the modified Heidelberg criteria.
The event must include imaging-confirmed intracranial hemorrhage, an increase of at least 4 points in the total NIHSS score compared with baseline or a previously recorded NIHSS score, and neurological deterioration that cannot be explained by a cause other than the intracranial hemorrhage.
Qualifying hemorrhage types include parenchymal hematoma type 1 or 2, remote intracranial hemorrhage, subarachnoid hemorrhage, intraventricular hemorrhage, or subdural hemorrhage.
