Este estudo é um ensaio clínico prospetivo, multicêntrico, duplamente-cego, randomizado e controlado por placebo, concebido para avaliar se a administração precoce de inibidores da PCSK9 pode melhorar efetivamente os resultados funcionais aos 90 dias em doentes com acidente vascular cerebral isquémico (AVCI) associado a estenose aterosclerótica intracraniana (EAI), avaliados principalmente através da Escala de Rankin modificada aos 90 dias.

O acidente vascular cerebral isquémico agudo (AVCi) continua a ser uma das principais causas de morte e incapacidade em todo o mundo.
Embora a trombólise intravenosa e as terapias endovasculares tenham melhorado significativamente as taxas de sucesso da reperfusão, persistem três desafios críticos na prática clínica que determinam o prognóstico: Primeiro, apenas uma proporção limitada de doentes pode realmente receber terapia de reperfusão dentro da janela terapêutica; Segundo, mesmo com reperfusão bem-sucedida, lesões secundárias, como falha de perfusão microcirculatória, cascatas inflamatórias e reformação de trombo, ainda podem ocorrer.
Terceiro, as flutuações na fase aguda, particularmente a deterioração neurológica precoce e o risco de recorrência precoce, permanecem proeminentes, limitando diretamente as melhorias nos resultados funcionais.
A estenose aterosclerótica intracraniana (ICAS) representa até 50% das etiologias de acidente vascular cerebral isquémico na China.
A sua cadeia patológica – "instabilidade da placa-trombose-comprometimento microcirculatório" – permeia tanto as fases aguda como subaguda, correlacionando-se intimamente com a recorrência precoce e maus resultados.
A proproteína convertase subtilisina/kexina tipo 9 (PCSK9) classicamente promove a degradação dos recetores de colesterol de lipoproteínas de baixa densidade (LDL-C) e eleva os níveis de LDL-C, impulsionando assim a progressão da aterosclerose.
Mais importante, pesquisas básicas e translacionais crescentes sugerem que a PCSK9 pode não funcionar apenas como uma "proteína do metabolismo lipídico", mas também participar em processos como ativação endotelial, cascatas inflamatórias, reatividade plaquetária e disfunção microcirculatória.
Isso posiciona-a como um potencial centro conectando o eixo "placa-trombo-inflamação".
Consequentemente, os inibidores de PCSK9 podem oferecer benefícios neurovasculares protetores adicionais além dos efeitos de redução de lípidos durante a fase aguda do acidente vascular cerebral isquémico agudo (AVCi).
Evidências básicas e clínicas existentes sugerem que os inibidores de PCSK9 podem exercer um efeito de intervenção combinada nos processos patológicos-chave que impulsionam a aterosclerose durante a fase aguda do AVCi através de um mecanismo duplo envolvendo vias dependentes e não dependentes de lípidos.
Estudos mecanicistas revelam que a inibição da PCSK9 modula simultaneamente respostas inflamatórias, ativação endotelial, disfunção e tendências trombogênicas.
Em modelos de isquemia-reperfusão, demonstra sinalização neuroprotetora ao mitigar lesões cerebrais e melhorar a função neurológica.
Clinicamente, grandes ensaios randomizados confirmam a sua capacidade de melhorar rapidamente os efeitos de redução de lípidos além das estatinas e reduzir o risco de acidente vascular cerebral isquémico.
Evidências translacionais adicionais em doença cerebrovascular indicam que em populações com ICAS sintomática, a terapia de redução de lípidos potenciada por inibidor de PCSK9 mais estatina reduz a carga de placa, alivia a estenose e aumenta a estabilidade da placa.
Na fase aguda do AVCi, a adição precoce de inibidores de PCSK9 correlaciona-se com redução da deterioração neurológica dentro de 7 dias, diminuição do risco de recorrência dentro de 30 dias e melhoria dos resultados funcionais aos 90 dias.
Coletivamente, esta cadeia de evidências abrangendo mecanismos até a prática clínica fornece uma clara justificação científica e necessidade clínica para adicionar inibidores de PCSK9 ao tratamento do AVCi associado a ICAS.
Este estudo é um ensaio clínico nacional, prospetivo, multicêntrico, duplo-cego, randomizado e controlado por placebo.
Incluirá 1.212 doentes que preencham os critérios de inclusão e exclusão.
Os doentes serão aleatoriamente atribuídos a dois grupos usando um método de alocação aleatória: (1) Grupo experimental: 450 mg de Recaticimab para Injeção (3 frascos) administrados como uma única injeção subcutânea.
(2) Grupo de controlo: Placebo de Recaticimab para Injeção 450 mg (3 frascos) administrado como uma única injeção subcutânea.
Cada grupo incluirá 606 doentes.
Ambos os grupos receberão tratamento padrão recomendado pelas diretrizes, além do medicamento do estudo.
A eficácia a longo prazo foi avaliada através de visitas e avaliações dos doentes às 0 horas, 24 horas (±2 ou ±12 horas), 7 dias (±2 dias) ou na alta, 90 dias (±7 dias) e 1 ano.
As medidas de resultado primárias incluíram: Análise baseada no princípio de intenção de tratar usando um modelo ANCOVA para todos os doentes randomizados com HRMRI basal e uma pontuação na Escala de Rankin Modificada (mRS) de 0-2 aos 90 dias.

Critérios de Inclusão:
- Idade >=30 e <=80;2.Acidente vascular cerebral isquémico agudo dentro de 72 horas do início (determinado por TC/RM em conjunto com sintomas de défice neurológico)
- Acidente vascular cerebral isquémico agudo dentro de 72 horas do início (diagnosticado por TC/RM em conjunto com sintomas de défice neurológico)
- Pontuação NIHSS de 4-25 na admissão
- Estudos de imagem (CTA/DSA/ARM) apoiam a aterosclerose intracraniana como causa do AVC, cumprindo um dos seguintes critérios: i. O vaso culpado apresenta estenose aterosclerótica intracraniana (ICAS) com grau de estreitamento de 50-99%; ii. O vaso culpado apresenta oclusão aterosclerótica intracraniana (ICAS-LVO), com recanalização bem-sucedida alcançada via trombectomia mecânica (grau de trombólise expandida imediata em enfarte cerebral [eTICI] 2b50-3)
- Consentimento informado assinado
Critérios de Exclusão:
- Estenose arterial intracraniana não aterosclerótica (como dissecção arterial, doença de Moyamoya, vasculite sistémica, etc.)
- Qualquer fonte identificável de embolia cardiogénica (como fibrilação auricular, válvulas mecânicas, trombo ventricular esquerdo, forame oval patente, etc.)
- Resultados de imagem e apresentação clínica sugerem que a fisiopatologia primária deste evento é mais consistente com doença de pequenos vasos cerebrais (ex., oclusão da artéria perfurante/enfarte lacunar)
- Incapacidade pré-existente antes deste evento isquémico (Escala de Rankin Modificada ≥ 2 pontos)
- Resultados de TC ou RM sugerem enfarte cerebral extenso (ex., pontuação ASPECTS < 6 ou volume de enfarte ≥ 70 ml)
- Submeteu-se ou está agendado para procedimento de implantação de stent vascular nos próximos três meses
- Qualquer hemorragia intracraniana ocorrida nos 3 meses anteriores à inscrição;
- Tumores intracranianos, aneurismas cerebrais ou malformações arteriovenosas avaliados como tendo indicações para tratamento intervencionista
- Tendência hemorrágica ativa grave ou distúrbio de coagulação
- Disfunção grave de órgãos vitais como coração, fígado e rins
- Recebeu terapia inibidora de anticorpo monoclonal PCSK9 dentro de 1 mês antes da inscrição ou terapia inibidora de siRNA PCSK9 dentro de 6 meses antes da inscrição
- Contraindicação clara para estatinas ou histórico de intolerância às mesmas

A serious adverse event is an event that results in death, is life-threatening, requires inpatient hospitalization or prolongation of hospitalization, results in persistent or significant disability, causes a congenital anomaly or birth defect, or is considered an important medical event requiring intervention to prevent a serious outcome.
