背景:
仮想現実(VR)を活用したリハビリテーションは、最近、筋骨格系疾患における患者の関与を高め、リハビリテーションの結果を改善する有望なアプローチとして浮上しています。 しかし、棘上筋腱部分断裂患者におけるその有効性に関するエビデンスは依然として限られています。
目的:
この無作為化比較試験は、棘上筋腱部分断裂患者におけるVR支援リハビリテーションの、疼痛、障害、運動恐怖症、生活の質、および棘上筋腱厚への効果を調査することを目的としました。
方法:

説明
選定基準:18〜65歳、肩の痛みが4週間以上持続し、肩の磁気共鳴画像法で確認された棘上筋腱部分断裂と診断されたこと
除外基準:過去6か月以内に理学療法治療または肩への注射を受けた場合、腱修復手術を受けた場合、コミュニケーションまたは認知の問題がある場合、めまい、前庭障害、またはその他の平衡不安定性を引き起こす状態がある場合、てんかん、発作、光過敏症の病歴がある場合、不安障害や閉所恐怖症を含む診断された精神疾患がある場合、同じ肩に影響を与える追加の整形外科的状態がある場合、妊娠中または授乳中の場合、または機能的能力を制限する可能性のある神経学的、血管、または心臓疾患がある場合

実験的:virtual reality group
Participants received the same physical therapy modalities as the control group, consisting of hot pack, therapeutic ultrasound, and transcutaneous electrical nerve stimulation.
They subsequently performed approximately 30 minutes of immersive virtual reality-based upper-extremity exercise using a Meta Quest Pro headset instead of the conventional therapeutic exercise component.
The virtual reality exercise was therefore a replacement rather than an additional treatment.

The virtual reality activities involved repetitive, task-oriented shoulder and upper-extremity movements within an interactive three-dimensional environment.
The intervention was delivered five days per week for three weeks, for a total of 15 sessions.

After receiving physical therapy modalities consisting of hot pack, therapeutic ultrasound, and transcutaneous electrical nerve stimulation, participants performed approximately 30 minutes of supervised conventional therapeutic shoulder exercise.
The program included scapular mobilization; stretching exercises for shoulder flexion, abduction, internal rotation, and external rotation; and strengthening exercises for the shoulder and scapular stabilizer muscles.
Exercises were performed in three sets of 10 repetitions.
The intervention was delivered five days per week for three weeks, for a total of 15 sessions.

Supraspinatus tendon thickness was measured with the participant seated and the affected arm placed in the modified Crass position.
Using a Logiq V2 portable ultrasound device, the transducer was positioned longitudinally over the supraspinatus tendon, and tendon thickness was measured at the point of maximal thickness and recorded in centimeters.
The measurement procedure was based on the EURO-MUSCULUS shoulder scanning protocol.
