新規容積クロス形状膣ペッサリーの骨盤臓器脱管理における有効性と安全性:前向き多施設共同研究
この前向き多施設研究は、女性の骨盤臓器脱(POP)管理のための新しい体積測定クロス型膣ペッサリーの有効性と安全性を評価しました。
クロス型ペッサリーは、直角に交差する2つの医療用シリコンリング(トーラス)から構成され、脱出した骨盤臓器に対して多方向の解剖学的サポートを提供します。このデバイスは、保持力を向上させ、腟分泌物の排出を促進し、自己管理のための便利なグリップポイントを提供するよう設計されています。
ウクライナの5つの医療センターで、従来の腟ペッサリーを少なくとも12週間使用している症候性POP(ステージ2以上)の18歳以上の女性が登録されました。ベースラインでは、既存のペッサリーを新しいクロス型ペッサリーに交換しました。各参加者は自身の対照として機能し、以前のデバイスを使用した文書化されたベースラインと比較されました。
フォローアップ評価は1、3、6ヶ月後に、骨盤底障害目録(PFDI-20)、骨盤底影響質問票(PFIQ-7)、痛みのビジュアルアナログスケール、Baden-Walker脱出ステージ、患者全体の改善感(PGI-I)を含む検証済みのツールを使用して実施されました。

背景:
骨盤臓器脱(POP)は経産婦の最大50%に影響を及ぼし、生活の質を著しく損ないます。膣ペッサリーは第一選択の保存的治療として推奨されていますが、従来のデザイン(リング型、ゲルホーン型、キューブ型)では12ヵ月後の継続率が60〜80%にとどまります。リング型ペッサリーは、進行した脱出や大きな直腸瘤の場合に不十分な支持となることがよくあります。空間占拠型ペッサリーは優れた解剖学的矯正を提供しますが、膣びらん、分泌物、自己管理の困難など、有害事象の発生率が高くなります。
デバイスの説明:
新しい立体的な十字型尿生殖器用ペッサリーが開発されました(国際特許出願PCT/UA2024/000020;特許WO2025159727A1)。このペッサリーは、共通の対称中心に対して直径方向反対の2点で交差する、直角に配置された2つの交差リング(トーラス)から構成されています。デバイスは医療用シリコーンで製造され、外径は55 mmです。十字形状の形状は多方向の支持を提供し、保持力を向上させ、真空効果を低減して分泌物の排出を改善する可能性があります。
研究デザイン:
前向き、単群、多施設共同研究で、被験者内比較を行いました。ウクライナの16医療機関から19名の婦人科医が参加しました。2024年9月から2025年6月の間に、38〜88歳の有症候性POPの女性51名が連続的に登録されました。ベースライン時、既存の従来のペッサリーを試験用の十字型ペッサリーに交換しました。
追跡調査:1ヵ月後に構造化電話インタビュー、3ヵ月後に対面診察、6ヵ月後に構造化電話インタビューを実施しました。
研究はウクライナの戦時中に実施され、追跡調査に物流上の障壁をもたらした可能性があります。
統計手法:
主要アウトカム(PFDI-20スコアの変化)について、Cohen d ≧ 0.5を仮定し、Wilcoxon符号順位検定の式を用いて両側α=0.05、検出力90%でサンプルサイズを事前に計算しました。必要最小数:46名の評価可能な参加者。登録数:51名(10%の脱落を見込む)。解析はintention-to-treatの原則に従いました。
感度分析では、連鎖方程式による多重代入法(MICE、M=20)を使用しました。

採用基準:

- 現在少なくとも12週間膣ペッサリーを使用している(ドーナツ型、厚いリング型、立方体型、セルクラージュ型、またはボウル型有孔ペッサリー)
- 書面によるインフォームドコンセントを提供できる
- 登録時に活動性の腟感染がない
- 腟長≧8 cm
除外基準:
- 過去のペッサリー治療の失敗
- 腟長<8 cm
- 妊娠中または6ヶ月以内の妊娠予定
- 深部腟びらん、瘻孔、または開放創
- 腟、直腸、または膀胱の腫瘍
- 骨盤感染症または炎症性腸疾患
- 過去12ヶ月以内の骨盤手術
- 試験参加に支障をきたす可能性のある状態

実験的:Cross-Shaped Vaginal Pessary
Participants with pelvic organ prolapse and prior traditional pessary use are fitted with a volumetric cross-shaped vaginal pessary and followed for 6 months.

A medical-grade silicone vaginal pessary consisting of two intersecting closed rings arranged at approximately right angles around a shared center of symmetry, forming a three-dimensional cross-shaped structure intended to provide multi-vector support for pelvic organ prolapse.
The pessary is fitted by a trained gynecologist and participants receive standardized instructions for self-removal, reinsertion, and hygiene.
